Astarion is a male elf approximately 5'11" tall. As a high elf (likely a moon elf specifically) he has elongated, pointy ears and curly silver hair that is kept short and meticulously styled. His vampiric nature means he has a pale complexion, sharp fangs, red eyes and bite marks on the right side of his neck. He has broad shoulders and, while not the most muscular in BG3, does have clear muscle definition. Yes, this boy has abs and nice pecs. I'm sorry to destroy all your twink misconceptions, hunks can be effeminate fruits too.
Astarion is described in-universe multiple times as being a very beautiful man. As with all elves, he stopped aging in his 20's, although his facial features and tendency to hold all his tension in his face can make him look older. He has very prominent, high cheek bones, and sunken in eyes that cause perpetual dark circles and creases under them. He has visible laugh lines when he smiles and a pair of moles on his left cheek under his eye, one darker than the other. When he's displeased he tends to scrunch his face up and look kind of scrungly.
Astarion usually dresses in dandy adjacent fashion, often in jewel tones or blacks and whites. Lace, frills and floral embroidery are constants - much of which he will add himself. The clothes he arrived in Rosavelle in have been repaired in multiple spots over decades of use. Due to his condition and canon point, he must use a parasol whenever out during the day. He has several that he rotates through now. His skin is always cold to the touch and though he does not need to breathe, he does so anyway out of habit and to hide his condition. He uses a homemade perfume to mask any "undead" scent and so smells like bergamot and rosemary with a hint of brandy.
Lastly he has scars on his back that are actually part of an infernal contract. To the unfamiliar, it will look like some kind of Satantic ritual circle. They are clearly scars and not a tattoo. It is rare for him to let others see them though.
