Who May Apply?
You may book an online appointment if you are:
  1. The document owner, or
  2. An authorized representative
Requirements for Authorized Representatives
The authorized representative must bring the following:
  1. Signed authorization letter
    Note for Minor Document Owners:
    • A Special Power of Attorney from the minor’s parent/s is required (original).
    • If either parent of the minor is abroad, the Special Power of Attorney must be notarized by the Philippine Embassy or Consulate General.
    • Proof of kinship of the parent to the minor document owner must also be presented.
  2. Copy of the document owner’s valid government-issued ID (must bear the document owner’s signature)
  3. Representative’s valid ID (photocopy and original)
  4. Proof of Affiliation/Kinship (if applicable)
    • Company representatives: Proof of affiliation, such as SEC GIS, company ID indicating position, or similar documents.
    • Death Certificate processing: Proof of relationship from the next-of-kin.
    • Foreign nationals processing any employment-related document: Alien Employment Permit from DOLE and the Alien Certificate of Registration from the Bureau of Immigration.
List of Acceptable IDs:
  1. All Digitized Philippine Government-Issued IDs bearing the signature of the applicant (SSS, UMID, Driver’s License, PRC, Postal ID, PhilSys ID, etc.)
  2. Other Acceptable IDs:
    • Senior Citizen’s ID
    • Solo Parent ID
    • PWD ID
    • Voter’s ID
    • Voter’s Certificate issued by the COMELEC Main Office in Intramuros, Manila
    • Passport
    • Seaman’s Book
    • Alumni ID (1 year upon graduation)
    • Student ID (1 year upon graduation)
Note: All IDs presented should be readable, untampered, and contain consistent information with the documents presented upon application.
Procedures for Application
  1. PRINT and present your confirmed appointment letter at the Appointment Verification Window Counter.
    Note: Editing, revising or altering the appointment letter is considered falsification of public document/s and is punishable by law.
  2. Prepare the necessary documents before you proceed to the processing window.
  3. Present your valid government-issued ID (original and photocopy) together with the documents to the processor for assessment.
  4. Pay the corresponding fees to the Cashier.
