This paper explores the potential roles AI could play in interpreting, documenting, preserving, and making accessible intangible forms of cultural heritage for future generations. The paper starts by analysing the changing role of documentation in collections and archives (primarily museum archives and/or archives that include artworks). The paper then investigates what these changes in documentation may mean for archives, introducing also the notion of a dynamic archive. Finally, the paper utilises ChatGPT to explore how AI defines documentation, (...) how it states documentation can be used, and what research might be necessary to establish the responsible and trustworthy use of AI in this context. The two case studies explored through ChatGPT are Lynn Hershman Leeson’s Agent Ruby (1998-2002) and Blast Theory’s Cat Royale (2022). Our argument is that the use of AI in the documentation of intangible cultural heritage will impact not only its presentation, exhibition, and conservation, but also change the role that archives will play in years to come. A formal Ethics Consultation Service (ECS) can provide significant help to patients, families and hospital staff. As with any other form of clinical consultation, documentation of the process and the advice rendered is very important. Upon review of the published consult documentation practices of other ECSs, we judged that none of them were sufficiently detailed or structured to meet the needs and purposes of a clinical ethics consultation. Thus, we decided to share our method in order to advance (...) the practice of ethics consultation. Here, we describe a method of ECS documentation practice, including use of a formal consult report template, as well as a log for maintaining a chronological record of the consultations performed. These two documents facilitate order and organisation of the ECS. They also enable the ECS to keep an account of professional time and experience, enable quick consult trend assessments (by consult theme or ward, for example) and establish a potential registry of consults for future research study. This method of documentation, we believe, not only contributes significantly to the primary purpose of the consultation—namely, the evincing and sharing of ethical opinion about a case—but also enables consultants to improve their practice and to pursue research on clinical ethics consultation. This work addresses heritage themes based on the texts and reflections carried out during ourdoctoral studies in Information Science. Its objective is to discuss the silencing resulting frominstitutional and intellectual decisions about what constitutes heritage and how these choices can lead to the silencing and exclusion of collections and the communities that produced them. Conducted through bibliographic research, it explores the ideas presented by various authors and briefly contextualizes the discovery of the Dembos Archive and its journey until being inscribed (...) in UNESCO Memory of the World Program. It concludes that initiatives like the Memory of the World Register,when given the necessary visibility, can significantly contribute to the visibility of important collections and their respective communities. Healthcare ethics consultants in the Veterans Health Administration (VHA) document consults in an enterprise-wide web-based database entitled IEWeb, serving as a system of record for healthcare ethics documentation at 1300 VA facilities. The need arose to evolve the database from an ethics process training resource into a more streamlined documentation repository that captures essential consult elements. A VHA National Center for Ethics in Health Care (NCEHC) Improvement Team convened for three tasks: (1) Specify and prioritize IEWeb changes (occurred (...) via six focus groups composed of “new user” and “super user” cohorts with analysis of existing documentation patterns); (2) Pilot the changes regionally (via regional communication, training, and reviews of pre-post use patterns); and (3) Measure the impact of national change implementation on user perspectives (via pre-and post-change implementation polls). Focus groups identified six implementable priority areas for ethics consult documentation improvement, including the development of a usable consult summary note for ready conversion from IEWeb fields into the electronic health record. Post-IEWeb updates showed an increased number of consults documented, a reduction in “time to consult documentation closure” by a mean of 4.5 days, and a clinically-meaningful improvement in the quality of documentation (78% of ethics questions scored “above-bar” on the validation tool pre- vs. 89% scored “above-bar” post-IEWeb changes, n = 140). According to national survey findings, the number of consultants documenting “all” consults in IEWeb increased, satisfaction increased, and perception of documentation difficulty decreased. IEWeb simplification enabled ethics consultants to re-focus their documentation completion efforts by decreasing perception of documentation burden while improving documentation frequency and quality in a clinically-meaningful way. Stigmatizing language in medical documentation is a barrier to achieving patient‐centered care and health justice: it can contribute to patient distrust and alienation, and it appears more frequently in the charts of patients from marginalized groups, especially Black patients, impacting the future care they receive. One powerful avenue to address this issue is within undergraduate medical education; however, while some have investigated educational interventions to improve accuracy and technical proficiency in charting, there is significantly less focus on instruction surrounding (...) the ethical and social consequences of documentation. We contend that incorporating activities designed to elicit critical thinking about language, power, and structural injustice can encourage students to reflect upon and improve their documentation practices and can arm them with a critical lens through which to view the documentation generated by their peers and preceptors as they progress through training and establish the habits that will persist throughout their careers. In the course of his closing remarks at the Copenhagen Congress, the then President of the Association International des Études Byzantines reminded those who worried about the state of our field of “the rule that science may begin with imagination but … rests on factual documentation rather than conjecture”. As one who was (and is) less troubled about the way things were (and are) going, an art historian may be permitted to point to an example in which a document (...) can be shown to corroborate educated guesswork, and consequently to demonstrate one way in which peace between the two camps can (at least on this occasion) be achieved. The result may not be the absolute certainty demanded by our ancien président, but it comes as close to proof as anything available in a domain notorious for its lack of “factual documentation”. The method in question is triangulation—a third datum brought to bear on two previously known bodies of evidence—and the example that of the Middle Byzantine silks that display images of eagles. At issue is the belief that surviving examples were originally gifts offered to foreigners who in turn presented them to institutions or individuals of the Latin church. (shrink)

With reference to medieval England M. T. Clanchy argues that “the growth in the uses of literacy [between 1066 and 1307] is indicated by, and was perhaps primarily a consequence of, the production and retention of records on an unprecedented scale.” That is, rather than view literacy as a step on the road to literature, to learning and high culture, Clanchy stresses the interrelationship between literacy and administrative practice. Writing creates the possibility of documentation; documentation brings more people (...) into contact with the written word ; and, in time, as documentation becomes accepted as necessary, as mental habits adjust to it, as reliance upon it increases, so it in turn stimulates further expansion in the uses of literacy. Nowadays, Artificial Intelligence (AI) is present in all sectors of the economy. Consequently, both data-the raw material used to build AI systems- and AI have an unprecedented impact on society and there is a need to ensure that they work for its benefit. For this reason, the European Union has put data and trustworthy AI at the center of recent legislative initiatives. An important element in these regulations is transparency, understood as the provision of information to relevant stakeholders to support (...) their understanding of AI systems and data throughout their lifecycle. In recent years, an increasing number of approaches for documenting AI and datasets have emerged, both within academia and the private sector. In this work, we identify the 36 most relevant ones from more than 2200 papers related to trustworthy AI. We assess their relevance from the angle of European regulatory objectives, their coverage of AI technologies and economic sectors, and their suitability to address the specific needs of multiple stakeholders. Finally, we discuss the main documentation gaps found, including the need to better address data innovation practices (e.g. data sharing, data reuse) and large-scale algorithmic systems (e.g. those used in online platforms), and to widen the focus from algorithms and data to AI systems as a whole. Modes of rationality in nursing documentation: biology, biography, and the ‘voice of nursing’ This article is based on a discourse analysis of the complete nursing records of 45 patients, and concerns the modes of rationality that mediated text‐based accounts relating to patient care that nurses recorded. The analysis draws on the work of the critical theorist, Jürgen Habermas, who conceptualised rationality in the context of modernity according to two types: purposive rationality based on an instrumental logic, and value rationality (...) based on ethical considerations and moral reasoning. Our analysis revealed that purposive rationality dominated the content of nursing documentation, as evidenced by a particularly bio‐centric and modernist construction of the workings of the body within the texts. There was little reference in the documentation to central themes of contemporary nursing discourses, such as notions of partnership, autonomy, and self‐determination, which are associated with value rationality. Drawing on Habermas, we argue that this nursing documentation depicted the colonisation of the sociocultural lifeworld by the bio‐technocratic system. Where nurses recorded disagreements that patients had with medical regimes, the central struggle inherent in the project of modernity became transparent — the tension between the rational and instrumental control of people through scientific regulation and the autonomy of the subject. Generative artificial intelligence (AI) chatbots such as ChatGPT have several potential clinical applications, but their use for clinical documentation remains underexplored. AI-generated clinical documentation presents an appealing solution to administrative burden but raises new and old ethical concerns that may be overlooked. This article reviews the potential use of generative AI chatbots for purposes such as note-writing, handoffs, and prior authorisation letters, and the ethical considerations arising from their use in this context. AI-generated documentation may offer standardised (...) and consistent documentation across encounters but may also embed biases that can spread across clinical teams relying on previous notes or handoffs, compromising clinical judgement, especially for vulnerable populations such as cognitively impaired or non-English-speaking patients. These tools may transform clinician–patient relationships by reducing administrative work and enhancing shared decision-making but may also compromise the emotional and moral elements of patient care. Moreover, the lack of algorithmic transparency raises concerns that may complicate the determination of responsibility when errors occur. To address these considerations, we propose notifying patients when the use of AI-generated clinical documentation meaningfully impacts their understanding of care, requiring clinician review of drafts, and clarifying areas of ambiguity to protect patient autonomy. Generative AI-specific legislation, error reporting databases and accountable measures for clinicians and AI developers can promote transparency. Equitable deployment requires careful procurement of training data representative of the populations served that incorporate social determinants while engaging stakeholders, ensuring cultural sensitivity in generated text, and enhancing medical education. Researchers seeking to engage in projects related to Tribal communities and their citizens, lands, and non-human relatives are responsible for understanding and abiding by each Tribal nation’s research laws and review processes. Few studies, however, have described the many diverse forms of Tribal research review systems across the United States (US). This study provides one of the most comprehensive examinations of research review processes administered by Tribal Research Review Boards (TRRBs) in the US. Through a systematic analysis, we consider TRRBs’ (...) online presence, online documentation, and themes across documents, for five entity types: Tribal nations and Tribal consortiums, Tribal colleges and universities, Tribal health organizations, Indian Health Services, and other Tribal organizations. Results include an assessment of online presence for 98 potential TRRBs, identification of 118 publicly available online documents, and analysis of 41 themes across four document types: Tribal research codes and TRRBs’ guidelines, applications, and post-approval documents. Altogether, this research provides a macro-level analysis of the most common types of TRRBs in the US in an effort to increase researchers’ understanding of these important processes as they prepare to ethically engage Tribal communities in research. These results aim to empower Tribal decision makers as they align their TRRBs’ online visibility and documentation with community priorities and strengthen their protections for the rights and wellbeing of their citizens and community. Ultimately, by expanding our knowledge of TRRBs across the US, this contribution seeks to uphold Tribal sovereignty in research and promote ethical approaches to research with Indigenous communities. Modern Western medical individualism has had a significant impact on health care in China. This essay demonstrates the ways in which such Western-style individualism has been explicitly endorsed in China’s 2010 directive: The Basic Norms of the Documentation of the Medical Record. The Norms require that the patient himself, rather than a member of his family, sign each informed consent form. This change in clinical practice indicates a shift toward medical individualism in Chinese healthcare legislation. Such individualism, however, is (...) incompatible with the character of Chinese familism that is deeply rooted in the Chinese ethical tradition. It also contradicts family-based patterns of health care in China. Moreover, the requirement for individual informed consent is incompatible with numerous medical regulations promulgated in the past two decades. This essay argues that while Chinese medical legislation should learn from relevant Western ideas, it should not simply copy such practices by importing medical individualism into Chinese health care. Background To characterize patients dying in a community hospital with or without attempting cardiopulmonary resuscitation and to describe patient involvement in, documentation of, and compliance with decisions on resuscitation. Methods All patients who died in Kalmar County Hospital during January 1, 2016 until December 31, 2016 were included. All information from the patients’ electronic chart was analysed. Results Of 660 patients female), 30 were pronounced dead in the emergency department after out-of-hospital CPR. Of the remaining 630 patients a DNAR (...) order had been documented in 558 patients. Seventy had no DNAR order and 2 an explicit order to do CPR. In 43 of these 70 patients CPR was unsuccessfully attempted while the remaining 27 patients died without attempting CPR. In 2 of 558 patients CPR was attempted despite a DNAR order in place. In 412 patients the DNAR order had not been discussed with neither patient nor family/friends. Moreover, in 75 cases neither patient nor family/friends were even informed about the decision on code status. Conclusions In general, a large percentage of patients in our study had a DNAR order in place. However, 27 patients died without CPR attempt or DNAR order. DNAR orders had not been discussed with the patient/surrogate in almost three fourths of the patients. Further work has to be done to elucidate the barriers to discussions of CPR decisions with the patient. (shrink)

This paper discusses the issue of informed consent from an African perspective with a particular emphasis on the problems posed by the documentation of consent in the African socio-cultural environment. The paper presents two small-scale surveys which typify and exemplify the African perspective with regard to procedures for obtaining consent (agreement) and for documenting it. To avoid causing moral pain to African research participants and the feeling of having been used as mere sources of data, this paper suggests, as (...) a short-term solution, that African cultural values be incorporated in the procedures and regulations aimed at protecting their human rights. As a long-term solution, the paper encourages universities and research institutions operating in Africa to come up with clear guidelines of ethical conduct which would satisfy both the interests of ordinary Africans (particularly the rural and uneducated population) and legal requirements to which Western research institutions and funding agencies are subjected. The OpenNotes (ON) mandate in the 21st Century Cures Act requires that patients or their legally authorized representatives be able to access their medical information in their electronic medical record (EMR) in real time. Ethics notes fall under the domain of this policy. We argue that ethics notes are unique from other clinical documentation in a number of ways: they lack best-practice guidelines, are written in the context of common misconceptions surrounding the purpose of ethics consultation, and often answer (...) questions of a different nature than other documentation. Thus, we believe the clinical ethics community would benefit from clarification on when the withholding of ethics notes is justified. We provide recommendations for excluding information from ethics documentation based on the likelihood and magnitude of harm that may occur with particular disclosures and suggest approaches to decrease the potential harms that may occur. We define and explain six types of reasons to exclude information from ethics notes based on significant harms that are not addressed in the ON policy: (1) harmful revelations from a protected chart note; (2) negative emotional effects on patients or families; (3) the purpose of the consultation is undermined by harmful consequences; (4) avoidable negative impact on interpersonal dynamics; (5) inappropriate labeling or disclosure of medical, social, or financial information; and (6) inclusion of biasing or otherwise unfair information. We also suggest approaches to mitigate harm when excluding, including, reframing, or delaying release of information that is perceived to be relevant to an ethics case. Overall, we hope our analysis and recommendations will initiate a much-needed discussion about the impact of the ON mandate on clinical ethics documentation. (shrink)

The use of wireless, electronic, medical records and communications in the prehospital and disaster field is increasing. Objective: This study examines the role of wireless, electronic, medical records and com- munications technologies on the quality of patient documentation by emergency field responders during a mass-casualty exercise.

In mining, manufacturing and industrial process industries, maintenance procedures are used as an aid to guide technicians through complex manual tasks. These procedures are not machine-readable, and cannot support reasoning in digitally integrated manufacturing systems. Procedure documents contain unstructured text and are stored in a variety of formats. The aim of this work is to query information held in real industrial maintenance procedures. To achieve this, we develop an ontology for maintenance procedures using the OWL 2 description language. We leverage (...) classes and object properties from the ISO 15926 Part 14 Upper Ontology and create a domain ontology. The key contribution of this paper is a demonstration of trade-offs required when modelling an existing engineering artifact, where an abstraction of its contents is given a-priori. We provide an ontologically rigorous abstraction of notions captured in procedure documentation to a set of classes, relations and axioms that allow reasoning over the contents. Validation of the ontology is performed via a series of competency questions based on queries relevant to technicians, engineers and schedulers in industry. Healthcare organizations and workers are under pressure to produce increasingly complete and accurate data for multiple data-intensive endeavors. However, little research has examined the emerging occupations arising to carry out the data work necessary to produce “improved” data sets, or the specific work activities of these emerging data occupations. We describe the work of Clinical Documentation Integrity Specialists, an emerging occupation that focuses on improving clinical documentation to produce more detailed and accurate administrative datasets crucial for evolving data-intensive (...) forms of healthcare accountability, management, and research. Using ethnographic methods, we describe the core of CDIS’ work as a translation practice in which the language, interests, and concerns of clinicians and clinical documentation are translated via real-time “nudging” and ongoing education of clinicians into the language, interests, and concerns of medical coders, structured administrative datasets, and the various stakeholders of these datasets. Further, we show how the institutional context of CDIS’ work shapes the occupational virtues that guide CDIS’ translation practice, including financial reimbursement, quality measures, clinical accuracy, and protecting clinician’s time. Despite the existence of these multiple virtues, financial reimbursement is the most prominent virtue guiding CDIS’ limited attention. Thus, overall clinical documentation is “improved” in specific, partial ways. This research provides one of the first studies of the emergent data work occupations arising in the wake of digitization and big data opportunities, and shows how local data settings shape large scale data in specific ways and thus may influence outcomes of analyses based on such data. Background: Since the passage of the Patient Self-Determination Act, numerous policy mandates and institutional measures have been implemented. It is unknown to what extent those measures have affected end-of-life care, particularly with regard to the do-not-resuscitate order.Methods: Retrospective cohort study to assess associations of the frequency and timing of DNR orders with advance directive status, patient demographics, physician’s specialty and extent of documentation of discussion on end-of-life care.Results: DNR orders were more frequent for patients on a medical service than (...) on a surgical service and were made earlier in the hospital stay for medicine than for surgical patients . 22.18% of all patients had some form of an advance directive in their chart, yet this variable had no impact on the frequency or timing of DNR ordering. Documentation of DNR discussion was significantly associated with the frequency of DNR orders and the time from DNR to death .Conclusions: The physician’s specialty continues to have a significant impact on the frequency and timing of DNR orders, while advance directive status still has no measurable impact. Additionally, documentation of end-of-life discussions is significantly associated with varying DNR ordering rates and timing. (shrink)

This chapter explores the linguistic diversity of Nagaland and examines the various challenges, including linguistic and non-linguistic factors that field linguists encounter during documentation and fieldwork. Nagaland, the 26th largest state in India, spans 16,579 square kilometres and yet, the state is home to a remarkably high number of indigenous languages. According to the Census of India (2011), the state accommodates twenty-five major languages, along with several undocumented languages, making it a hub of linguistic diversity. The region’s complex cultural (...) and geographical landscape, characterized by mountainous terrain, limited inter-village communication, and traditional practices such as headhunting, agricultural self-sufficiency, and reliance on indigenous medicinal herbs, further complicates linguistic documentation efforts. This chapter draws from firsthand experiences in linguistic data collection across Nagaland and other northeastern states, as well as extensive engagement with multiple indigenous language communities in Nagaland. This paper attempts to analyse the images that animated a political movement called the Odessa Collective in Kerala since 1984. It produced six films – Amma Ariyan (A Report to Mother, 1986), Ithrayum Yathabhagam (Journey So Far, 2003), Vettayadapetta Manasu (Haunted Mind, 2006), Mortuary of Love (2009), Agnirekha (Line of Fire, 2011) and Holy Cow (2015). This paper tries to argue that the twenty-two years of this movement's politics can be studied as an assemblage of the man and machine in (...) a Deleuzian framework on cinema of the Third World. It tries to conceptualise the linkages between the Cinéma Vérité of Jean Rouch and the Third Cinema of Solanas and Getino or the milieu of the political which creates realistic documentation in cinema. It looks at the concepts like desire and representation that intersect between these two different genres in order to reconceptualise the political in relation to Deleuze on cinema. (shrink)

The ethics of describing and indexing works which have an element of deception is an important topic within the arena of knowledge organization (KO). However, what happens if the unreal element is for artistic purposes and is part of the experience of the document? The focus of this article is on the KO of art documentation. It considers art documents where unreality and mistruth are part of a document’s creative and artistic purpose. Three examples of exhibition documentation for (...) contemporary art are explored. These illuminate the KO implications of unreal elements in artworks, exhibitions and documents, and the interplay between them. Three models are then presented relating to the KO of unreality. Model one shows how the wishes of the creator are a significant part of ethical KO for art and the ethical KO decisions which result from the performative and experiential aspects of art documentation. Model two illustrates how the placement of the unreality in relation to metadata creation can have a critical impact on ethical decisions in KO. Model three posits unreal-ness as a novel type of information about metadata creation, introducing categories such as motive for the deception. It shows how this original way of contemplating applied ethics in KO is vital for all ethically challenging works. Ultimately, considering the KO ethics of art documentation extends our thinking about how to deal with deception and unreality and adds an important aesthetic-ethical dimension to the corpus of work on KO ethics. The semantic web ontology languages RDFS and OWL are widely used but limited in both their expressivity and their support for modularity and integrated documentation. Expressivity, modularity, and documentation of formal knowledge have always been important issues in the MKM community. Therefore, we try to improve these ontology languages by well-tried MKM techniques. Concretely, we propose OM- Doc as an alternative. We show how OMDoc can be made compatible with semantic web ontology languages, focusing on knowledge representation, modular (...) design, documentation, and metadata. We evaluate our technology by re-implementing the Friend-of-a-friend (FOAF) ontology and applying it in a novel metadata framework for technical documents (including ontologies). (shrink)

Background Discussions on ethical aspects of life-and-death decisions within the hospital are often made in plenary. The prehospital physician, however, may be faced with ethical dilemmas in life-and-death decisions when time-critical decisions to initiate or refrain from resuscitative efforts need to be taken without the possibility to discuss matters with colleagues. Little is known whether these considerations regarding ethical issues in crucial life-and-death decisions are documented prehospitally. This is a review of the ethical considerations documented in the prehospital medical records (...) of patients in a Danish prehospital setting for whom the decision to resuscitate or not was made at the scene. Methods The study is based on discharge summaries of all patients subjected to crucial life-and-death decisions by the Mobile Emergency Care Unit in Odense in the years 2010 to 2014. The medical records with possible documentation of ethical issues were independently reviewed by two philosophers in order to identify explicit ethical or philosophical considerations pertaining to the decision to resuscitate or not. Results In total, 1275 patients were either declared dead at the scene without exhibiting layman’s reliable signs of death or admitted to hospital following resuscitation. In a total of 62 patients, 85 specific ethical issues related to resuscitation were documented. The expressions of the ethical considerations were generally vague or unclear and almost exclusively concerned the interests of the patient and not the relatives. In the vast majority of cases where an ethical content was identified, the ethical considerations led to a decision to terminate treatment. Conclusions A strengthened practice of documenting ethical considerations in prehospital life-and-death decision-making in the patient’s medical records is required. We suggest that a template be implemented in the prehospital medical records describing the basis for any ethical decisions. This template should contain information regarding the persons involved in the deliberations and notes on ethical considerations. The documentation should include considerations concerning the patient’s end-of-life wishes, the estimations of the quality of life before and after the incident, and a summary of other ethical concerns taken into account, such as the integrity of the patient and frame of mind of relatives. The theory of document acts is an extension of the more traditional theory of speech acts advanced by Austin and Searle. It is designed to do justice to the ways in which documents can be used to bring about a variety of effects in virtue of the fact that, where speech is evanescent, documents are continuant entities. This means that documents can be preserved in such a way that they can be inspected and modified at successive points in time and (...) grouped together into enduring document complexes. We outline some components of a theory of document acts, and show how it can throw light on certain problems in Searle’s ontology of social reality. (shrink)

There are two important ways in which, when dealing with documents, we go beyond the boundaries of linear text. First, by incorporating diagrams into documents, and second, by creating complexes of intermeshed documents which may be extended in space and evolve and grow through time. The thesis of this paper is that such aggregations of documents are today indispensable to practically all complex human achievements from law and finance to orchestral performance and organized warfare. Documents provide for what we can (...) think of as a division of intellectual, instructional, and deontic labour, allowing plans, orders, and obligations to be enmeshed together in a way that often involves the use of diagrammatic elements, as for example in a musical score.
