Pay $0 out of pocket for covered, in-network services.
All Curative members start with no out-of-pocket costs for covered care and preferred prescriptions. To maintain these benefits, you and any dependents aged 18 and older must complete an annual Baseline Visit within the first 120 days of the plan start. Your Baseline is confidential and won’t impact premiums.
Primary care services
Please note some services listed above are only available during an in-person visit. Curative Primary Care is a physician-owned medical practice and its clinicians exercise their own independent medical judgment. Its administrative operations are managed by an affiliate of Curative Inc.
No Curative Pass required
On the EPO Value plan, you normally need a Curative Pass for care. But not with Curative Primary Care—you can schedule your appointment online and get care right away.
