In recent years, the growing demands of an aging population and market forces have led to an increase in nursing staff, including nurses and nursing aides. This expansion necessitates the surveillance of nursing activities to ensure the quality and efficiency of care. However, there is still a limited understanding of surveillance dynamics in long‐term care (LTC) and their implications. This qualitative study explored the surveillance dynamics affecting nurses and nurse aides in LTC facilities. The research used semi‐structured interviews to (...) describe how surveillance affected LTC work and care quality. In total, 14 registered nurses and 18 nurse aides were interviewed. Analysis indicated three themes. Vertical surveillance involves oversight by higher management and families, as well as patient complaints and camera recordings. Horizontal surveillance includes peer monitoring and gossip, creating an informal oversight system. Finally, patients' body surveillance consists of monitoring patient health and hygiene, and nurse aides' compliance with regulations. The study found that these surveillance practices were described as only partly effective. Although they aimed to improve safety and compliance, they often resulted in heightened stress, diminished autonomy, and lower care standards. The results and implications are discussed. (shrink)

Using theoretical analysis of self-consciousness concept and experimental evidence on the brain default mode network (DMN) that constitutes the neural signature of self-referential processes, we hypothesized that the anterior and posterior subnets comprising the DMN should show differences in their integrity as a function of meditation training. Functional connectivity within DMN and its subnets (measured by operational synchrony) has been measured in ten novice meditators using an electroencephalogram (EEG) recording in a pre-/post-meditation intervention design. We have found that while the (...) whole DMN was clearly suppressed, different subnets of DMN responded differently after 4 months of meditation training: The strength of EEG operational synchrony in the right and left posterior modules of the DMN decreased in resting post-meditation condition compared to a pre-meditation condition, whereas the frontal DMN module on the contrary exhibited an increase in the strength of EEG operational synchrony. These findings combined with published data on functional–anatomic heterogeneity within the DMN and on trait subjective experiences commonly found following meditation allow us to propose that the first-person perspective and the sense of agency (the witnessing observer) are presented by the frontal DMN module, while the posterior modules of the DMN are generally responsible for the experience of the continuity of ‘I’ as embodied and localized within bodily space. The realities of long‐term care call for a refurbished, concrete concept of autonomy that systematically attends to the history and development of persons and takes account of the experiences of daily living.

Short-term humanitarian medical volunteerism has grown significantly among both clinicians and trainees over the past several years. Increasingly, both volunteers and their respective institutions have faced important challenges in regard to medical ethics and professional codes that should not be overlooked. We explore these potential concerns and their risk factors in three categories: ethical responsibilities in patient care, professional responsibility to communities and populations, and institutional responsibilities towards trainees. We discuss factors increasing the risk of harm to patients and (...) communities, including inadequate preparation, the use of advanced technology and the translation of Western medicine, issues with clinical epidemiology and test utility, difficulties with the principles of justice and clinical justice, the lack of population-based medicine, sociopolitical effects of foreign aid, volunteer stress management, and need for sufficient trainee supervision. We review existing resources and offer suggestions for future skill-based training, organisational responsibilities, and ethical preparation. (shrink)

Term Functor Logic is a term logic that recovers some important features of the traditional, Aristotelian logic; however, it turns out that it does not preserve all of the Aristotelian properties a valid inference should have insofar as the class of theorems of Term Functor Logic includes some inferences that may be considered irrelevant (e.g. ex falso, verum ad, and petitio principii). By following an Aristotelian or syllogistic notion of relevance, in this contribution we adapt a tableaux (...) method for Term Functor Logic in order to avoid irrelevance and we offer some sort of intuitive semantics in terms of traditional inferential situations (e.g. propter quid, quia, non causa ut causa, and non sequitur). A world product time series covering two million years is well fit by either a sum of four exponentials, or a constant elasticity of substitution (CES) combination of three exponential growth modes: “hunting,” “farming,” and “industry.” The CES parameters suggest that farming substituted for hunting, while industry complemented farming, making the industrial revolution a smoother transition. Each mode grew world product by a factor of a few hundred, and grew a hundred times faster than its predecessor. The termination of rewrite systems for parameter recursion, simple nested recursion and unnested multiple recursion is shown by using monotone interpretations both on the ordinals below the first primitive recursively closed ordinal and on the natural numbers. We show that the resulting derivation lengths are primitive recursive. As a corollary we obtain transparent and illuminating proofs of the facts that the schemata of parameter recursion, simple nested recursion and unnested multiple recursion lead from primitive recursive functions to primitive recursive functions.

The paper attempts to show certain consequences, resulting from the ambiguous use of the term 'culture', for the protection of human rights, by comparing the use of the term in the Declaration of the Principles of International Cultural Cooperation (UNESCO 1966), with its use in the Universal Declaration of Human Rights and the International Covenant on Economic, Social and Cultural Rights. Long-term depression (LTD) of synaptic transmission at parallel fibre-Purkinje cell synapses is thought to be a cellular substrate of motor learning in the cerebellum. This use-dependent change in synaptic efficacy is induced by conjunctive stimulation of Parallel fibres and climbing fibres. Researchers agree that the induction of LTD requires, as an initial step, a calcium influx via voltage-gated Ca2+ channels into a Purkinje cell, together with activation of ionotropic (AMPA) and probably metabotropic subtypes of glutamate receptors of this cell. (...) Indeed, due to the lack of specific antagonist, the final demonstration of the contribution of metabotropic receptors in the LTD induction process, under functional conditions, remains unanswered. The debate is now focused on the second-messenger processes leading to LTD of synaptic transmission at parallel fibre-Purkinje cell synapses, after the calcium influx into the cell. All researchers agree that a calcium-dependent cascade of events including activation of protein kinase C is necessary for LTD induction. In contrast, the recruitment in the LTD induction of another cascade, also triggered by Ca2+, that is, through synthesis of nitric oxide and cyclic GMP, remains controversial. On the other hand, growing evidence suggests that these chains of reaction underlying LTD might ultimately lead to a genuine change in the functional characteristics of AMPA receptors at the parallel fibre-Purkinje cell synapses. The term bhavaṅga is regarded as a unique technical term of Theravāda abhidhamma tradition, and the text Jié tuō dào lùn, i.e. the Chinese translation of *Vimuttimagga, mentions yŏufēnxīn the Chinese counterpart of bhavaṅga eleven times. These occurrences are found in the section of the text on the cognitive process. The text is, however, too abstruse to understand the term easily, and the existing translations of it are imperfect. Subsequently, the term in the Jié tuō dào (...) lùn has been considered and interpreted by former translations in the same way as it is in other Pāli abhidhamma texts. Indeed, the Jié tuō dào lùn has more than similar definitions for bhavaṅga. It is described as the inactive state of mind from which the active states of mind arise. On the other hand, there are also a number of small but significant differences in its description of bhavaṅga. It has no sub-states, and there are no technical terms for conditional relations with other states of mind. The similarities and differences found in the discussion of bhavaṅga shed light on some salient features of the Jié tuō dào lùn. (shrink)

The term “hypnozoite” is derived from the Greek words hypnos and zoon. Hypnozoites are dormant forms in the life cycles of certain parasitic protozoa that belong to the Phylum Apicomplexa and are best known for their probable association with latency and relapse in human malarial infections caused by Plasmodium ovale and P. vivax. Consequently, the hypnozoite is of great biological and medical significance. This, in turn, makes the origin of the name “hypnozoite” a subject of interest. Some “missing” history (...) that is now placed on record shows that Miles B. Markus coined the term “hypnozoite”. While a PhD student at Imperial College London, he carried out research that led to the identification of an apparently dormant form of Cystoisospora. In 1976, he speculated: “If sporozoites of Isospora can behave in this fashion, then those of related Sporozoa, like malaria parasites, may have the ability to survive in the tissues in a similar way.” He adopted the term “hypnozoite” for malaria in 1978 when he wrote in a little-known journal that this name would “… describe any dormant sporozoites or dormant, sporozoite-like stages in the life cycles of Plasmodium or other Haemosporina.” At that time, the existence of a hypnozoite form in the life cycle of Plasmodium was still a hypothetical notion. In 1980, however, Wojciech A. Krotoski published details concerning his actual discovery of malarial hypnozoites, an event of considerable importance. (shrink)

The term “economic self-management” stood at the center of economic and political debates in Soviet Estonia in 1987–1988. This article traces its transformation from an economic term to a political concept, reconstructing the intellectual resources that the reformers were drawing on in this process. Navigating the constraints of Soviet discourse, reform-minded academics in Soviet Estonia radically expanded the original meaning of isemajandamine, which ultimately provided an argumentative platform for declaring the republic’s “sovereignty” within the Soviet Union. The article (...) brings out the linguistic, political, and transnational dimensions of this conceptual innovation, which started in 1987 and was completed when the law on the “economic independence” of the Baltic republics was adopted by the Soviet Union in 1989. The psychological effects of aesthetic education have often been discussed, and major studies such as Michael Parsons’s inquiry into art understanding show that the development of understanding works of visual art is influenced by education.1 His findings show that the way people talk about art can be structured in five stages of development according to the model of Jean Piaget’s theory of cognitive development.
