Abstract
Healthcare systems worldwide face significant challenges, including the rising prevalence of chronic diseases, ageing populations, workforce shortages, and sustainability issues. Care journeys are often plagued by fragmentation and inefficiencies, negatively affecting clinical outcomes, patient experiences, and the sustainability of care. Digital health innovations offer transformative potential across the care continuum, from prevention to ongoing monitoring and follow-up. This chapter examines how digital innovations can enhance care journeys by improving prevention strategies, streamlining screening and diagnosis, optimising treatment and rehabilitation processes, and enabling effective monitoring and follow-up care. By integrating digital health solutions into these key areas, we envision a more connected, efficient, and human-centred healthcare. As we enhance primary prevention through digital innovations, we also lay the foundation for a paradigm shift from reactive to preventive care—emphasising a holistic approach that prioritises wellness and sustainability and the transformation towards digitally enhanced care.
