Maybe I will get everything I need to Apply?

Ok so I sent in my PCQ back in June as soon as I had the time to fill it out. Well now I have the problem of I never got the information from DODMERB so that I can schedule my medical examies. So basicaly I was wondering who can I/ should I bug to get those forms or to see if there is a problem with my PCQ so that is why I never got the DODMERB?

Any help what so ever would be greatly appriciated. I have contacted my Liason, but only got a machine and so I am hoping one of you can help me.

I don't want to loose my chance because for some reason I never got the medical evaluations done.
