Survey - Because I really should do this.
Body and Appearance
1. Describe the character's height and build. Is he heavyset, thin, short, rangy?
Tall, scrawny type. Can generally be described as "rabbity", minus the whole...buck toothed thing.
2. How old is he?
22
3. Describe his posture. Does he carry himself well or does he slouch?
Given his upbringing he generally carries himself fairly well, although he does have a tendency to lounge, mostly on furniture.
4. How is his health? Is he fit or out of shape? Any illnesses or conditions? Any physical disabilities?
While he tends to at least /look/ sickly and in poor health more often than he doesn't, he's actually fairly healthy most of the time. He's just one of those people.
5. How does he move? Is he clumsy, graceful, tense, fluid?
Fluid. He's reasonably comfortable in his own skin, and is actually significantly less awkward than his general scrawny, gangly appearance would imply. Again, it's the Pureblood thing.
6. How attractive is this character physically? How does he perceive himself in the mirror?
Depends on your taste; if you go in for the scrawny, unconventional ones you'd probably find him reasonably attractive, but if your tastes run more towards the pretty or built probably not so much. Personally, he finds himself neither exceptionally attractive nor exceedingly ugly. He just is; he doesn't much care for the whole preening, showy, absurdity that is modern courtship, so he tends not to pay much attention to how he looks aside from hygiene.
7. Describe his complexion. Dark, light, clear, scarred?
Pale, and fortunately he didn't have to deal with many acne problems during his teenage years. Theo does however have a fairly visible scar on his left cheek from a broomstick accident when he was nine; he fell into a rose bush from six feet in the air, which just so happened to be sheltering a baby chamrosh which...wasn't particularly pleased to be landed on. He walked away with a few deep gashes and a sprained wrist, not to mention an intense dislike of flying, and is fortunate that's all he got.
8. Describe his hair: color, texture, style.
Dark, fairly average as hair goes. Generally short and on the messy side, although stylishly so.
9. What color are his eyes?
Grey-blue.
10. Does the character have any other noteworthy features?
Not really, aside from maybe the fact that he never seems to go anywhere without a book. Just in case.
11. What are his chief tension centers?
Shoulders, generally, although he tends not to get particularly tense. Getting tense would indicate actually /caring/.
12. What is the character's wardrobe like? Casual, dressy, utilitarian? Bright colors, pastels, neutrals? Is it varied, or does he have six of the same suit?
He mostly goes for functional and comfortable, although again, Pureblood upbringing would necessitate that it's also presentable. It's mostly neutrals and while not all the same there's a fair amount of similarity.
13. Do his clothes fit well? Does he seem comfortable in them?
Again, the mores of his social class would in fact indicate his clothes need to fit well. He doesn't necessarily get everything /tailored/, but it all at least fits him. And he's pretty comfortable in them; he tends not to wear clothing he's not comfortable in.
14. Does he dress the same on the job as he does in his free time? If not, what are the differences?
Yep. Technically he probably shouldn't, since it's a ministry job and all, but he quite frankly doesn't care.
15. You knew it was coming: Boxers, briefs or commando? ...If you're a girl, panties, thong or commando?
Boxers.

1. What does this character's voice sound like? High-pitched, deep, hoarse?
Middle of the road, bordering on deep-ish.

Quiet, even, and carefully enunciated. He likes it when people can actually understand him when he mocks them horribly. And he seldom hesitates.
3. Does the character have a distinct accent or dialect? Any individual quirks of pronunciation? Any, like, you know, verbal tics?
Aside from British? Nothing special, although he does tend to be more of an ass the more uncomfortable he is.
4. What language/s does he speak, and with how much fluency?
He's completely fluent in English, and is passable in French, although he doesn't use it often.
5. Does he switch languages or dialects in certain situations?
Nope.
6. Is he a good impromptu speaker, or does he have to think about his words?
He's pretty good on his feet, even if he doesn't actually know what he's talking about.
7. Is he eloquent or inarticulate? Under what circumstances might this change?
Eloquent. He would need to be under quite a bit of stress to change that.
Mental and Emotional
1. How intelligent is this character? Is he book-smart or street-smart?
Very. A little of both, being both an excellent student and a Slytherin. For comparison, he's said to be more clever than Draco.
2. Does he think on his feet, or does he need time to deliberate?
He thinks pretty well on his feet, although if you give him time to deliberate he thinks even better.
3. Describe the character's thought process. Is he more logical, or more intuitive? Idealistic or practical?
Logical and practical. He doesn't have much patience for idealism, and while he's fairly intuitive he tends to rely more upon logic in most situations.
4. What kind of education has the character had?
Completed all seven years of school at Hogwarts; knowledge is power, after all.
5. What are his areas of expertise? What, if anything, is he interested in learning more about?
Potions, Ancient Runes, and Transfiguration tend to be his best areas; he's kind of weak in charms.
6. Is he an introvert or an extrovert?
Introvert
7. Describe the character's temperament. Is he even-tempered or does he have mood swings? Cheerful or melancholy? Laid-back or driven?
Theo generally has little patience for idiots, and won't hesitate to belittle someone if he thinks they're being one. Overall he's apathetic towards most people and things, which makes him more even-tempered than not simply because he doesn't care enough one way or the other. He was driven academically only because it gave him an excuse not to talk to people.
8. How does he respond to new people or situations? Is he suspicious, relaxed, timid, enthusiastic?
"Calculated indifference" tends to be the best way to describe the way Theo responds to most things. He doesn't trust easily, or make friends easily simply because he dislikes people in general and can't be bothered.

Act. If he feels like it.
10. Which is his default: fight or flight?
Fight if he needs to, remain neutral if he can.
11. Describe the character's sense of humor. Does he appreciate jokes? Puns? Gallows humor? Bathroom humor? Pranks?
He has a very dry sense of humour, leaning towards the morbid. If it's clever he can appreciate it, but anything slapstick or base (including bathroom humour and pranks) he's more likely to sneer at than laugh about.
12. Does the character have any diagnosable mental disorders? If yes, how does he deal with them?
I'm sure you could go through a variety of disorders he might have based on personality, but nothing diagnosed.
13. What moments in this character's life have defined him as a person?
His mother's death. His father's being sentenced to life in Azkaban.
14. What does he fear?
Dementors. Mostly because when the Ministry came to take his father away he was overcome with a sudden fear that they would take him too simply for being his son. It's irrational, he knows this, which is why it's something which he will take to his grave.
15. What are his hopes or aspirations?
He has none, really. Not right now, anyway.
16. What is something he doesn't want anyone to find out about him?
That he was almost (almost) tempted to throw in with Draco's lot during his first year at Hogwarts. It seemed nice, being surrounded by so many adoring classmates. ...But then he realised that half of them secretly hated Malfoy behind his back, and three quarters of them were only his friend because of who he was, much like most of the people who wanted to be friends with Harry Potter. He was disgusted by the whole thing, and vowed from that day on to stay out of it because he wanted no part of it.
It took him three years before he stopped hating Malfoy himself, and only because he realised Malfoy found the whole thing just as asinine as he did. They've been almost-friends ever since.
Relationships
1. Describe this character's relationship with his parents.
Virtually nonexistant. His mother died when he was very young, so he didn't get to know her very well, and his father, while fairly good to him for most of his childhood (he was there, at least, and taught him valuable lessons such as to think for himself and never go with the crowd simply because it's popular opinion), is essentially deceased as well.
2. Does the character have any siblings? What is/was their relationship like?
No
3. Are there other blood relatives to whom he is close? Are there ones he can't stand?

No
5. Who is/was the character's best friend? How did they meet?

6. Does he have other close friends?

1. Do you know your character's astrological (zodiac of choice) sign? How well does he fit type?
Taurus. Not very; he tends to prefer when things are on an even keel and likes being in control (which is Taurus-like), but he tends to avoid doing the "popular" thing just because it's the status quo (which isn't Taurus-like). Although he does keep to his own social class for the most part (which is Taurus-like).
2. Is this character religious, spiritual, both, or neither? How important are these elements in his life?

The usual Pureblood ones; Purebloods are inherently superiour, Mudbloods and Muggles are inherently lesser. Although he holds them more as a matter of course and breeding than because he truly /believes/ them.

2. What is his social status? Has this changed over time, and if so, how has the change affected him?
He's a Pureblood, and works for the Ministry to boot, and as such enjoys reasonably high social status. Reading, mostly. Anything and everything.
7. What are his eating habits? Does he skip meals, eat out, drink alcohol, avoid certain foods?

It's entirely too much effort to try to sort out only the good ones, and more often than not vomit tends to find its way into the mix regardless of how much you think you've avoided it.
