After examination of the outer and middle ear, tympanometry (GSI 38 Auto
Tymp; Grason-Stadler Inc., Milford, NH, USA) was employed as a means of screening children for middle ear function.
Quantitative gene expression assays for excision repair (ERCC1), thymidine phosphorylase (
TYMP), stathmin (STMN1), ribonucleotide reductase (RRM1) and other genes provide valuable information to help predict chemotherapy responses.
Within the included operations, 5 patients had undergone bilateral surgery, 4 had a second look combined approach
tymp anoplasty, and further 4 patients had revision surgery or a conversion procedure (e.g., canal wall up converted to canal wall down).
Thymidine phosphorylase (
TYMP) is an enzyme involved in pyrimidine metabolism and also known to be a platelet-derived endothelial cell growth factor (PD-ECGF).
The Grason-Stadler
Tymp Star middle ear analyzer (Grason-Stadler; Eden Prairie, Minnesota) and MAICO EasyTymp (MAICO Diagnostic, GmbH; Eden Prairie, Minnesota) used in the study were calibrated annually.
MDDSs cause a reduction in cellular mtDNA content (2).At the present time, mutations in at least 9 genes [POLG, [4] polymerase (DNA directed), gamma; DGUOK, deoxyguanosine kinase; TK2, thymidine kinase, mitochondrial;
TYMP, thymidine phosphorylase; MPV17, MpV17 mitochondrial inner membrane protein; SUCLA2, succinate-CoA ligase, ADP-forming, beta subunit; SUCLG1, succinate-CoA ligase, alpha sub unit; RRM2B, ribonucleotide reductase M2 B (TP53 inducible); C10orf2, chromosome 10 open reading frame 2 (also known as TWINKLE)] have been found to cause mtDNA depletion (3-17).

In this study--a follow-up to an earlier study (N Engl J Med 2005; 353:576; discussed in the POEM "Early
tymp tubes do not improve outcomes after 3+ years," J Fam Pract 2005; 54:929)--429 children between the ages of 2 months and 3 years with middle-ear effusion for at least 90 days (bilateral) or 135 days (unilateral) were randomized to receive either prompt or delayed tympanostomy tube insertion.
Physicians prescribed antibiotics to 29% of 341 children in the "
Tymp Aware" group, and 27% of children in the "
Tymp Unaware" group.
