`TxPolicyViolationException` should contain `InnerException`

BlockChain<T>.Append() method currently throws TxPolicyViolationException without filling its InnerException if a transaction is invalid according to the BlockChain<T>.Policy. Although such behavior is not a bug, but it makes hard to investigate why it is invalid.
