This is a very good article that challenges the in vitro fertilization for all approaches for patients with severe deep infiltrative endometriosis. It challenges us to again look at whether surgery should be considered prior to IVF. Obviously, randomized clinical trials are needed but may be very difficult to perform.

At the last World Congress on Endometriosis (online, in 2021), we presented our own results on IVF in endometriosis patients and noted that our pregnancy rates were similar to these of patients with other diagnoses. The secret: they all had previous surgeries. This interesting meta-analysis by Carmona, Abrao and colleagues shows why it is crucial to practice surgery in deep infiltrating endometriosis (DIE) before IVF. It was nice to read, with good evidence.
