Ethics consultation has become an integral part of the fabric of U.S. health care delivery. This article summarizes the second edition of the Core Competencies for Health Care Ethics Consultation report of the American Society for Bioethics and Humanities. The core knowledge and skills competencies identified in the first edition of Core Competencies have been adopted by various ethics consultation services and education programs, providing evidence of their endorsement as health care ethics consultation (HCEC) standards. This revised report was prompted (...) by thinking in the field that has evolved since the original report. Patients, family members, and health care providers who encounter ethical questions or concerns that ethics consultants could help address deserve access to efficient, effective, and accountable HCEC services. This article investigates the moderating effects of organizational social consciousness on the natural environmental competency and innovativeness relationship. Organizational social consciousness reflects the organization’s awareness of its place and contribution to the larger system in which it exists and is developed through an organization’s social responsibility, ethics, culture, corporate values, and the view of its stakeholders. Through our study of key strategic decision makers from organizations located in the USA, we operationalize organizational social consciousness and demonstrate the efficacy of (...) this construct in relation to the organizational-level constructs of environmental management competency and innovativeness. In this paper I get clearer on the considerations that ought to inform the evaluation and development of medico-legal competency criteria—where this is taken to be a question regarding the abilities that ought to be needed for a patient to be found competent in medico-legal contexts. In the “Decisional Competency in Medico-Legal Contexts” section I explore how the question regarding the abilities that ought to be needed for decisional competence is to be interpreted. I begin by considering an (...) interpretation that takes the question to be asking about the abilities needed to satisfy an idealized view of competent decision-making, according to which decisional competency is a matter of possessing those abilities or attributes that are needed to engage in good or effective or, perhaps, substantially autonomous or rational decision-making. The view has some plausibility—it accords with the way decisional competency is understood in a number of everyday contexts—but fails as an interpretation of the question regarding the abilities that should be needed for decisional competence in medico-legal contexts. Nevertheless, consideration of why it is mistaken suggests a more accurate interpretation and points the way in which the question regarding the evaluation of medico-legal competency criteria is to be answered. Building on other scholarly work in the area, I outline in the “Primary and Secondary Requirements” section several requirements that decisional competence criteria ought to satisfy. Then, in the “Applying the Framework” section, I say something about the extent to which medico-legal competency criteria, as well as some models of decisional competency proposed in the academic literature, fulfil those requirements. Harvey Siegel argues that minimum competency testing (MCT) is incompatible with strong sense critical thinking. His arguments are reviewed and contrasted with positions held by John E. McPeck and Michael Scriven. Siegel's arguments seem directed against the prevailing form of MCT. However, alternative formats which allow for the aggregate and context-sensitive nature of critical thinking are not doomed to the arbitrariness Siegel finds. Law Schools are now required to provide education to law students on bias, cross-cultural competency, and racism under ABA Standard 303(c). Law clinics, with their social justice orientation, have long taught about structural causes of bias and oppression and ways to intervene at system levels to prevent problems. Medical legal partnership (MLP) clinics have done so by employing concepts from social work and health science programs on structural competency. Background: The field of clinical ethics is examining ways of determining competency. The Assessing Clinical Ethics Skills (ACES) tool offers a new approach that identifies a range of skills necessary in the conduct of clinical ethics consultation and provides a consistent framework for evaluating these skills. Through a training website, users learn to apply the ACES tool to clinical ethics consultants (CECs) in simulated ethics consultation videos. The aim is to recognize competent and incompetent clinical ethics consultation skills by (...) watching and evaluating a videotaped CEC performance. We report how we set a criterion cut score (i.e., minimally acceptable score) for judging the ability of users of the ACES tool to evaluate simulated CEC performances.Methods: A modified Angoff standard-setting procedure was used to establish the cut score for an end-of-life case included on the ACES training website. The standard-setting committee viewed the Futility Case and estimated the probability that a minimally competent CEC would correctly answer each item on the ACES tool. The committee further adjusted these estimates by reviewing data from 31 pilot users of the Futility Case before determining the cut score.Results: Averaging over all 31 items, the proposed proportion correct score for minimal competency was 80%, corresponding to a cut score that is between 24 and 25 points out of 31 possible points. The standard-setting committee subsequently set the minimal competency cut score to 24 points.Conclusions: The cut score for the ACES tool identifies the number of correct responses a user of the ACES tool training website must attain to “pass” and reach minimal competency in recognizing competent and incompetent skills of the CECs in the simulated ethics consultation videos. The application of the cut score to live training of CECs and other areas of practice requires further investigation. (shrink)

The knowledge that organizations possess, produce, and acquire adds to their strategic competency and intelligence. Organizations develop intelligence from practice and learning by doing. There is a definite relationship that exists between organizational learning and productivity that contributes to the development of organizational intelligence. Organizational intelligence is of difference kinds, but almost all of them develop from organizational actions and learning that includes but are not restricted to gaining market information, consumer interactions, business communications, creating new products and services, (...) managing actions, solving emerging problems, etc. Intelligence is the ability to comprehend things or immediate situations and act rationally and according to what the situation demands. In this paper, we examine on what respect organizational intelligence positively relates to organizational competency, and how intelligence help develop strategic competency which adds to their advantage and makes them more robust. We find using a theoretical model that the development of organizational intelligence confers competitive strength to an organization, and contributes to the overall development of higher productivity and organizational competence. How to maintain professional competence and demonstrate cross-platform communication abilities in a multimedia convergence context has become crucial. Based on a case analysis of the 2023 CCTV Hosts Competition, this paper explores the core competencies and qualities that hosts should possess in the new era, constructing a competency model for hosts in the omnimedia age. The inclusion of structural competency training in pre-health undergraduate programs may offer significant benefits to future healthcare professionals. This paper presents the results of a comparative study of an interdisciplinary pre-health curriculum based in structural competency with a traditional premedical curriculum. The authors describe a new evaluation tool, the Structural Foundations of Health Survey ©, developed to evaluate structural skills and sensibilities. The authors use the survey to evaluate two groups of graduating seniors at Vanderbilt University—majors in an (...) interdisciplinary pre-health curriculum titled Medicine, Health, and Society, and premed science majors—with particular attention to understanding how political, cultural, economic, and social factors shape health. Results suggest that MHS majors identified and analyzed relationships between structural factors and health outcomes at higher rates and in deeper ways than did premed science majors. MHS students also demonstrated higher understanding of structural and cultural competency in their approaches to race, intersectionality, and racial health disparities. The skills that MHS students exhibited represent proficiencies increasingly emphasized by the MCAT, the AAMC, and other educational bodies that, in an era of epigenetics and social determinants, emphasize how contextual factors shape expressions of health and illness. Se ha realizado un análisis de contenido de las guías docentes mediante el software ‘ATLAS.ti’. Abstract:The concept of competency (or competence) entered educational policy by the end of last century. Due to this concept’s background in recruitment and because of its generic character, many definitions have been given, but so far no unified, transparent, and consistent definition has come forward. The concept of competency has also reached art education. Due to the “artistic” character of this domain of learning, this concept seems to be a powerful instrument to describe the types of behavior and (...) learning goals that are typical for this domain and for the related ways of thinking, working, and learning. This article gives a critical overview of what is seen as typical for what competency refers to in the domain of (art) education. A new definition of competency is presented that covers both the productive and the receptive domains of learning in visual art education. The constituent elements of this definition are discussed in detail. The analysis is broadened to include aspects of human cognition and behavior that can be considered as typical for visual art education, such as the aesthetic experience and its moral dimension. Advances in genomics are anticipated to contribute to the development of more effective, personalized approaches to the prevention and treatment of diseases. Recent studies that compare trends in the genetic curricula in the U.S. and Canadian medical schools have demonstrated inadequate incorporation of genomics material into the curriculum; as a result, the health care workforce is not adequately prepared to incorporate genomics into regular practice in the era of PM, thereby, slowing down the implementation of PM in the clinical settings. Moreover, advances in PM also raises ethical, legal and social implications (ELSIs) that requires good understanding of genomics as well as ethical principles in order to address issues and utilize this translation of genomic information into practice to provide patients, families, and communities with competent, safe, effective healthcare. From studies, it has been identified that there is a need for health provider education and competency rich in genomics and ethics. Lack of PCP’s knowledge in ordering genomic tests has been an indicator in lack of PCP’s confidence in ordering genomic tests for patients. The evaluation of competency in the field of psychology has gained international attention in recent years. Focusing on international competencies can promote high standards of care and identify common values of the profession. However, there is little research regarding international perceptions of competency. Examining ethics codes from different countries can highlight international standards of competent practice, as ethics codes outline professional expectations of the field. The current study compared the ethics standards found in 47 codes of ethics representing (...) 51 countries. Seventeen competency standards emerged after ethics codes were analyzed. This paper looks at the issue of consent from children and whether the test of Gillick competency, applied in medical and healthcare practice, ought to extend to participation in research. It is argued that the relatively broad usage of the test of Gillick competency in the medical context should not be considered applicable for use in research. The question of who would and could determine Gillick competency in research raises further concerns relating to the training of the (...) researcher to make such a decision as well as to the obvious issue of the researcher’s personal interest in the project and possibility of benefiting from the outcome. These could affect the judgment of Gillick competency if the researcher is charged with making this decision. The above notwithstanding, there are two exceptional research situations in which Gillick competency might be legitimately applied: when the research is likely to generate significant advantages for the participants while exposing them to relatively minor risks, and when it is likely to generate great societal benefit, pose minimal risks for the participants and yet raise parental objection. In both cases, to ensure that autonomy is genuinely respected and to protect against personal interest, Gillick competency should be assessed by an individual who has no interest or involvement in the research. This conceptual paper refines and expands the Geometry Pedagogical Improvement Cycle (GeoPIC), a framework designed to improve the teaching and learning of geometry. Rooted in the Van Hiele Theory of Geometric Thinking, GeoPIC was initially developed to integrate the strengths of both Conventional Van Hiele Phased Instruction and its technology-enhanced variant while addressing their contextual limitations. Building on prior empirical findings, the paper deepens the framework’s theoretical foundations, clarifies its six instructional phases, and illustrates how it aligns with the principles (...) of Competency-Based Curriculum (CBC). Additionally, it offers brief, practical classroom scenarios to illustrate the application of each phase. The refined framework provides a dynamic, adaptive, and learner-centered instructional approach that facilitates progression through Van Hiele levels while accommodating diverse classroom contexts. Despite the burgeoning literature on professionalism in other health professions, psychology lags behind in the level of attention given to this core competency. In this article, we review definitions from other health professions and how they address professionalism. Next, we review how this competency evolved within health service psychology (HSP), and we propose a definition. We offer an approach for assessing professionalism within HSP. Consideration is given to strategies and methods for providing effective education and training in this (...) multifaceted competency. As second generation immigrants, children of Chinese origin in Spain confront a complicated linguistic setting. The objective of this comparative sociolinguistic research, with the participation of 160 children of Chinese origin, is to analyze their sociolinguistic situation in Malaga (Spain), including both external and internal factors (i.e., socio-economic status, education level, language attitudes, identity, motivations, etc.) that affect their linguistic competency and learning. Our methodology is based on quantitative and qualitative data from questionnaires, observations, tests and interviews to explain (...) the linguistic patterns of immigrant children. Health research ethics training programmes are being developed and implemented globally, often with a goal of increasing local capacity to assure ethical conduct in health-related research. Yet what it means for there to be sufficient HRE capacity is not well-defined, and there is currently no consensus on outcomes that HRE training programmes should collectively intend to achieve. Without defining the expected outcomes, meaningful evaluation of individual participants and programmes is challenging. In this article, we briefly describe the evolution of formal (...) education in HRE, articulate the need for a framework to define outcomes for HRE training programmes, and provide guidance for developing HRE competency frameworks that define outcomes suited to their contexts. We detail critical questions for developing HRE competency frameworks using a six-step process: define the purposes, intended uses and scope of the framework; describe the context in which practice occurs; gather data using a variety of methods to inform the competency framework; translate the data into competencies that can be used in educational programmes; report on the competency development process and results and evaluate and update the competency framework. We suggest that competency frameworks should be feasible to develop using this process, and such efforts promise to contribute to programmatic advancement. (shrink)

The purpose of this paper was to examine moral judgment competency of a sample of 811 Chinese college students from a Southern university in mainland China with DIT-2; and test whether gender, education, major, academic performance, and one-child policy have significant effect on the participants’ moral judgment competency. Results show that the participants score high in both Personal Interest schema and Postconventional schema, while low in Maintaining Norms schema. Gender and major have significant effect on participants’ moral judgment (...) competency in China, while education level, academic performance and one-child family have no significant effect on their moral judgment competency. It is imperative to understand the factors that influence clinical competency. Consequently, it is essential to study those that have an impact on the process of attaining clinical competency. A grounded theory approach was adopted for this study. Professional competency empowers nurses and enables them to fulfill their duties effectively. Internal and external factors were identified as affecting clinical competency. A total of 36 clinical nurses, nurse educators, hospital managers and members of the Nursing Council in (...) Tehran participated in this research. Data were obtained by semistructured interviews. Personal factors and useful work experience were considered to be significant, based on knowledge and skills, ethical conduct, professional commitment, self-respect and respect for others, as well as from effective relationships, interest, responsibility and accountability. Effective management, education systems and technology were named as influential environmental factors. Personal and environmental factors affect clinical competency. Ethical persons are responsible and committed to their work, acquiring relevant work experience. A suitable work environment that is structured and ordered also encourages an ethical approach by nurses. (shrink)

Depending upon their nature, indiscretions can be classified as mistakes, misconduct, or improprieties. After committing an indiscretion, firms can attempt to reestablish positive stakeholder relationships by strengthening their technical competency (for mistakes), improving their reputation (for misconduct), and enhancing their goodwill with relevant stakeholders (for improprieties). Endorsing deliberative rationality, he furnishes no account of how to achieve it. Meyers argues that her conception of autonomy competency fills the gap in Rawls's theory. Moreover, it is compatible with the emotional bonds of a relational self, and, acknowledging human fallibility, it provides an account of how autonomous people can recognize and correct their missteps. In the context of (...) a critique of Michael Sandel's distinction between the cognitive self and the voluntarist self, Meyers shows that autonomy competency allows for individual control and innovation without denying the social situatedness of the autonomous subject. This is a qualitative descriptive study of how competency to take care of oneself and one's financial affairs was evaluated in New York City during the years 1989-1991 by the Human Resources Administration's Visiting Psychiatric Service . Most VPS clients are indigent senior citizens. A visit by VPS can result in forced institutionalization or lost control over one's finances. Data were collected from interviews with key informants , written materials about VPS, court cases and a report summarizing a recent (...) City Council investigation of VPS. Major problems concerning how VPS operated were found. VPS conducted competency evaluations in such a way that some people's rights may have been suspended inappropriately and others may not have been given the type of intervention they needed. These problems appear to stem from several interlocking factors: VPS operates with very little public exposure of its work. Most clients served had in essence little recourse to respond through legal channels to VPS recommendations that may have been based on faulty evaluations. In most cases the court approved the actions recommended by VPS. This study highlights dangers of the system and the need to change the way that competency is evaluated. (shrink)

Psychiatrists are the health care professionals most frequently called upon to determine the competency of a patient to refuse treatment. The motives for determining competency vary in morally significant ways. This paper explores what I term ‘the ideal motivational situation’ for determining a patient's competency: a desire to respect the patient's autonomy, a desire to promote the patient's overall best interests, and a belief that when these two motives conflict the patient's autonomy should not be dismissed out (...) of hand as a partial patient interest which is naturally outweighed by the totality of his or her interests. I claim that in a liberal, democratic society autonomy ought to trump best interests and be the sole criterion of patient competence. I conclude by offering an essentially aesthetic criterion for determining autonomy. (shrink)

A Heideggerian reading of J.H. van den Berg's writings contributes to an appreciation of phenomenological psychology as a cultural therapeutics. Both van den Berg's structural phenomenology of human existence and his Metablectic theory of historical changes lead to a notion of culture as a disclosive construction of the world. Our technological culture, in its reduction of all forms of relatedness to functionality (what van den Berg refers to as secularization), has repressed the spiritual dimension of contemporary life. The resultant derangement (...) of social existence gives rise to the individual distress brought in to psychotherapy. Attention to the spiritual unconscious is the ethical obligation of phenomenological psychology that transcends, and in so doing contextualizes considerations of professional competency. (shrink)
