Early support can be crucial in suicide prevention, but many people experiencing suicidal thoughts lack access to the care and connection they need. Our Choose Tomorrow program uses data-driven insights to identify and connect with members at-risk of suicide to ensure they have access to vital care and available resources. Care managers conduct weekly outreach to simply ask "How are you?" and listen without judgment, providing the connectedness that can transform feelings of isolation into hope. Healthcare goes beyond medicine. After Sinai was diagnosed with leukemia and later developed Type 1 diabetes, her needs went well beyond medical treatment. Centene company Coordinated Care connected her with senior care manager Dawn to help navigate the complexities of her health challenges and other needs to succeed. Together, they worked to address her medical, housing, school, and transportation needs. With Dawn’s help, Sinai was able to get the physical therapy she needed, recover from four surgeries, get her driver's license, and now lives on her own while managing her care. In Los Angeles, many unhoused people struggle to get the care they need. It’s hard to keep doctor's appointments when you don’t have a way to get there or a safe place to sleep. That’s why Centene company, Health Net, and L.A. Care Health Plan are partnering to launch the L.A. County Field Medicine Program and the Skid Row Care Collaborative. To help change this, Centene company Arizona Complete Health partnered with the National Fitness Campaign and local organizations to create a free, 24/7 outdoor fitness court in Douglas, AZ where anyone can focus on their health, anytime. When Stephanie needed support for her 5-year-old son Noah, who has level three autism and required constant supervision, Meridian of Illinois care coordinator Joe worked with her to access resources like an adaptive bed, specialized stroller, and ABA therapy services.
