Fannie Barrier Williams made history as a controversial African American reformer in an era fraught with racial discrimination and injustice. She first came to prominence during the 1893 Columbian Exposition, where her powerful arguments for African American women's rights launched her career as a nationally renowned writer and orator. In her speeches, essays, and articles, Williams incorporated the ideas of Booker T. Washington and W.E.B. DuBois to create an interracial worldview dedicated to social equality and cultural harmony. Williams's writings illuminate (...) the difficulties of African American women in the Progressive Era. She frankly denounced white men's sexual and economic victimization of black women and condemned the complicity of religious and political leaders in the immorality of segregation. Citing the discrimination that crushed the spirits of African American women, Williams called for educational and professional progress for African Americans through the transformation of white society. Committed to aiding and educating Chicago's urban poor, Williams played a central and continuous role in the development of the Frederick Douglass Center, which she called "the black Hull House." An active member of the NAACP and the National Urban League, she fought a long and successful battle to become the first African American admitted to the influential Chicago Women's Club. Her efforts to promote the well-being of African American women brought her into close contact with such influential women as Celia Parker Woolley, Jane Addams, Susan B. Anthony, and Ida B. Wells-Barnett. Accompanied by Deegan's introduction and detailed annotations, Williams's perceptive writings on race relations, women's rights, economic justice, and the role of African American women are as fresh and fascinating today as when they were written. This article gives an account of my recent research in educational science. It comes from the researcher’s own personal experience of chronic disease. It tries to establish a concept capable of rendering the phenomenon first observed in himself: the transformation of chronicity traumatism into a positive experience. This reflection based on personal experience, finds a theoretical echo in George Canguilhem’s concept of norm and pathology. To express the factual experience of chronic disease, that is to say the introspective experience of (...) a remarkable link with health norm, it is also supported by the phenomenological method. Relying on ethnomethodology, a qualitative survey addressed at thirty patients point out, at diverse degrees, the existence of the same tendency of “self-normativity.” This self-normativity allows the patient, by a process of appropriation of illness, to determine for himself a “global health norm.” It also permits a harmonious connection between the subject, his illness, his treatment, and his life in general with all its dimensions. Finally, we have also contemplated the impact of this self-normativity concept on the doctor-patient relationship which reverses the traditional problematic of this link. And also, its implications on therapeutic educational programmes oriented towards the “autonomising” aim induced or reinforced by this concept. (shrink)

RésuméComment percevons nous les gestes d'autrui et où se porte l'attention durant l'émission de ces signes? Nous avons enregistré avec un système eye-tracking les trajectoires oculaires de spectateurs d'une situation dialogique diffusée sur un écran 19 pouces. Les durées et fréquences de fixations montrent que le visage de l'actrice est regardé la plupart du temps tandis que les mouvements gestuels sont plutôt aperçus en périphérie. Le basculement de l'attention focale du visage vers les gestes a lieu lorsque ces derniers démontrent (...) une certaine saillance, nous analyserons ces facteurs de visibilité. Les mouvements des yeux montrent que le pointage manuel et l'orientation sagittale du buste et du regard de l'actrice jouent un rôle de guidage oculaire. Sur des interfaces, ces fonctions signalétiques peuvent permettre à un agent virtuel d'orienter l'attention de l'utilisateur vers une cible. BackgroundThe practice of clinical research is strictly regulated by law. During submission and review processes, compliance of such research with the laws enforced in the country where it was conducted is not always correctly filled in by the authors or verified by the editors. Here, we report a case of a single institution for which one may find hundreds of publications with seemingly relevant ethical concerns, along with 10 months of follow-up through contacts with the editors of these articles. We (...) thus argue for a stricter control of ethical authorization by scientific editors and we call on publishers to cooperate to this end.MethodsWe present an investigation of the ethics and legal aspects of 456 studies published by the IHU-MI (Institut Hospitalo-Universitaire Méditerranée Infection) in Marseille, France.ResultsWe identified a wide range of issues with the stated research authorization and ethics of the published studies with respect to the Institutional Review Board and the approval presented. Among the studies investigated, 248 were conducted with the same ethics approval number, even though the subjects, samples, and countries of investigation were different. Thirty-nine (39) did not even contain a reference to the ethics approval number while they present research on human beings. We thus contacted the journals that published these articles and provide their responses to our concerns. It should be noted that, since our investigation and reporting to journals, PLOS has issued expressions of concerns for several publications we analyze here.ConclusionThis case presents an investigation of the veracity of ethical approval, and more than 10 months of follow-up by independent researchers. We call for stricter control and cooperation in handling of these cases, including editorial requirement to upload ethical approval documents, guidelines from COPE to address such ethical concerns, and transparent editorial policies and timelines to answer such concerns. The modernization of agriculture in Northern countries has led to a loss of crop diversity, as well as a loss of knowledge, know-how and rights of farmers regarding on-farm seed breeding. In France, the Réseau Semences Paysannes (RSP) brings together collectives of actors (farmers, bakers, citizens, gardeners) mobilized in a quest to reclaim these aspects. Within the framework of the decentralized participatory breeding program conducted in collaboration with INRAE, farmers have co-constructed knowledge in terms of dynamic management of heterogeneous wheat (...) populations. Yet, as the RSP network grows, these actors face new types of challenges regarding collective action. To address them, we proposed to consider peasant seeds as « common unknowns » rather than « common goods » and we adapted and applied an innovative design approach drawing upon the KCP (Knowledge-Concept-Proposals) method. This approach consisted in implementing, from 2019 to 2021, five decentralized design workshops with local seed breeding collectives. The replication of design workshops in different contexts provided a framework for interaction among RSP members, allowed the sharing of both scientific and experiential knowledge. Noteworthily, it fostered the generation of many proposals for enhancing collective action, and led to the development of operational proposals with a view to implement them in the future. This methodology-oriented paper mainly describes the decentralized design approach implemented, its outputs and outcomes, as well as its interests and limits in the context of crop diversity management. It also analyses how the workshop outputs enriches previous works in the field; in particular how such a participatory design approach leads to produce original operational solutions to respond to challenges identified in the literature. In vitro fertilisation (IVF) daily practice reveals that couples are willing to take greater risks than doctors if there is a higher chance of pregnancy. Arising from this is a frequently addressed issue regarding the embryo transfer strategy: single or double embryo transfer? The dilemma is faced by patients, as well as physicians, who are caught between the possibility of no pregnancies at all and facing the prospect of iatrogenic twin gestation. How could the couple's preferences concerning how many children (...) they would like be taken into account in determining the number of embryos transferred? How should the physician exercise his medical responsibility towards his patients and the unborn child? An effective approach could be to enhance a couple’s autonomy by advocating a medical beneficence that incorporates patients’ values. This can be achieved through a case-by-case approach in a deliberative process of decision-making which includes real patient-centred communication. (shrink)

A barrier to entailment exists if you can't get conclusions of a certain kind from premises of another. One of the most famous barriers in philosophy is Hume's Law, which says that you can't get normative conclusions from descriptive premises, or in slogan form: you can't get an ought from an is. This barrier is highly controversial, and many famous counterexamples were proposed in the last century. But there are other barriers which function almost as philosophical platitudes: no (...) Universal conclusions from Particular premises, no Future conclusions from premises about the Past, and no claims that attribute Necessity from premises that merely tell us how things happen to be in the Actual world. Barriers to Entailment proposes a unified logical account of five barriers that have played important roles in philosophy, in the process showing how to diagnose proposed counterexamples and arguing that the case for Hume's Law is as strong as that for the platitudinous barriers. -/- The first two parts of the book employ techniques from formal logic, but present them in an accessible way, suitable for any reader with some background in first-order model theory (of the kind that might be taught in a first class in logic). Gillian Russell introduces tense, modal, indexical, and deontic formal logics, but always avoids unneeded complexity. Each barrier is connected to broader philosophical topics: universality, time, necessity, context-sensitivity, and normativity. Russell brings out under-recognised connections between the domains and lays the groundwork for further work at the intersections. -/- The last part of the book transposes the formal work to informal barrier theses in the philosophy of language, in the process doing new work on the concept of logical consequence, and providing new responses to proposed informal counterexamples to Hume's Law which employ hard-to-formalise tools from natural language, such as speech acts and thick normative expressions. Contemporary realities of global population movement increasingly bring to the fore the challenge of quality and equitable health provision across language barriers. While this linguistic challenge is not unique to immigration contexts and is likewise shared by health systems responding to the needs of aboriginal peoples and other historical linguistic minorities, the expanding multilingual landscape of receiving societies renders this challenge even more critical, owing to limited or even non‐existing familiarity of modern and often monolingual health systems with the (...) particular needs of new linguistic minorities. The centrality of language to health beliefs, attitudes, practices, cultural scripts, and conceptual frameworks emphasizes its pivotal role in the healthcare process, and consequently in the adverse effects of treatment that is language‐insensitive and unaware. Such an attitude on the part of medical authorities risks considerable epistemic injustice in the form of a (mis)judgement of patients’ intelligence, credibility, and rationality based on the language that they speak and the manner in which they speak it, consequently impacting the quality and equity of care provided. This danger, I argue, may be effectively countered by fostering among the participants in the healthcare process a sense of epistemic humility through greater metalinguistic awareness. Outlining a range of operative steps that can be used to facilitate this. I argue that the reality of language barriers in the healthcare process, while not entirely eliminable, may nevertheless be successfully addressed, in order to mitigate the challenge of quality and equitable healthcare provision in multilingual societies. This article reflects upon the current state of Equity, Diversity and Inclusion (EDI) on Research Ethics Board (REB) membership in Canada. As post-secondary education institutions strive to increase EDI initiatives across all areas, diversity among the REB membership becomes increasingly critical. Increasing EDI on the REB is complex in the context of colonialism and a discipline that has historically mistreated those from equity-deserving groups. Many barriers to achieving diversity in academia exist that are also reflected in the REB membership. (...) REBs lacking in diversity may struggle to conduct robust ethical reviews, and without full institutional support, increasing diversity in the membership remain a challenge. Diversity amongst community members adds another layer to this complexity with additional barriers such as lack of inclusive recruitment strategies and equitable compensation. Despite community members being central the mandate of the REB, they can be perceived as secondary to affiliated subject matter expert members. This perception de-values the work of the non-affiliated community member, creating conditions of tokenism and power imbalances. Given the unique standing of the REB within the research enterprise, it is well positioned to be a leader in the EDI space. Barriers identified are surmountable and with genuine effort, the REB can champion EDI. It will take full institutional support to enact change and disrupt barriers to EDI for the REB to reach an ideal state of authentic EDI in its membership and processes. Such endeavors can only act to strengthen the ethics review of research and increase research excellence throughout Canada. (shrink)

Background Speaking up to safeguard patients is a crucial ethical and moral obligation for nurses, but it is also a difficult and potentially dangerous component of nursing work. Health advocacy is gaining impetus in the medical literature, despite being hampered by barriers resulting in many nurses in Ghana remaining mute when faced with advocacy-required situations. We explored situations that thwart nurses from performing their health advocacy role. Research question What would cause nurses to take no action when they witness (...) situations that require them to act as health advocates for their clients or communities? Research Design An inductive, descriptive qualitative design was used to collect and analyse data on barriers that prevent nurses from practising their health advocacy role in Ghana. Individual one-on-one in-depth interviews were conducted using a semi-structured interview guide. The data were analysed using qualitative content analysis. Participants and research context Twenty-four professional nurses and midwives registered with the Nursing and Midwifery Council were recruited from three regional hospitals in Ghana. These public hospitals were chosen from the upper, middle and coastal regions. Ethical considerations The UKZN Ethics Review Committee in South Africa and the GHS Ethics Review Committee in Ghana both gave their approval for this study. Findings Intrapersonal barriers, interpersonal barriers, and structural barriers emerged as major obstacles that nurses experience when performing their health advocacy role. Conclusions Barriers to health advocacy have undermined nurses' ability to function as health advocates and are preventing them from utilising their health advocacy position in nursing practise. Giving nursing students positive role models in the classroom and in the clinic can help them become more effective health advocates. (shrink)

The National Health Service (NHS) was the first healthcare system globally to declare ambitions to become net carbon zero. To achieve this, a shift away from metered-dose inhalers which contain powerful greenhouse gases is necessary. Many patients can use dry powder inhalers which do not contain greenhouse gases and are equally effective at managing respiratory disease. This paper discusses the ethical issues that arise as the NHS attempts to mitigate climate change. Two ethical issues that pose a barrier to moving (...) away from metered-dose inhalers are considered: patients who decline an inhaler with a smaller carbon footprint and increased cost. I argue that while a patient is not morally justified in refusing a more environmentally sustainable inhaler due to the expected harms, a doctor may still prescribe a metered-dose inhaler if they believe that switching without consent might undermine trust or substantially worsen the patient’s health. Turning to cost, I argue that the imperative to combat climate change means the NHS should accept small increased financial costs for lower carbon inhalers, even though this provides no additional direct benefit for the patient. I then go on to consider the implications of the preceding analysis for policy and practice. I argue for a policy that minimises the impact of inhalers on the climate by advocating for a principle of environmental prescribing and explore decision-making in practice. While the arguments here pertain primarily to inhalers, the discussion has broader implications for debates around healthcare’s responsibility to be environmentally sustainable. (shrink)

Implication barrier theses deny that one can derive sentences of one type from sentences of another. Hume’s Law is an implication barrier thesis; it denies that one can derive an ‘ought’ (a normative sentence) from an ‘is’ (a descriptive sentence). Though Hume’s Law is controversial, some barrier theses are philosophical platitudes; in his Lectures on Logical Atomism, Bertrand Russell claims: You can never arrive at a general proposition by inference particular propositions alone. You will always have to have at least (...) one general proposition in your premises. (Russell, 1918, p. 206) We will refer to this claim—that one cannot derive general sentences from particular sentences—as Russell’s Law.1 A third barrier thesis claims that one cannot derive sentences about the future from sentences about the past or present. Hume’s endorsement of this barrier thesis is well-known: all inferences from experience suppose, as their foundation, that the future will resemble the past . . . if there be any suspicion that the course of nature may change, and that the past may be no rule for the future, all experience becomes useless, and can give rise to no inference or conclusion. It is impossible, therefore, that any argument from experience can prove this resemblance of the past to the future; since all these arguments are founded on the supposition of that resemblance. (Hume, EHU 4.21/37) We will refer to this barrier thesis as Hume’s Second Law. A fourth barrier thesis says that one cannot derive a necessary sentence from one about the actual world and we will refer to this last thesis Kant’s Law. Achieving the global benefits of artificial intelligence (AI) will require international cooperation on many areas of governance and ethical standards, while allowing for diverse cultural perspectives and priorities. There are many barriers to achieving this at present, including mistrust between cultures, and more practical challenges of coordinating across different locations. This paper focuses particularly on barriers to cooperation between Europe and North America on the one hand and East Asia on the other, as regions which currently have an (...) outsized impact on the development of AI ethics and governance. We suggest that there is reason to be optimistic about achieving greater cross-cultural cooperation on AI ethics and governance. We argue that misunderstandings between cultures and regions play a more important role in undermining cross-cultural trust, relative to fundamental disagreements, than is often supposed. Even where fundamental differences exist, these may not necessarily prevent productive cross-cultural cooperation, for two reasons: (1) cooperation does not require achieving agreement on principles and standards for all areas of AI; and (2) it is sometimes possible to reach agreement on practical issues despite disagreement on more abstract values or principles. We believe that academia has a key role to play in promoting cross-cultural cooperation on AI ethics and governance, by building greater mutual understanding, and clarifying where different forms of agreement will be both necessary and possible. We make a number of recommendations for practical steps and initiatives, including translation and multilingual publication of key documents, researcher exchange programmes, and development of research agendas on cross-cultural topics. In Paradigms and Barriers Howard Margolis offers an innovative interpretation of Thomas S. Kuhn's landmark idea of "paradigm shifts," applying insights from cognitive psychology to the history and philosophy of science. Building upon the arguments in his acclaimed Patterns, Thinking, and Cognition, Margolis suggests that the breaking down of particular habits of mind—of critical "barriers"—is key to understanding the processes through which one model or concept is supplanted by another. Margolis focuses on those revolutionary paradigm shifts— such as (...) the switch from a Ptolemaic to a Copernican worldview—where challenges to entrenched habits of mind are marked by incomprehension or indifference to a new paradigm. Margolis argues that the critical problem for a revolutionary shift in thinking lies in the robustness of the habits of mind that reject the new ideas, relative to the habits of mind that accept the new ideas. Margolis applies his theory to famous cases in the history of science, offering detailed explanations for the transition from Ptolemaic to cosmological astronomy, the emergence of probability, the overthrow of phlogiston, and the emergence of the central role of experiment in the seventeenth century. He in turn uses these historical examples to address larger issues, especially the nature of belief formation and contemporary debates about the nature of science and the evolution of scientific ideas. Howard Margolis is a professor in the Harris Graduate School of Public Policy Studies and in the College at the University of Chicago. Industry over the last ten years has been characterized by a high level of digital transformation, affecting all layers of production and all areas of the economy. One of the key trends in the digitalization of industry is the digital twin – a system that combines a physical object, its digital model, and a continuous link between the two. However, the integration of such a complex technological solution is fraught with a number of barriers that arise in one way (...) or another in the way of technology users. The authors of the article analyzed 100 examples of digital twin use in industry according to 7 criteria, among which the scope of technology application, the most problematic stage of its integration, key technological, organizational, ecosystem and economic barriers, as well as recommendations on overcoming these barriers, which have been applied or are in the process of application, were highlighted. The article contains a layer of statistical data and infographics that confirm the key conclusions about the main barriers to the integration of digital twins, among which the lack of devices and equipment with the necessary capacity, the lack of existing strategies for the technology implementation, the lack of standards for industry regulation and high initial investment are the main ones. Each block of barriers is accompanied by real examples (cases) of use of digital twin in the domestic and foreign industry, among which there are such companies as KamAZ, UEC-Klimov, Tesla, Volvo, Norske Shell and others. In her recent book, Russell (2023) examines various so-called “barriers to entailment,” including Hume’s law, roughly the thesis that an ‘ought’ cannot be derived from an ‘is.’ Hume’s law bears an obvious resemblance to the proscription on fallacies of modality in relevance logic, which has traditionally formally been captured by the so-called Ackermann property. In the context of relevant modal logic, this property might be articulated thus: No conditional whose antecedent is box-free and whose consequent is box-prefixed is valid (...) (for the connection, interpret box deontically). While the deontic significance of Ackermann-like properties has been observed before, Russell’s new book suggests a more broad-scoped formal investigation of the relationship between barrier theses of various kinds and corresponding Ackermann-like properties. In this paper, I undertake such an investigation by elaborating a general relevant bimodal logical framework in which several of the barriers Russell examines can be given formal expression. I then consider various Ackermann-like properties corresponding to these barriers and prove that certain systems satisfy them. Finally, I respond to some objections Russell makes against the use of relevance logic to formulate Hume’s law and related barriers. (shrink)

ABSTRACT Clinical ethics committees have recently been established in nearly all Norwegian hospital trusts. One important task for these committees is clinical ethics consultations. This qualitative study explores significant barriers confronting the ethics committees in providing such consultation services. The interviews with the committees indicate that there is a substantial need for clinical ethics support services and, in general, the committee members expressed a great deal of enthusiasm for the committee work. They also reported, however, that tendencies to evade (...) moral disagreement, conflict, and ‘outsiders’ are common in the hospitals. Sometimes even the committees comply with some of these tendencies. The committees agree that there is a need to improve their routines and procedures, clarify the committees' profile and field of responsibility, to make the committees well‐known, to secure adequate operating conditions, and to develop organizational integration and support. Various strategies to meet these challenges on a local, regional or national level are also explored in this paper. (shrink)

Background The impetus for policies promoting medical data sharing in China has gained significant traction. Nonetheless, the present legal and ethical framework governing the research use of medical data in China, is characterized by a more restrictive rather than permissive approach. The proportion of Chinese medical data being leveraged for scientific research still has room for improvement at present, indicating a significant untapped potential for advancing medical knowledge and improving healthcare outcomes. Building upon this research, we aim to delve deeper (...) into the challenges researchers encounter in the sharing of medical data through focus group interviews. Methods We conducted two focus group interviews study with researchers representing diverse disciplines to explore their perspectives on 21 June 2021 and 28 July 2021. A total of seventeen researchers willingly participated in this study, representing various professional backgrounds. Similar codes were merged. Research team discussions were also utilized to select interviewees’ statements that were regarded as typical or representative. Results The respondents demonstrated a strong understanding that medical data should not be disseminated arbitrarily, recognizing the importance of sharing data in compliance with laws. Through the interview, we found that although respondents stressed the importance of careful consideration regarding if and when this information can be responsibly released, none of the respondents raised the issue of necessitating consent from data subjects for the research use of medical data. This observation sharply contrasts with the stringent separate consent provisions for secondary data use outlined in the PIPL. Conclusions The findings from the focus group studies shed light on researchers’ barriers and ethical challenges towards medical data sharing for scientific research, highlighting their deep concern for data security and cautious approach to sharing. The key objectives aimed at facilitating and enabling the reuse of medical data encompass enhancing interoperability, harmonizing data standards, improving data quality, safeguarding privacy, ensuring informed consent, incentivizing patients, and establishing explicit regulations pertaining to data access and utilization. I argue that the present (if not insuperable) lack of fixed standards in philosophy is associated with three barriers to philosophical progress, pertaining to intra-disciplinary siloing, sociological rather than philosophical determinants of philosophical attention, and the encouraging of bias.

Aims This study aimed to investigate barriers in the observation of professional ethics during clinical care from a nursing viewpoint. Also, it examined the association between these barriers and nurse demographic variables. Methods A descriptive-analytic design was carried out on 207 nurses working in selected hospitals within an urban area of Iran in 2019. Data were collected using a standard questionnaire containing 33 questions that measured barriers to observation of professional ethics. The questionnaire measures three domains of (...) management, environment and individual care. Results In the management dimension, the most important barrier preventing observation of professional ethics was the shortage of personnel. For the environmental dimension, biological changes due to working on the nightshift, and for the individual care dimension, a lack of time were the most important barriers preventing observation of clinical ethics. Findings additionally highlighted a significantly inverse association between barriers to observing professional ethics and the male gender, participating in the ethics workshop and high work-level experiences. Conclusions It is suggested that nursing managers in hospitals correct and remove obstacles with careful planning and emphasis on principles and standards of care including environmental and managerial factors and training professional ethics to nurses. Hospitals in many countries have had clinical ethics committees for over 20 years. Despite this, there has been little research to evaluate these committees and growing evidence that they are underutilized. To address this gap, we investigated the question ‘What are the barriers and facilitators nurses and physicians perceive in consulting their hospital ethics committee?’ Thirty-four nurses, 10 nurse managers and 31 physicians working at four Canadian hospitals were interviewed using a semi-structured interview guide as part of a larger (...) investigation. We used content analysis of the interview data related to barriers and facilitators to use of hospital ethics committees to identify nine categories of barriers and nine categories of facilitators. These categories as well as their subcategories are discussed and those specific to nurses or physicians are identified. The need to increase health professionals' use of clinical ethics committees through reducing barriers and maximizing facilitators is discussed. (shrink)

Topology change is discussed as a barrier penetration process and illustrated by explicit calculations in Witten's Kaluza-Klein bubble geometry. Initial values are given for other bubble geometries, including ones with negative total mass.

The purpose of the considerations is to present and systematise barriers to creating innovation in the Polish economy in 2012–2016. The desk research was based on the results of Community Innovation Surveys (CIS 2012 and CIS 2014), thematic studies of the Central Statistical Office and a report from the Infuture hatalska foresight institute. The use of statistical and comparative analysis in conjunction with the review of the literature on innovation barriers leads to the conclusion that Polish enterprises recognise (...) the lack of financial resources as a major barrier to creating innovation, while non-financial barriers are much less important. The marginal treatment of non-financial barriers is a reversal of attitudes and directions of activities that are undertaken in the field of creating innovation in highly developed countries. Background: Emergency care providers are frequently faces with situations in which they have to make decisions quickly in stressful situations. They face barriers to ethical decision-making and recognizing and finding solutions to these barriers helps them to make ethical decision. Objectives: The purpose of this study was to identify barriers of ethical decision-making in Iranian Emergency Medical Service personnel. Methods: In this qualitative research, the participants (n = 15) were selected using the purposive sampling method, and the (...) data were collected by deep and semi-structured interviews. Finally, the data are analyzed using the content analysis approach. Ethical considerations: Permission to conduct the study was obtained from the Ethics Committee of the Shahid Beheshti University of Medical Sciences. The objectives of the study were explained to the participants and written consent was received from them. Also, participants were assured that necessary measures were taken to protect their anonymity and confidentiality. Findings: The results of the analysis are classified in five main categories. It encompasses the following areas: perception of situation, patient-related factors, input and output imbalance, uncoordinated health system, and paradoxes. Conclusion: Emergency Medical Service personnel make ethical decisions every day. It is important that prehospital personnel know how to manage those decisions properly so that clients’ moral rights are respected. Hence, by identifying the dimensions and obstacles of ethical decision-making in Emergency Medical Service personnel, it is possible to enhance the moral judgment and ethical accountability of the personnel and develop the strategies necessary for ethical decision-making in them. This paper explores the barriers faced by primary schools in England looking to reform their curricula. Drawing on six varied case studies of schools involved in a programme of curriculum innovation support, we examine how Ofsted inspections, assessment pressure, school location, lack of time, and staffing concerns all limit schools’ ability to revise their curricula. We argue that, even with dedicated time and support, schools found it challenging to instigate and maintain curriculum reform due to the policy terrain they (...) operate within. Our analysis is informed by the different dimensions of context identified in policy enactment literature. We find that school leaders’ autonomy to revise and update their school curricula can be limited by various professional, material, situated and external dimensions of context, which they often are unable to change. In taking this approach, and viewing curriculum reform as a process of policy enactment, the paper further affirms the importance of context in understanding how curriculum policy operates in schools. It also demonstrates the significance of context in understanding how policy affects schools’ priorities and strategies for improvement. (shrink)

This paper is Part II of a trilogy derived from the works on the ICB ( ’The Infinite Choice Barrier: A Structural Limit of Algorithmic Cognition’ , 'AGI is Impossible'(Schlereth, 2025)), expanding upon specific sub-domains of the original proof. -/- Building upon the conceptual diagnosis presented in Part I (“The Canary in the Algorith- mic Coal Mine”), this paper provides the rigorous mathematical proof of the Infinite Choice Barrier. (ICB)1 We demonstrate that the limitations of algorithmic cognition described in the (...) prelude are not merely engineering bottlenecks, but formal inevitabilities derived from a triangulation of three fundamental mathematical domains. Specifically, we prove: (1) via Computability Theory (Rice’s Theorem), that semantic frame adequacy is undecidable from within a system; (2) via Information Theory, that entropy diverges in heavy- tailed decision spaces (α≤1), rendering probabilistic inference structurally unstable; and (3) via Algorithmic Complexity (Chaitin’s Incompleteness), that frame-transcendent insights are algorithmically unrecognizable. Furthermore, weunifytheseresultsthroughacategoricalproofusingSheafTheory, demon- strating that “Gluing Failures” between consistent local semantic sections are mathematically necessary in irreducibly infinite decision spaces. In spite of its success, Neo-Darwinism is faced with major conceptual barriers to further progress, deriving directly from its metaphysical foundations. Most importantly, neo-Darwinism fails to recognize a fundamental cause of evolutionary change, “niche construction”. This failure restricts the generality of evolutionary theory, and introduces inaccuracies. It also hinders the integration of evolutionary biology with neighbouring disciplines, including ecosystem ecology, developmental biology, and the human sciences. Ecology is forced to become a divided discipline, developmental biology is stubbornly difficult to (...) reconcile with evolutionary theory, and the majority of biologists and social scientists are still unhappy with evolutionary accounts of human behaviour. The incorporation of niche construction as both a cause and a product of evolution removes these disciplinary boundaries while greatly generalizing the explanatory power of evolutionary theory. Barriers to mental health interventions globally remain a health concern; however, these are more prominent in low- and middle-income countries. The barriers to accessibility include stigmatization, financial strain, acceptability, poor awareness, and sociocultural and religious influences. Exploring the barriers to the utilization of mental health services might contribute to mitigating them. Hence, this research aims to investigate these barriers to mental health service utilization in depth at the Kabutare District Hospital of the Southern Province of Rwanda. (...) The qualitative approach was adopted with a cross-sectional study design. The participants were patients with mental illnesses seeking mental health services at the hospital. Ten interviews were conducted in the local language, recorded, and transcribed verbatim and translated by the researchers. Thematic analysis was applied to analyze the data collected. The results revealed that the most common barriers are fear of stigmatization, lack of awareness of mental health services, sociocultural scarcity, scarcity of financial support, and lack of geographical accessibility, which limit the patients to utilize mental health services. Furthermore, it was revealed that rural gossip networks and social visibility within the communities compounded the stigma and social exclusion for patients with mental health conditions. Stigmatization should be reduced among the community members for increasing their empathy. Then, the awareness of mental disorders needs to be improved. Further research in Rwanda on the factors associated with low compliance to mental health services with greater focus on the community level is recommended. (shrink)

This paper examines pain and pain relief in the Caribbean, where pain is widely perceived as an unavoidable part of life, and where unnecessary suffering results from untreated and under treated pain. Barriers to pain relief in the Caribbean include patient and family attitudes, inadequate knowledge among health professionals and unduly restrictive regulations on the medical use of opioids. Similar barriers exist all over the world. This paper urges medical, nursing and public health professionals, and educators to examine (...) attitudes towards pain and pain relief and to work towards making effective pain relief and palliation more accessible. It recommends that i) health professionals and officials be better educated about pain, palliation and opioids, ii) regulatory restrictions be updated in light of clinical and scientific evidence, iii) opioid procurement policies be adjusted to facilitate increased medical use, iv) medical charts and records be modified to routinely elicit and document patients levels of pain, and v) educational campaigns be developed to inform the public that moderate and severe pain can be safely relieved at the end of life and other stages of life. The professional, respectful, and beneficent response to patients in pain is to provide rapid and aggressive pain relief or to urgently consult a pain or palliative specialist. When a health system hinders such efforts the ethical response is to identify, facilitate and advocate for overcoming barriers to improvement. (shrink)

The ethical recruitment of participants with neurological disorders in clinical research requires obtaining initial and ongoing informed consent. The purpose of this study is to characterize barriers faced by research personnel in obtaining informed consent from research participants with neurological disorders and to identify strategies applied by researchers to overcome those barriers. This study was designed as a web-based survey of US researchers with an optional follow-up interview. A subset of participants who completed the survey were selected using (...) a stratified purposeful sampling strategy and invited to participate in an in-depth qualitative interview by phone or video conference. Data were analyzed using a mixed methods approach, including content analysis of survey responses and thematic analysis of interview responses. Over 1 year, 113 survey responses were received from US research personnel directly involved in obtaining informed consent from participants in neurological research. Frequently identified barriers to informed consent included: cognitive and communication impairments (e.g. aphasia), unrealistic expectations of research participants, mistrust of medical research, time constraints, literacy barriers, lack of available social support, and practical or resource-related constraints. Strategies to enhance informed consent included: involving close others to support participant understanding of study-related information, collaborating with more experienced research personnel to facilitate training in obtaining informed consent, encouraging participants to review consent forms in advance of consent discussions, and using printed materials and visual references. Beyond conveying study-related information, researchers included in this study endorsed ethical responsibilities to support deliberation necessary to informed consent in the context of misconceptions about research, unrealistic expectations, limited understanding, mistrust, and/or pressure from close others. Findings highlight the importance of training researchers involved in obtaining informed consent in neurological research to address disease-specific challenges and to support the decision-making processes of potential research participants and their close others. International schools increasingly admit marginalized and indigenous students through scholarship or charity programs intended to promote educational equity. However, access to international education does not automatically translate into successful learning outcomes. Many students entering such environments possess profound gaps in foundational literacy, numeracy, and language skills, which may be compounded by behavioral, environmental, and technological factors. This conceptual paper examines the barriers faced by indigenous charity-sponsored students enrolled in English-medium international schools, focusing on four interacting domains: educational poverty, foundational (...) literacy deficits, attention and behavioral regulation difficulties, and digital overexposure. Drawing from classroom observations and scholarship on experiential learning, contextualized instruction, and learner readiness, the paper proposes a conceptual framework explaining how these interacting conditions impede English language acquisition and broader academic success. The study argues that international schools must move beyond mere access and develop structured bridging programs, glocalized learning materials, and controlled digital environments to address extreme learning gaps among marginalized learners. Using the transition from Newtonian to Einsteinian physics as an existence proof, we demonstrate that semantic innovation requires a primitive that invalidates the system’s own evaluation metric–a move no algorithm can execute without external intervention. The barrier is not empirical, but topological: an algorithmic map cannot act as the territory. We conclude that the future of intelligence can impossibly be the automation of judgment (AGI). The wall that this endeavor would have to take is iterally infinitely high (and wide), and invisible on top of that. However, there probably is a way to work with this restraint, but only when truly understanding what this barrier is. There are barriers that translation must scale through to be effective, some of which are social and cultural. Social and cultural differences exist between and amongst nations. In most cases too, we find a country having several social/cultural diversities. These differences impede, in one way or the other, effective translation because of varying conceptions of reality occasioned by languages. By mid-2019 there were more than 80 AI ethics guides available in the public domain. Despite this, 2020 saw numerous news stories break related to ethically questionable uses of AI. In part, this is because AI ethics theory remains highly abstract, and of limited practical applicability to those actually responsible for designing algorithms and AI systems. Our previous research sought to start closing this gap between the ‘what’ and the ‘how’ of AI ethics through the creation of a searchable typology (...) of tools and methods designed to translate between the five most common AI ethics principles and implementable design practices. Whilst a useful starting point, that research rested on the assumption that _all_ AI practitioners are aware of the ethical implications of AI, understand their importance, and are actively seeking to respond to them. In reality, it is unclear whether this is the case. It is this limitation that we seek to overcome here by conducting a mixed-methods qualitative analysis to answer the following four questions: what do AI practitioners understand about the need to translate ethical principles into practice? What motivates AI practitioners to embed ethical principles into design practices? What barriers do AI practitioners face when attempting to translate ethical principles into practice? Background Coping with end-of-life issues is a major challenge for governments and health systems. Despite progress in legislation, many barriers exist to its full implementation. This study is aimed at identifying these end-of-life barriers in relation to Israel. Methods Qualitative in-depth interviews using professionals and decision makers in the health-care and related systems were carried out, along with two focus groups based on brainstorming techniques consisting of nurses and social workers. Data was managed and analyzed using Naralyzer software. (...) Results Qualitative analysis showed identification of six primary barriers: 1) law, procedures, and forms; 2) clinical aspects; 3) human aspects; 4) knowledge and skills of medical teams; 5) communication; and 6) resource allocation. These were further divided into 44 sub area barriers. Conclusions This study highlights the role of the family doctor in end-of-life by training physicians in decision-making workshops and increasing their knowledge in the field of palliative medicine. Effectively channeling resources, knowledge, and support for medical teams, by accounting for the structure and response of the units for home treatment will improve patient’s access to information on and support for end-of-life laws, as well as reduce legislative barriers in other countries that face the same issues. Purpose This study aims to examine the psychological barriers to Metaverse-based education, focusing on the impact of technophobia and digital fatigue constructs. While the metaverse offers innovative educational experiences, these psychological factors may hinder its adoption, necessitating a deeper understanding of their influence. Design/methodology/approach This study adopted a quantitative research approach utilizing a survey methodology to examine how technophobia and digital fatigue shape students’ perceptions of metaverse-based education. Data were gathered from university students with prior experience with virtual learning (...) platforms. Structural equation modeling (SEM) was conducted using SmartPLS 4.0 to evaluate the relationships among key variables, ensuring a robust analysis of the proposed model. Findings The findings reveal that technophobia impacts students’ perceptions of ease of use, while digital fatigue adversely affects their perceived usefulness. Furthermore, perceived ease of use improves perceived usefulness, and both factors play a crucial role in shaping students’ intention to adopt metaverse-based education. These results underscore the significant influence of psychological barriers on technology acceptance within digital learning environments. Originality/value This research expands the discussion on metaverse adoption in education by incorporating psychological factors into existing technology acceptance frameworks. Identifying critical psychological barriers offers valuable insights for educators, policymakers and platform developers to enhance accessibility, reduce digital fatigue and optimize the usability of metaverse-based learning environments. Background Since dignity is one of the fundamental rights of each patient, maintaining patients’ dignity is essential. Unfortunately, in many cases, particularly among patients with schizophrenia (SCZ), dignity is not fully respected. Nonetheless, there is limited knowledge regarding this matter in Middle Eastern Nations. Research Objective This study aimed to identify the barriers to maintaining dignity for patients with schizophrenia from the perspective of patients with schizophrenia, their family caregivers, and healthcare personnel. Research Design This qualitative study was conducted (...) with patients ( n = 16), family caregivers ( n = 4), and healthcare personnel ( n = 6) who were selected using a purposeful sampling method. The data were collected through semi-structured, in-depth face-to-face interviews until the data reached saturation. Data were analyzed using the Graneheim and Lundman method. Ethical Considerations The Research Ethics Committee of Urmia University of Medical Sciences (IR.UMSU.REC.1401.099). Findings Analysis of the interviews revealed two main themes and five subthemes as the barriers to maintaining dignity for patients with schizophrenia. The main themes included “Black shadow of stigma” (three subthemes: “Facing the fear and discrimination of others,” “Misconceptions in society,” and “Social rejection”) and “Facing violations” (two subthemes: “Basic human rights violation” and “Negative emotional responses”). Conclusion Patients’ dignity with schizophrenia was at a low level due to the negative attitude of society toward these patients. In this respect, the basic human rights violations, social rejection, and misconceptions were abundant in society and care centers. An in-depth comprehension of the threatening factors affecting dignity can assist policymakers in safeguarding their dignity through effective programs. (shrink)

Cette analyse a pour objet d’explorer des études scientifiques récentes afin d’aboutir à une synthèse des conceptions académiques sur la participation dans la société des personnes qui présentent une incapacité. Nous avons exploré six bases de données pertinentes relevant du domaine des sciences sociales en utilisant comme termes de recherche “societal participation of people with disabilities/participation dans la société des personnes handicapées” sous différentes combinaisons. Les critères d’inclusion étaient les suivants: personnes handicapées en âge de travailler, participation dans la société, (...) obstacles et/ou facilitateurs à la participation, zone limitée géographiquement à l’Europe ou liée à l’Europe, études quantitatives ou qualitatives évaluées par des pairs et publiées en anglais entre janvier 2012 et décembre 2013. Trente-deux recherches ont répondu à ces critères d’inclusion. Chacune de ces recherches a fait l’objet d’une analyse selon les quatre paramètres suivants: identité des participants du groupe, type de la participation, type du facilitateur et type de l’obstacle. Les résultats montrent que les études s’intéressent tout particulièrement à la participation sur le marché du travail et qu’elles portent principalement sur les personnes handicapées en général et non sur un groupe particulier. Les principaux obstacles identifiés concernent les aspects financiers, les attitudes, les problèmes liés à la santé et le chômage. Les facilitateurs les plus fréquemment identifiés relèvent de la législation et des politiques en faveur des personnes handicapées, du soutien de l’entourage, des attitudes dans la société et des possibilités d’emploi des personnes handicapées. Disparities in research publications persist between low- and middle-income countries (LMICs) and high-income countries, with Arab researchers from LMICs in the Middle East facing significant barriers including the potential impact of linguistic biases. These disparities are reflected in the lower acceptance rates and underrepresentation in high-impact journals. This study aimed to quantitatively identify the specific shortcomings of manuscript submission that contribute to publication inequities. We categorized reviewers’ critiques into predefined areas of manuscript deficiencies, such as lack of novelty, methodological (...) flaws, poor language quality, and misalignment with journal scope, and rated each item as significant (1), moderate (2), or minor (3). Among the 60 letters analyzed, the most common deficiencies were a lack of novelty (51.7%), flawed methodology (40.0%), misalignment with the journal’s scope (22.3%), and poor English language quality (18.3%). These results highlight the key areas for improvement: identifying novel research questions, using sound methodologies, choosing appropriate journals, and strengthening scientific writing skills. These challenges might disproportionately affect researchers of LMICs, reinforcing global disparities in research visibility. To address these disparities, institutions in LMICs must provide researchers with robust training on research methods and academic writing. Simultaneously, journals have an ethical responsibility to ensure an equitable and unbiased evaluation of submissions across diverse global contexts. These combined efforts are essential to fostering a more inclusive and representative academic publishing landscape. (shrink)

This paper explores gender barriers to the formation of the female mentor – male protégé relationship. The authors consider both physiological as well as social gender as a way to help understand the scarcity of these relationships. A number of gender-related factors are considered, including organizational demographics, relational demography, sexual liaisons, gender stereotypes, gender behaviors, and power dynamics. In this paper I argue that by using methods of encouragement, derived from the fields of social psychology, cognitive neuroscience, and behavioral economics, healthcare workers can potentially provide their patients with tools for increasing adherence to their treatment plans. I claim that the shared decision-making model can, and should, be enriched to include a component that encourages patients to follow through with their plans. It is commonsense that it is one thing to decide on a plan, and quite another to (...) stick to it. Even if a plan is one’s own, people often backslide with respect to their prior commitments. I appeal to the extensive literatures on decision-making, delay discounting, and willpower to provide some empirically verified tools for motivating patients. Importantly, I argue that contrary to appearances, motivating others to act with respect to their commitments expresses a respect for autonomy and is non-paternalistic. (shrink)

The aim of this article is to describe the general conditions for the development of creative industries in Podlaskie Voivodship from Poland. This region on the background of the country is characterized by the highest level of cultural diversity and multiculturalism policy. However, there are a number of barriers for the creative industries.
