The federal government is funding a sea change in health care by investing in interventions targeting social determinants of health, which are significant contributors to illness and health inequity. This funding power has encouraged states, professional and accreditation organizations, health care entities, and providers to focus heavily on social determinants. We examine how this shift in focus affects clinical practice in the fields of oncology and emergency medicine, and highlight potential areas of reform.

It is well established that demographic representation in clinical research is important for understanding the safety and effectiveness of novel therapeutics and vaccines in diverse patient populations. In recent years, the National Institutes of Health and Food and Drug Administration have issued guidelines and recommendations for the inclusion of women, older adults, and racial and ethnic minorities in research. However, these guidelines fail to provide an adequate explanation of why racial and ethnic representation in clinical research is important. This article (...) aims to both provide the missing arguments for why adequate representation of racial and ethnic minorities in clinical research is essential and to articulate a number of recommendations for improving diversity going forward. Appropriate racial and ethnic representation and fair inclusion help (1) increase the generalisability of clinical trial results, (2) equitably distribute any benefits of clinical research and (3) enable trust in the research enterprise. In this paper I argue that, in addition to having an obligation to resist the oppression of others, people have an obligation to themselves to resist their own oppression. This obligation to oneself, I argue, is grounded in a Kantian duty of self-respect.

In earlier work ( Cleland [2001], [2002]), I sketched an account of the structure and justification of ‘prototypical’ historical natural science that distinguishes it from ‘classical’ experimental science. This article expands upon this work, focusing upon the close connection between explanation and justification in the historical natural sciences. I argue that confirmation and disconfirmation in these fields depends primarily upon the explanatory (versus predictive or retrodictive) success or failure of hypotheses vis-à-vis empirical evidence. The account of historical explanation that I (...) develop is a version of common cause explanation. Common cause explanation has long been vindicated by appealing to the principle of the common cause. Many philosophers of science (e.g., Sober and Tucker) find this principle problematic, however, because they believe that it is either purely methodological or strictly metaphysical. I defend a third possibility: the principle of the common cause derives its justification from a physically pervasive time asymmetry of causation (a.k.a. the asymmetry of overdetermination). I argue that explicating the principle of the common cause in terms of the asymmetry of overdetermination illuminates some otherwise puzzling features of the practices of historical natural scientists. The question ‘what is life?’ has long been a source of philosophical debate and in recent years has taken on increasing scientific importance. The most popular approach among both philosophers and scientists for answering this question is to provide a “definition” of life. In this article I explore a variety of different definitional approaches, both traditional and non-traditional, that have been used to “define” life. I argue that all of them are deeply flawed. It is my contention that a scientifically (...) compelling understanding of the nature of life presupposes an empirically adequate scientific theory (vs. definition) of life; as I argue, scientific theories are not the sort of thing that can be encapsulated in definitions. Unfortunately, as I also discuss, scientists are currently in no position to formulate even a tentative version of such a theory. Recent discoveries in biology and biochemistry have revealed that familiar Earth life represents a single example that may not be representative of life. If this is the case, life on Earth today provides an empirically inadequate foundation for theorizing about life considered generally. I sketch a strategy for procuring the needed additional examples of life without the guidance of a definition or theory of life, and close with an application to NASA’s fledgling search for extraterrestrial life. Hearing the difference between a patriarchal voice and a relational voice defines a paradigm shift: a change in the conception of the human world. Theorizing connection as primary and fundamental in human life leads to a new psychology, which shifts the grounds for philosophy and political theory. A crucial distinction is made between a feminine ethic of care and a feminist ethic of care. Voice, relationship, resistance, and women become central rather than peripheral in this reframing of the human world.

This study assessed the challenges with transitioning from start-up to scaleup in Lagos’ emerging entrepreneurial ecosystem (EE)Entrepreneur/entrepreneurialecosystem. Current studies on transitional challengesTransitional challenges in emerging EEs appear nascent, fragmented, or narrow focused, making it challenging to focus policy initiatives. The study is based on a qualitative methodology based on an interpretive philosophy. Primary data were collected from forty ecosystem actors in Lagos comprising founders, funders, support providers such as hubs, NGOs, academia, and government agencies, using semi-structured interviews. Additional data were (...) collected from a two-day focus group discussion Summit in LagosLagos. Thematic analysis was used to analyse the data retrieved. Micro-level challenges featured issues like corporate governanceCorporate governance, competence and capabilities, and business model-related challenges; at the meso-level, talent leakage, access to finance, markets, information, and supports and networks; and at the macro-level, tax, regulation, infrastructural deficiency, and security. We found spillover and feedback effects across the levels. The study argues the need to recraft internal configurations of start-ups at the micro-level, shift from competition to cooperation at the meso-level, and leverage private–public actor collaboration at the macro-level. The study addresses the transitional challenges in multi-levels from an ecosystem lens which is a major shift from mainstream literature in emerging economies. (shrink)

This paper compares the educational thoughts of Obafemi Awolowo and Tai Solarin. Its methodology is critical and comparative. The paper argues that the variations of the political philosophies of the two thinkers account for the differences in their views on education. It further contends that the educational ideas of both thinkers reflect African cultural experiences. The paper also explores the possibility of integrating the insights of the two thinkers into the educational policies and practices of contemporary African societies.

In a Guest Lecture delivered by Professor Thaddeus Metz at a Colloquium organized in honour of my 50th birthday, he critically interrogated various aspects of my African philosophical scholarship with a particular focus on what I consider as the task of an African philosopher in the twenty-first century. Drawing on the existential and social problems in contemporary Africa, I have argued that African philosophy should be tailored towards ameliorating these problems as a way of making life meaningful. Metz’s striking criticism (...) is that doing philosophy that does not necessary address existential and socio-political problems in Africa is worth taking seriously in African philosophy. He adds that the very idea of “meaningfulness constitutes a strong, competing reason,” to do philosophy for its own sake. In this article, I reply Metz, contending that his critique only differs in degree from the position I earlier defended but not in kind regarding the connection between theory and praxis. While we both agree on the imperativeness of theorizing in African philosophy, I argue further that African philosophy should go beyond this to solve the practical issues relevant to the advancement of humanity and the society. The paper attempts to provide a philosophical justification for the belief in the living-dead among the traditional Africans using the Yoruba as an example. It argues that in spite of the various criticisms leveled against the belief in the living-dead among the traditional Africans, this belief can be rationally defended and philosophically understood within the conceptual scheme of the traditional Yoruba thought. The paper argues that the link between the living and the livingdead possesses social as well as moral functions. (...) It encourages people to live morally so as to merit a good place in the other world. It imbues them with the spirit of hard work, industry, and integrity in the community. It symbolizes the continuity not only of the social structure but of the human community as well. Finally, the paper points to the need of reappraising this traditional belief in the living-dead in the quest of relieving contemporary African societies from their plethora of social predicaments. This paper posits that there are elements of oppression in some of the Yoruba proverbs that relate to women. It argues that these proverbs violate the rights and dignity of women, and that they are indicators of discrimination against women in Yoruba culture. The paper further argues that the most fundamental but neglected aspect in gender discourse lies in the proverbial resources of the community. The paper provides textual evidence of proverbial oppression of the feminine gender in Yoruba culture, and (...) also underscores their pernicious effects on the struggle for gender balance. The flurry of debate that trailed the existence of African philosophy in the 1960s and 70s and the consequent demise of the controversies in the late 1990s have occasioned a periodiszation shift from traditional African philosophy to contemporary African philosophy. While the scope and nature of predominant issues inthese periods differ considerably, what ought to constitute the basis and shape the direction of discourse in contemporary African philosophy remain controversial. In this regard, this paper argues that rethinking African philosophy should (...) be high on the agenda. It harps that more fundamental to contemporary African philosophy, is the critical need for self-assessment and re-evaluation, which would involve rethinking the nature, direction, scope, method and place of African philosophy. Rethinking African philosophy is a cognitive process of charting a new course of pragmatic reflection on metaphysical, epistemological, ethical, aesthetical, social and political themes in contemporary African philosophy, in order to make relevant philosophical abstraction to practical human problems inthe continent. The case is justifiably made that those African philosophers should make the influence of their speculations spill beyond the confine of academic citadel to the outside world such that will influence the lives of contemporary Africans. The book reviews the theory and concepts of happiness, explaining how these concepts underpin a line of research which is both an attempt to understand the determinants of happiness and a tool for understanding the effects of a host of phenomena on human well being.

The assumption that all life on Earth today shares the same basic molecular architecture and biochemistry is part of the paradigm of modern biology. This paper argues that there is little theoretical or empirical support for this widely held assumption. Scientists know that life could have been at least modestly different at the molecular level and it is clear that alternative molecular building blocks for life were available on the early Earth. If the emergence of life is, like other natural (...) phenomena, highly probable given the right chemical and physical conditions then it seems likely that the early Earth hosted multiple origins of life, some of which produced chemical variations on life as we know it. While these points are often conceded, it is nevertheless maintained that any primitive alternatives to familiar life would have been eliminated long ago, either amalgamated into a single form of life through lateral gene transfer (LGT) or alternatively out-competed by our putatively more evolutionarily robust form of life. Besides, the argument continues, if such life forms still existed, we surely would have encountered telling signs of them by now. These arguments do not hold up well under close scrutiny. They reflect a host of assumptions that are grounded in our experience with large multicellular organisms and, most importantly, do not apply to microbial forms of life, which cannot be easily studied without the aid of sophisticated technologies. Significantly, the most powerful molecular biology techniques available—polymerase chain reaction (PCR) amplification of rRNA genes augmented by metagenomic analysis—could not detect such microbes if they existed. Given the profound philosophical and scientific importance that such a discovery would represent, a dedicated search for ‘shadow microbes’ (heretofore unrecognized ‘alien’ forms of terran microbial life) seems in order. The best place to start such a search is with puzzling (anomalous) phenomena, such as desert varnish, that resist classification as ‘biological’ or ‘nonbiological’. Ó 2007 Elsevier Ltd.. (shrink)

In this essay, I connect the sexual victimization of women, children, and pet animals with the violence manifest in a patriarchal culture. After discussing these connections, I demonstrate the importance of taking seriously these connections because of their implications for conceptual analysis, epistemology, and political, environmental, and applied philosophy. My goal is to broaden our understanding of issues relevant to creating peace and to provide some suggestions about what must be included in any adequate feminist peace politics.

This paper explores the conduct of performance appraisals of nurses in a New Zealand hospital, and how fairness is perceived in such appraisals. In the health sector, performance appraisals of medical staff play a key role in implementing clinical governance, which, in turn, is critical to containing health care costs and ensuring quality patient care. Effective appraisals depend on employees perceiving their own appraisals to be fair both in terms of procedure and interaction with their respective appraiser. We examine qualitative (...) data from interviews and focus groups, involving 22 nurses in a single department, to determine whether perceived injustices impact on the effective implementation of the appraisal system. Our results suggest that particular issues had been causing some sense of injustice, and most of these were procedural. Potential solutions focus on greater formalisation of the performance appraisal process, and more training for appraisers and appraisees. (shrink)

A serial reaction time experiment tested the hypothesis that there are two independent forms of implicit learning: learning that is linked to making judgments about stimuli, and learning that is linked to motor processing. Participants performed 2, 6, or 12 blocks of single task SRT, dual task SRT, or observation with one of five sequences; each sequence had the same underlying structure. Participants then performed two implicit tests, SRT and pattern judgment, as well as a generation test and an explicit (...) knowledge test. There was a double dissociation of the effect of sequence surface structure on SRT and pattern judgment. SRT and pattern judgment competence developed independently of each other across length of acquisition task. Performance on both implicit tasks did not depend on explicit knowledge. These results indicate that the SRT and pattern judgment measures tap independent forms of implicit learning, which is consistent with the modular nature of memory and cognition. (shrink)

The main question examined is: How do nurses and other healthcare professionals ensure ethical interprofessional collaboration-in-practice as an everyday practice actuality? Ethical interprofessional collaboration becomes especially relevant and necessary when interprofessional practice decisions are contested. To illustrate, two healthcare scenarios are analyzed through three ethics lenses. Biomedical ethics, relational ethics, and virtue ethics provide different ways of knowing how to be ethical and to act ethically as healthcare professionals. Biomedical ethics focuses on situated, reflective, and nonabsolute principled justification, all things (...) considered; relational ethics on intersubjective, professional, and institutional relations; and virtue ethics on prephilosophical tradition and what it means to be good and to be human embedded in social and political community. Analysis suggests that interprofessional collaboration-in-practice may be more rhetoric than actuality. Key challenges of interprofessional collaboration-in-practice and specific conditions perpetuating dissension and conflict are outlined with specific education and policy recommendations included. The philosophical dispute about personal identity thrives in part because common sense supports both sides. That is, our commonsense notion of a person is rich enough to accommodate both the animalist view that we are human beings whose lives are bounded by the biological events of birth and death and, also, the Lockean view that our lives as reflective beings could in principle come apart from a given animal life. Consequently, there is no way to arrive at a consistent position (...) about personal identity without revising some aspect of our commonsense thinking about persons. This puts philosophers on a somewhat awkward footing. Other things being equal, they would prefer to provide conclusive arguments in favor of their views, arguments that rule out all of the alternatives as untenable. But this is not feasible in the case of personal identity. Any consistent account of personal identity that anyone wishes to defend must be put forward as a revisionary proposal that selects and develops some aspect of our inconsistent commonsense outlook over others. And anyone can resist the proposal by choosing to select and develop some other strand instead. (shrink)

Feminist studies in the history and philosophy of science have suggested that supposedly neutral and objective discourses are shaped by pairs of dualisms, which though value-laden are assumed to inhere in the order of nature. These hierarchical pairs devalue women, particularly their bodies and their labor, as they sanction the domination of nature. Readers of literature can draw on these studies to address texts and genres which do not thematize gender but rather purport to portray "the human condition." Samuel Beckett's (...) Molloy, with its clear structure of Cartesian divisions, provides a dramatic example of how an examination of dualisms reveals the presence of a language of gender informing a minimalist literary text. This book consists of notes of lectures given by G. E. Moore at Cambridge during the three terms of 1934-35. They were compiled by the editor, Alice Ambrose, who was then Student of Newnham College, and the late Margaret Macdonald, who was then Fellow of Girton College. The lectures contain discussions of some material on which Moore published little or nothing, for example, types and tokens, propositional functions and their relation to common properties and relations, and the objects of false (...) judgements. They also include detailed examinations of the views of a number of Moore's contemporaries, including C. D. Broad, Bertrand Russell, F. P. Ramsey, W. E. Johnson, and John Wisdom. Moreover, the lectures do a nice job of illustrating Moore's general approach to the philosophical enterprise, highlighting his emphasis upon ordinary language and revealing the high standards of clarity and precision that he demanded of himself and others. It is clear, however, that Moore is not so much concerned with formulating philosophical positions as with exploring philosophical problems. This lends a certain lack of focus to the lectures and makes them a bit confusing at times. Nevertheless, the lectures provide valuable insight into the evolution of Moore's later thought about a number of important philosophical problems. (shrink)

Working at the bedside and within communities as patient advocates, nurses frequently intervene to advance individuals’ health and well-being. However, the International Council of Nurses’ Code of Ethics asserts that nurses should expand beyond the individual model and also promote a rights-enabling environment where respect for human dignity is paramount. This article applies the results of an ethnographic human rights study with displaced populations in Rwanda to argue for a rights-based social advocacy role for nurses. Human rights advocacy strategies include (...) sensitization, participation, protection, good governance, and accountability. By adopting a rights-based approach to advocacy, nurses contribute to health agendas that include more just social relationships, equitable access to opportunities, and health-positive living situations for all persons.
