Following some strange caravan, Lust Doll traces them back to the remote farm, where shady people have turned it into a slave-driven milk factory; the leader of the prisoners introduces herself as Bell.
Lust Doll helps the prisoners to organize and fight back, kicking the slavers out. Most decide to just leave, but some choose to stay, including Bell. The Milk Farm is now free, albeit feels a bit empty; maybe some of its former population can be convinced to return?

Despite Bell obviously lacking the breast to milk, Lust Doll suggests trying anyway... and Bell produces no milk at all. She confirms that it's actually always like this. Lust Doll decides to help her. Fortunately, they have some meanings to do so.
The first whom they try is Rinny, a succubus from Transylvania; succubi has access to transformational magic, so it should be easy for her to just enlarge Bell's breasts, right? Unfortunately, while successful at first, the process quickly reverts back, as if something is resisting any transformations. Rinny gives up: it's clearly some curse, and curses are out of her competence. Just in case, she teaches Lust Doll a breast-enlarging spell: if they find a way to lift the curse, it may come in handy.
The second one whom Lust Doll asks is Pixie; being a sorceress, she may know something about curse. After the initial blunder gets resolved (Bell is scared of Pixie, afraid that she may do something to her with her magic), Pixie checks Bell with magic... and realizes that while she's indeed cursed, Bell's curse is beyond her abilities, either.
Failing with dark magic, Lust Doll decides to try Bathhouse Temple: its water is known to cure any and all ailments. Bell's absent-mindedness again causes issues when she wanders off and then gets herself locked, requiring rescue, but ultimately issue gets resolved. The water actually works... right until Bell exits the bath stall, and her breasts shrink back. She obviously can't stay in the bathhouse forever, but an alternative solution gets suggested: there's an ancient spring that has magic water capable of permanently removing any and all curses; if that can't help, nothing would. But where to look for that spring...?
The chance comes when Lust Doll meets the merchant Alynn, who just so happened to have the old (and badly damaged) map leading to the old pyramid in the Desert. Together, they manage to find the lost pyramid, and retrieve what little is left of the magic water. When Bell receives the dosage of Water of Life, the curse finally lifts, rendering Bell once again susceptible to breast-enlargening magic.

Soon after restoring her chest (and ability to milk), while both Bell and Lust Doll were away from the farm, another disaster struck: nearly all inhabitants of the farm went missing... It can be slavers returning – but there's no sign of a struggle, as if they left on their own. Not sure what may be the reason, Lust Doll decides to spend a night at the farm, in case abductors (if it was indeed abduction) shows up again; to attract their attention, they disguised as the farm inhabitant.
At night, the UFO showed up, and abducted Lust Doll; turned out, aliens are mass-abducting cows for their milk – and they wouldn't take "no" for an answer. Lust Doll gets sudden help from Bell (who got at the UFO by some unknown means), who detaches them from the wall, but she can't free them from their restrains, meaning that Bell has to help them with moving around and operating various contraptions. Together, they manage to sneak past the alien patrols and reach the command post – where they learn that the aliens are looking for someone they call "a messiah", capable of giving them enough milk to sustain all their needs; and according to the scan results, it's Bell. They use some sort of lasers on Bell, making her breasts grow to insane size. But before it could get even weirder, Lust Doll intervened and suggested a solution which would satisfy all sides: the aliens set all prisoners free, but will gain free access to the farm and as much milk as they need. Bell gets reverted to her normal state, and the friends return home.
- During the first attempt to cure Bell's condition, Lust Doll brings her to Succubus Tower, where she gets tied to a cross and put under a breast-enlarging spell (ultimately in vain). Before the process can start, she has to be kissed by someone; the player has to pick between Incubus, Succubus Teacher, or Succubus Trainee.
- During the second attempt to cure Bell's condition, Lust Doll brings her to the Manor:
- On the way there, the two will go through Graveyard, where Bell will be harassed by Grave Hands; the player may free her, or participate in teasing her.
- As Bell is scared of Pixie, several options are tried to build trust; once all of them fail, Bell decides that if her friend trusts Pixie, that's enough for her. This scene is non-lewd and only serves story progression.
- During the third attempt to cure Bell's condition, Lust Doll brings her to the Bathhouse Temple.
- While stuck in the stall, Bell can be teased by not responding to her "who's here?" question, making her panic.
- Its waters will temporarily make her grow breasts... but as soon as she leaves the water, they shrink back.
After helping Bell with her lactation problems, and resolving the alien abduction crisis, Bell can be invited on a date. She's acting extremely anxious and prone to panic, unsure of everything: how should she behave, what should she wear, even whether she should talk more or less than she's doing now. Player can reassure her and just tell her to be herself, or mess with her. If you choose to mess with her, you can stop it early and tell her to be herself anyway, or tease her a bit further and ask her to treat you like a god before finally stopping (in which case, Bell will comment on you acting very mean to her).
Eventually, you two will arrive to New Ark City, where Bell will notice a pretty dress to her liking, but initially feel too awkward to buy it (it will look weird on a farm girl, and likely would quickly get dirty) until reassured by Lust Doll. While she's trying it on, the player can decide to either peek on her, or wait a bit. If you peep, she wouldn't mind, instead mistaking it for her taking too long to change. Once she finally comes out, you can either say that she's looking great, or that she's looking weird; if you don't call it weird, then Bell herself will, with Lust Doll either way deciding that they need to find some accessories to make her new look complete. Amongst other things, Bell will bring glasses; from now on, you can make her put them on or off, with the chance to do so immediately, making her use the chosen option for the rest of the date.
Unexpectedly, Bell asks to let her suggest the next stop, as her attempt to show more initiative as the "new Bell" she's trying to be. You're given three options, each marked as "???" (only revealing their location after trying once); you will visit all three of them, but you can decide in what order.
- First stop is the Police Station. What's more, it's specifically Arlene's desk. Arlene feels annoyed at your presence, and if you joke that you're indeed here to report a crime (that Bell just stole your heart), she will promptly arrest you, with Bell having to bail you out. Alternatively, you can just quietly leave before anything happens.
- Second stop is the Gym. Bell will ask you to teach her, with you having options to do so properly, teach her aggression, or take the chance to do so lewdly.
- Normal exercising will not cause Bell discomfort, but will also give you no scene.
- Aggressive exercising will make Bell impress the audience that gathered to watch the show, but will cause her run out of panic once she realises she's in the centre of attention.
- Lewd exercising will make her feel awkward due to people watching you groping her, but she will not panic, unlike aggressive training.
- Third stop is... the strip club. Surprisingly, Bell suggests to stay and give it a try. Only when she gets a lap dance, does she realise what place she went to, but feels too shy to stop it until it ends naturally.
- After trying all three options, you get the fourth "???" option. Bell will bring you to a park. Since lower levels of New Ark have no parks, you two quickly realise that it must be the Upper New Ark – and indeed, an UNA Guard soon shows up. If you're Indecent, it will notice it, giving you the chance to fix it, otherwise, you two have to run. Failing to find any logical explanation for how did they bypass the lengthy waiting lines at the entrance, Lust Doll realises that Bell must have some sort of teleportation powers. That also explains how did she get unto the UFO without aliens noticing her (as she wasn't a prisoner, unlike Lust Doll themselves). Maybe it has something to do with the curse she was suffering all this time? But that's a question for another time, for now, you two just spend the time together, before returning to the farm.
- If The dream quest is done, the player may hold her hand.
- After initial failure, the player may try to milk Bell as much as they want; it would be just as unsuccessful as the first attempt, but would have extra options, not available on the first session:
- By default, the following options are available:
- Pamper her
- Tickle her
- Tease and deny her
- Punish her
- Make her grow a dick (requires knowing the Dick Growing spell and have enough mana and the futa shrine enabled)
- Just wait till she finishes
- If the player is drunk, a special scene will trigger; such milking would be extra intense, and Bell will refuse to do another session during this day; she will reset after resting.
- After completing the Alien abduction quest she can be milked successfully. The player has the following options:
- Do it with her
- Make love to her
- Finger her from behind
- Wait till she's done
Bell is a sleepwalker, and may randomly wander off; the game rolls for that event each time the player sleeps. It can never happen if Bell is currently the only farm inhabitant (if you dismissed everyone, as two cowgirls always stay at the farm until you manually evict everyone).
She may be found at Forest Park, Transylvania, or New Ark City. On the first three wanderings, locations would be chosen in that order, but starting from the fourth wandering, they start randomizing. You can quickly check whether she's at specific location by going to the starting point of each zone: there will be the footprints, indicating that Bell recently was there. Specific locations are the following:
- Forest Park — north of the point where Rinny is initially trapped, being trapped herself; marked by an exclamation mark. Lust Doll may free her right away — or "punish" her with some tickle torture or spanking.
- Transylvania — on the streets in the eastern part of the town, surrounded by incubi. Lust Doll is free to save her right away... or join the fun, and then save her.
- New Ark City — in Police Station's prison. Lacks any scenes, but is the shortest to deal with.
In addition to the above, if Cassie is currently at the farm as part of her own random event, she may be asked to help, finding Bell immediately and skipping any events with her.
After she wanders off at least once, she can be gifted a Cowbell Collar, which would prevent her from running away again. Gifting her any other neck-slot clothes piece afterward would make her return the Cowbell Collar, reenabling this mini-quest.
If you find out about Bell's disappearance outside of the milk farm (for example, attempting to buy farm equipment will fail if she's missing), she will not appear in a wandering location until you have checked for her at the milk farm.
The event ceases to trigger once Bell receives the Water of Life, presumably it also cures her sleepwalking.

- She can only wear one item on her neck at the time; gifting her another makes her return the old one.
- Bell is the first character to be created specifically for Lust Doll+, with no counterpart in Lust Doll Classic.
- The plot about magic spring which can lift curses is a recycled story of the final Cassie's quest from Lust Doll Classic, where water was required to save Cassie from potentially-fatal illness.
