BackgroundNusinersen is one of an increasing number of new, expensive orphan drugs to receive authorization. These drugs strain public healthcare budgets and challenge principles for resource allocation. Nusinersen was introduced in the Norwegian public healthcare system in 2018. A national expert group consisting of physicians was formed to oversee the introduction and continuation of treatment in light of specific start and stop criteria.MethodsWe have studied experiences within the expert group with a special emphasis on their application of the start and (...) stop criteria, rationing of treatment, and experienced moral dilemmas. A research interview with six members of the national expert group was performed, then analysed with manifest content analysis. The analysis was supplemented with publically available sources on priority setting and the process leading up to the introduction of nusinersen and the establishment of the expert group.ResultsSixty-six patients have received treatment within the first 25 months since the national expert group’s establishment. Treatment has not been discontinued for any patient. No patients under 18 years of age have been denied treatment, as those who were referred at this age were all deemed to fulfill the start criteria. The expert group has, however, increased geographical treatment equity and facilitated important cooperation at the national level. Furthermore, it has enhanced open and critical discussions of both medical issues and new ethical dilemmas.ConclusionAlthough facilitating equal access to treatment for SMA patients, the national expert group has not discontinued treatment for any patient. It is suggested that in order for clinicians to be able to ration care for individual patients, they require both adequate support and sufficient formal authority. Start and stop criteria need to be re-evaluated as more knowledge and experience are gained regarding the treatment. (shrink)

Proponents of the Many Worlds Interpretation (MWI) of quantum mechanics are divided in their attitudes to the idea of quantum immortality. Some, e.g. Max Tegmark (immortalists), believe one should always expect to experience subjective survival in a quantum suicide thought experiment, because one can only ever be on a branch of the universal wavefunction where one is alive. Others, e.g. Sean Carroll, David Papineau and David Wallace (mortalists), believe that the truth of the MWI has no such consequences (...) and that our situation is analogous to that of an observer in a single, non-branching, stochastically-evolving universe. A related question concerns whether we can take survival of a quantum suicide experiment as evidence confirming the MWI. This paper focuses on the core principles underlying these debates by considering each of these questions in turn for idealised cases of quantum immortality, arguing that while rejecting such applications of the idea of quantum immortality is tenable, to do so requires Everettians to pay various methodological and metaphysical costs that are in tension with the particular strand of austere Everettianism exemplified by Carroll, Papineau and Wallace in particular. A defence of the idea that there are sui generis duties of love: duties, that is, that we owe to people in virtue of standing in loving relationships with them. I contrast this non‐reductionist position with the widespread reductionist view that our duties to those we love all derive from more generic moral principles. The paper mounts a cumulative argument in favour of the non‐reductionist position, adducing a variety of considerations that together speak strongly in favour of adopting it. However, the underlying assumption that this is the best approach has received almost no defence, and has been attacked by particularists, who argue that the traditional link between morality and principles is little more than an unwarranted prejudice. David Wallace argues that we should take quantum theory seriously as an account of what the world is like--which means accepting the idea that the universe is constantly branching into new universes. He presents an accessible but rigorous account of the 'Everett interpretation', the best way to make coherent sense of quantum physics.

R. Jay Wallace argues in this book that moral accountability hinges on questions of fairness: When is it fair to hold people morally responsible for what they do? Would it be fair to do so even in a deterministic world? To answer these questions, we need to understand what we are doing when we hold people morally responsible, a stance that Wallace connects with a central class of moral sentiments, those of resentment, indignation, and guilt. To hold someone (...) responsible, he argues, is to be subject to these reactive emotions in one's dealings with that person. Developing this theme with unusual sophistication, he offers a new interpretation of the reactive emotions and traces their role in our practices of blame and moral sanction. With this account in place, Wallace advances a powerful and sustained argument against the common view that accountability requires freedom of will. Instead, he maintains, the fairness of holding people responsible depends on their rational competence: the power to grasp moral reasons and to control their behavior accordingly. He shows how these forms of rational competence are compatible with determinism. At the same time, giving serious consideration to incompatibilist concerns, Wallace develops a compelling diagnosis of the common assumption that freedom is necessary for responsibility. Rigorously argued, eminently readable, this book touches on issues of broad concern to philosophers, legal theorists, political scientists, and anyone with an interest in the nature and limits of responsibility. Why is the human mind able to perceive and understand the truth about reality; that is, why does it seem to be the mind's specific function to know the world? Quine's set theory, New Foundations, has often been treated as an anomaly in the history and philosophy of set theory. In this book, Sean Morris shows that it is in fact well-motivated, emerging in a natural way from the early development of set theory. Morris introduces and explores the notion of set theory as explication: the view that there is no single correct axiomatization of set theory, but rather that the various axiomatizations all serve to explicate the notion of (...) set and are judged largely according to pragmatic criteria. Morris also brings out the important interplay between New Foundations, Quine's philosophy of set theory, and his philosophy more generally. We see that his early technical work in logic foreshadows his later famed naturalism, with his philosophy of set theory playing a crucial role in his primary philosophical project of clarifying our conceptual scheme and specifically its logical and mathematical components. According to public health data, the US Hispanic population is far healthier than would be expected for a population with low socioeconomic status. Ever since Kyriakos Markides and Jeannine Coreil highlighted this in a seminal 1986 article, public health researchers have sought to explain the so-called “Hispanic paradox.” Several candidate explanations have been offered over the years, but the debate goes on. This article offers a philosophical analysis that clarifies how two sets of obstacles make it particularly difficult to explain (...) the Hispanic paradox. First, different research projects define the Hispanic paradox phenomenon in substantially different ways. Moreover, using Bas van Fraassen’s pragmatic theory of explanation and Sean Valles’s extension of it with the concept of “phenomenon choice,” it also becomes clear that there are also multiple ways of explaining each individual definition of the phenomenon. A second set of philosophical and methodological challenges arises during any attempt to study “Hispanic” phenomena, with one key challenge being that the “Hispanic” panethnic concept was intentionally made vague as it was developed and popularized during the 1960s–1970s. After comparing this case with similar cases in the philosophical literature, the article concludes with observations on what makes this problem unique, particularly its ethical features. (shrink)

The intelligibility of nature was a persistent theme of William A. Wallace, OP, one of the most prolific Catholic scholars of the late twentieth century. This Reader aims to make available a representative selection of his work in the history of science, natural philosophy, and theology illustrating his defense and development of this central theme. Wallace is among the most important Galileo scholars of the past fifty years and a key figure in the recent revival of scientific realism. (...) Practical reason is the general human capacity for resolving, through reflection, the question of what one is to do. Deliberation of this kind is practical in at least two senses. First, it is practical in its subject matter, insofar as it is concerned with action. But it is also practical in its consequences or its issue, insofar as reflection about action itself directly moves people to act. Our capacity for deliberative self-determination raises two sets of philosophical problems. For one thing, (...) there are questions about how deliberation can succeed in being practical in its issue. What do we need to assume—both about agents and about the processes of reasoning they engage in—to make sense of the fact that deliberative reflection can directly give rise to action? Can we do justice to this dimension of practical reason while preserving the idea that practical deliberation is genuinely a form of reasoning? For another, there are large issues concerning the content of the standards that are brought to bear in practical reasoning. Which norms for the assessment of action are binding on us as deliberating agents? Do these norms provide resources for critical reflection about our ends, or are they exclusively instrumental? Under what conditions do moral norms yield valid standards for reasoning about action? Most historical accounts of "the West" take it for granted that the guiding principles of the Western tradition—reason, progress, and freedom—have been passed down directly from ancient Greece to modern Europe, evolving in isolation from all non-Western cultures. Today, many political analysts and cultural critics maintain that the Western tradition is fast approaching its end, for better or worse, as it becomes more and more integrated with non-Western cultures in an increasingly globalized world. But what if we are witnessing something (...) else entirely—not the "end" of the West but rather another historical mutation of the idea of the West itself? This groundbreaking work shows that whether the West is hailed as the source of all historical progress or scorned as the root of all cultural imperialism, it remains a deeply problematic concept that is intrinsically connected to an ethnocentric view of the world. In a critical reading of the continental philosophers Husserl, Heidegger, Levinas, and Derrida as well as the postcolonial thinkers Said, Mohanty, Bhabha, and Trinh, Sean Meighoo strikes at the intellectual foundations of Western exceptionalism until its ideological supports show through. Carroll notes that, despite popular stereotypes of existential “philosophers sitting in cafes, smoking cigarettes and drinking apricot cocktails” and neuroscientists decked out in lab coats, there is an undeniable connection between existentialism and science. I attempt to get as clear as possible on the chain of reasoning by which irreversible macrodynamics is derivable from time-reversible microphysics, and in particular to clarify just what kinds of assumptions about the initial state of the universe, and about the nature of the microdynamics, are needed in these derivations. What ontology does realism about the quantum state suggest? The main extant view in contemporary philosophy of physics is wave-function realism . We elaborate the sense in which wave-function realism does provide an ontological picture, and defend it from certain objections that have been raised against it. However, there are good reasons to be dissatisfied with wave-function realism, as we go on to elaborate. This motivates the development of an opposing picture: what we call spacetime state realism , a view (...) which takes the states associated to spacetime regions as fundamental. This approach enjoys a number of beneficial features, although, unlike wave-function realism, it involves non-separability at the level of fundamental ontology. We investigate the pros and cons of this non-separability, arguing that it is a quite acceptable feature, even one which proves fruitful in the context of relativistic covariance. In this book, Sean Homer addresses Slavoj Žižek’s work in a specific political conjuncture, his political interventions in the Balkans. The charge of inconsistency and contradiction is frequently levelled at Žižek’s politics, a charge he openly embraces in the name of "pragmatism." Homer argues that his interventions in the Balkans exposes the dangers of this pragmatism for the renewal of Leftist politics that he calls for. In this engaging study, Sean Illing examines the impact of Fyodor Dostoevsky and Friedrich Nietzsche on the development of Albert Camus's political philosophy. It innovatively attempt to offer a substantive examination of Camus's dialogue with Nietzsche and Dostoevsky. The connections among these writers have been discussed in the general context of modern thought or via overlapping literary themes. This project emphasizes the political dimensions of these connections. In addition to re-interpreting Camus's political thought, the aim is to clarify Camus's (...) struggle with transcendence and to bring renewed attention to his unique understanding of the relationship between nihilism, ideology, and political violence in the twentieth century. The book focuses on Camus's dialogue with Nietzsche and Dostoevsky for three reasons. First, these are the thinkers with whom Camus is most engaged. Indeed, the problems and themes of Camus's work are largely defined by Dostoevsky and Nietzsche; a full account of this dialogue will therefore enhance our understanding of Camus while also reinforcing the enduring importance of Nietzsche and Dostoevsky. Second, it allows a recasting of Camus' political philosophy as both a synthesis of and a response to Nietzsche and Dostoevsky's projects. Finally, this approach allows for a reassessment of Camus's broader political significance, which I contend has been undervalued in the literature. Ultimately, I argue that Camus remains among the most important moral and political voices of the twentieth century. Although limited, his philosophy of revolt offers a humane portrait of justice and articulates a meaningful alternative to the extremes of ideological politics. (shrink)

Ohio State University Press, 2020. 344pp.In Archive Feelings: A Theory of Greek Tragedy, Mario Telò takes aim at one of the most canonical (if also one of the most contested) features of Greek tragedy: its potential to deliver catharsis (12).1 Through careful close readings of Greek tragedies informed by (...) psychoanalytic theory and deconstruction, Telò makes a persuasive case that tragedy is anti-cathartic. “The Freudian take on catharsis is complicated, or even undercut,” he writes in the introduction, “by its own anti-cathartic latencies… We can accordingly look at tragedy as a form of dramatic poetry that finds its affective raison d’être… in these anti-cathartic feelings” (34).Telò states that the intent of Archive Feelings is mainly to intervene in the age-old debate, asking: “why do human beings enjoy tragedy?” (14). He suggests that the pleasure of tragedy is, in fact, a masochistic one: a denial of release or pleasure (such as would come from catharsis) that an audience or reader enjoys in its specifically joyless jouissance. At stake is “feeling material,” a “fantasy” of “merging into the impersonal, anorganic, universal matter of elemental life” (44) evoked in tragedy’s death-driven withdrawal from action. With this concept in mind, Telò goes beyond even his own ambitions for his project. More than merely intervening in debates about the pleasures of tragedy, he argues powerfully for the continuing relevance of Greek tragedy for critical theory today, particularly in light of the so-called “affective” and “material” turns in the humanities. Archive Feelings is thus a useful book for classicists conducting interdisciplinary and/or trans-historical research, but even more so for any scholar working within the constellation of psychoanalysis, deconstruction, queer theory, affect theory or critical race theory. Telò makes a persuasive case for tragedy as a lodestar within this firmament–a vital tool for thinking through problems within and across disciplines.In striking contradistinction to previous theorists of Greek tragedy, including Aristotle and Nietzsche, Telò locates the engine of Greek tragedy in the death drive. In so doing, he reads in ancient narratives a number of experiences such as “acceleration, hoarding, vertiginous suspension, [End Page 113] breathless looping, affective bulimia or binge-eating, serial cutting, trying to enter or fold in upon oneself, auto-immune inflammation” (14), and finds in “the orgasm as an unfinished, non-teleological pleasure,” a desire to return to a state of “inert matter” (13).2 Noting that the original meaning of the Greek word archê is “origin,” Telò connects this impulse to once again become “inert” with the desire to create archives. Thus, Telò sees Greek tragedies as both a model for projects of archival reconstruction as well as archival projects themselves: they are “anxious and debilitating” attempts to “re-create the past as it was” (13).3Telò’s study is divided into three parts: Archival Time, Archival Space, and Archival Endings, which attempt to “structurally reproduce the psychological temporalities of the archive” (49). In each of his five chapters, Telò elaborates the diverse narrative masochisms of Greek tragedy, exploring the various stakes and species of tragic anti-catharsis. In Chapter 1, Telò reads the “archive fever” of Sophocles’ Oedipus at Colonus against the “archive fatigue” of Euripides’ Phoenissae, finding in both of these plays—near the end of the epoch of ancient tragedy—an impulse toward “textual hoarding” (76) that both undermines the possibility of catharsis and “exhausts” (106) the possibilities of the genre (107). In both the “synchronic gathering of disparate fragments of experience” into the body of Oedipus before his death and the aesthetics of boredom, fatigue, and inertia that suffuse the Phoenissae, Telò locates a desire to store up, rather than purge, emotions such as pity and fear, in a flouting of the normal formula for catharsis (73).In Chapter 2, Telò looks at the figures of Heracles and Medea in their original myths and tragic incarnations, seeing in both the compulsion to recircle around scenes of their respective “primordial traumas” (145).
