A production environment is triggering the exception "Error Message: TypeError: Cannot read property 'insertBefore' of null".
This seems to happen on the admin interface on the url: "/#/admin/questionnaires" in relation to $scope.triggerFieldDialog
The setup is using version 3.0.2

Currently when I create a new profile it's not set as default. During command executions I also do not see what the profile is. As a result if I leave the --profile flag from the command I get errors that are quite strange and misleading. It would be good to:
- either set a default profile and see it on command executions
- or have some warnings / potential fixes with errors like: Are you runnin

Expected result
The "Hidden" text which replaces the token numbers is displayed as two lines if there's not enough room. This is annoying, for obvious reasons, more so if the phone happens to be horizontaly and vertically small. :-P
