- Jack Smith refers to the Commonwealth of Virginia.[1] According to a Fallout 4 loading screen, Massachusetts, was also known as The Commonwealth of Massachusetts before the Great War.[2] In real life, both Virginia and Massachusetts, along with Kentucky and Pennsylvania, have been officially called Commonwealths since their inceptions in the 1780s-90s.[3]
- The Thirteen Commonwealths were formed in 1969, an event noted on the Boston placard at the Massachusetts State House.[4]
- The Southwest Commonwealth Conservation Area is written on the Red Rock Sign. Additionally the Southwest Commonwealth is written on the Nevada Flag.
- An unnamed commonwealth included the city of Watoga.[5]
- The New England Commonwealth is mentioned in the Fallout: The Roleplaying Game Core Rulebook.[Pub 1]

- In Fallout Bible 8, Tim Cain states: "Leon said he used that flag because it looked cool and he didn't want to use a standard American flag with 50 stars. Eventually he planned to make up something about 13 super-states or something, but he never did."[Dev 1]
- In the Denver design document, Joshua Sawyer mentioned breaking up the pre-war United States into thirteen zones, with one of them being called the Great Midwest Commonwealth.[Doc 1]
- In the tech demo for the canceled Van Buren project, Corporal Armstrong, the player interacts with a soldier from the Great Midwest Commonwealth Armored Infantry Division.[Doc 2][Doc 3]
- In 2004, Swayer partially wrote his own Fallout role-playing game that included the United States being reformed into 13 Commonwealths.
