Some ‘naturalist’ accounts of disease employ a biostatistical account of dysfunction, whilst others use a ‘selected effect’ account. Several recent authors have argued that the biostatistical account offers the best hope for a naturalist account of disease. We show that the selected effect account survives the criticisms levelled by these authors relatively unscathed, and has significant advantages over the BST. Moreover, unlike the BST, it has a strong theoretical rationale and can provide substantive reasons to decide difficult cases. Accounts of the concepts of function and dysfunction have not adequately explained what factors determine the line between low‐normal function and dysfunction. I call the challenge of doing so the line‐drawing problem. Previous approaches emphasize facts involving the action of natural selection (Wakefield 1992a, 1999a, 1999b) or the statistical distribution of levels of functioning in the current population (Boorse 1977, 1997). Despite many authors in psychiatry endorsing a naturalist view of the mind, many still consider that mental dysfunctions cannot be reduced to brain dysfunctions. This paper investigates the main reasons for this view. Some arguments rely on the analogy that the mind is like software while the brain is like hardware. The analogy suggests that just as software can malfunction independently of hardware malfunctions, similarly the mind can malfunction independently of any brain malfunction. This view has been critically examined in (...) recent literature. However, other less discussed reasons suggest that mental dysfunctions cannot be reduced to brain dysfunctions because mental dysfunctions are appropri- ately ascribed at the level of intentional mental states, while brain dysfunctions are solely related to abnormalities in anatomy and physiological processes. This paper questions why such a view would be upheld. The discussion is framed within the interface problem in the philosophy of cognitive science, which concerns the relationship between personal and subpersonal levels of explanation. The paper examines the view that an autonomist perspective on the personal/ subpersonal distinction could justify the separation of mental dysfunctions, described in intentional terms, from brain dysfunctions, described in anatomical or physiological terms. Ultimately, the paper argues that the autonomist view cannot be upheld in psychiatry and, consequently, does not provide a principled justification for rejecting the reduction of mental dysfunctions to brain dysfunctions. This paper criticizes Jerome Wakefield’s harmful dysfunction analysis of disorder by arguing that the conceptual linkage it establishes between the medical concepts of health and disorder and the prudential notions of well-being and harm makes the account inapplicable to nonsentient organisms, such as plants, fungi, and many invertebrate animals. Drawing on a previous formulation of this criticism by Christopher Boorse, and noting that Wakefield could avoid it if he adopted a partly biofunction-based account of interests like that often advocated (...) in the field of environmental ethics, I argue that integrating such an account of interests into the HDA would generate serious concerns. Specifically, it would make dysfunction sufficient for disorder and so reestablish between dysfunction and disorder precisely the kind of sufficiency relation that harm-requiring accounts of disorder strive to avoid; blur the line between the HDA’s dysfunction and harm components and, in so doing, deprive the HDA of its alleged main advantage over monistic dysfunction-based accounts of disorders like Boorse’s; and tie the HDA to an understanding of harm that is in itself problematic. I argue that these three concerns, and the dilemmas they generate, rob the HDA of much of its prima facie appeal, ultimately indicating that a satisfactory account of disorder should most likely eschew all references to prudential notions of well-being and harm. Scientists, philosophers, and even the lay public commonly accept that schizophrenia stems from a biological or internal ‘dysfunction.’ However, this assessment is typically accompanied neither by well-defined criteria for determining that something is dysfunctional nor empirical evidence that schizophrenia satisfies those criteria. In the following, a concept of biological function is developed and applied to a neurobiological model of schizophrenia. It concludes that current evidence does not warrant the claim that schizophrenia stems from a biological dysfunction, and, in (...) fact, that unusual neural structures associated with schizophrenia may have functional or adaptive significance. The fact that current evidence is ambivalent between these two possibilities (dysfunction versus adaptive function) implies that schizophrenia researchers should be much more cautious in using the ‘dysfunction’ label than they currently are. This paper presents the interpersonal variability of harm challenge to Jerome Wakefield’s harmful-dysfunction account (HDA) of disorder. This challenge stems from the seeming fact that what promotes well-being or is harmful to someone varies much more across individuals than what is intuitively healthy or disordered. This makes it at least prima facie difficult to see how judgments about health and disorder could, as harm-requiring accounts of disorder like the HDA maintain, be based on, or closely linked to, judgments about (...) well-being and harm. This interpersonal variability of harm challenge is made salient by the difficulty faced by harm-requiring accounts of disorder in dealing satisfactorily with cases of intuitively disordered conditions that seem harmless because they do not deprive the individuals that they affect of anything that they value (e.g., desired infertility). I argue that this challenge is made more serious for the HDA by some clarifications Wakefield has recently made on harm. In recent publications, Wakefield dissociates himself from the sheer cultural-relativist view of harm attributed to him by some critics based on his linkage of harm to social values, and adopts a more qualified social-values-based view of harm that leaves room for criticism of the values endorsed by members of a cultural group at a given time. I show how Wakefield’s qualified view makes it more difficult for the HDA to deal with the interpersonal variability of harm challenge, at least when applied to a Western cultural context where a high value is placed on autonomy and individual choice. This paper introduces the concept of the functional ambivalence of mental symptoms as a contribution to Enactive Psychiatry. Drawing on the autonomy-centered foundations of the enactive approach, we argue that mental disorders are not purely dysfunctional, but complex, multi-scale patterns of organization that can display functional and dysfunctional aspects at once. Functionality is understood as graded and context-dependent, emerging from interactions among multiple domains of normativity and across temporal scales. In dialogue with Russell’s perspective problem, we argue that the ambiguity (...) of functional evaluation is not a shortcoming but a predictable consequence of plural autonomy. Our proposal is intended to be complementary to related theses developed in other frameworks. These perspectives can help clarify why certain symptoms persist and which meaningful and self-regulatory roles they may serve. Using anorexia, self-harm, and dietary restraint as examples, we show how behaviors typically treated as dysfunctional can play functional roles even as they generate harm. Clinically, the framework supports interventions that identify what symptoms do for the person while fostering alternative and more sustainable forms of regulation. Ethically, it promotes a less pathologising understanding of mental disorder by integrating first-person perspectives that can disclose functional dimensions often missed by third-person evaluation. This paper applies the Coherence Engine (CE) framework, a general architectural theory of consciousness developed through phenomenological observation and described in full elsewhere (Sowell, 2026a), to the domain of mental health dysfunction and recovery. The CE framework proposes that consciousness is produced by five functional modules: the Prediction Engine, the Orchestrator, the Gating System, the Error Integration Loop, and the Core Self Modulator (CSM), operating together to maintain a unified, prediction-generating, self-modeling subject. This paper argues that mental health (...) class='Hi'>dysfunction is not a collection of discrete biological disorders but a set of identifiable architectural failures, each occurring at a specific depth and through a specific mechanism. The framework is engaged with the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) throughout: where the DSM’s symptom-cluster descriptions correspond to architectural predictions, that correspondence is noted; where the architectural account reveals mechanisms the DSM does not explain or reframes conditions the DSM pathologizes, those divergences are examined directly. The paper develops architectural accounts of mood disorders, anxiety, trauma and PTSD, obsessive-compulsive disorder, personality disorders, dissociative identity disorder, reactive attachment disorder, autism spectrum condition, addiction, schizophrenia spectrum conditions, attention deficit hyperactivity disorder, eating disorders, grief, sleep disorders, substance-induced states and withdrawal, and somatic symptom disorders. Autism and ADHD are reframed as Gating System calibration variants rather than deficits. A depth-of-compilation framework is developed to explain why different interventions reach different conditions, and why insight frequently fails to produce durable change. Counterfactual thinking—the capacity to reflect on what would, could, or should have been if events had transpired differently—is a pervasive, yet seemingly paradoxical human tendency. On the one hand, counterfactual thoughts can be comforting and inspiring (Carroll & Shepperd, Chapter 28), but on the other they can be anxiety provoking and depressing (Zeelenberg & Pieters, Chapter 27). Likewise, such thoughts can illuminate pathways toward better future outcomes (Wong, Galinsky, & Kray, Chapter 11), yet they can also promote confusion and lead (...) us astray (Sanna, Schwarz, & Kennedy, Chapter 13). The first part of this chapter focuses on work that supports the prevailing Zeitgeist in the counterfactual thinking literature: Counterfactual thinking is beneficial. The second part of the chapter, however, strikes a more cautionary tone by reviewing work that describes some deleterious consequences of counterfactual thinking. We conclude by offering a tentative reconciliation of these conflicting perspectives and suggesting directions for future research. (shrink)

This treatise proposes a comprehensive diagnostic framework for Social Media Syndrome (SMS), conceptualized as a constellation of psychological, cognitive, and behavioral disturbances arising from moderate to excessive social media engagement. Drawing upon empirical research in neuropsychology, developmental psychology, and clinical psychology, alongside theoretical frameworks from Jungian depth psychology and psychodynamic theory, this paper examines the mechanisms through which social media platforms induce pathological states. The analysis integrates three domains of dysfunction: (a) affective dysregulation encompassing anxiety, depression, and mood instability; (...) (b) cognitive impairment, including attention fragmentation, reduced executive function, and diminished critical thinking capacity; and (c) identity pathology characterized by persona proliferation, false self-development, and individuation arrest. Central to this framework is the dopaminergic reward system's hijacking through variable-ratio reinforcement schedules, social comparison mechanisms, and the externalization of self-worth to quantified metrics. This paper argues that SMS represents a novel form of psychopathology warranting clinical recognition, distinct from but related to internet addiction disorder. Proposed diagnostic criteria, theoretical grounding in the False Self-Persona Proliferation Framework, and intervention recommendations are presented. The treatise concludes that SMS constitutes a structural assault on ego coherence and individuation, requiring multi-level intervention across individual, cultural, and technological domains. Democratic societies display epistemic dysfunctions that obstruct the ability of citizens to act as informed and competent political decision-makers. In this paper I contend that said dysfunctions are not merely undesirable but rather undercut democratic legitimacy. However, I explain this in terms that diverge from the common instrumentalist approach, which focuses on their impact on the quality of political outcomes. I argue, instead, that said dysfunctions have delegitimizing effects because they undercut the institutional integrity of practices such as voting and (...) political deliberation. They do so by thwarting the ability of these practices to achieve their own normative rationale: a proper joint co-authorship of political decisions by citizens. This amounts to a corrosion of the normative conditions that are meant to confer authoritativeness to democratic decision-making that is normatively significant regardless of its ultimate impact on the quality of political outcomes. According to Jerome Wakefield’s harmful dysfunction account, a mental disorder must involve an objective dysfunction couched in evolutionary terms. However, selected effects functions are indeterminate because (i) the same trait can be both selectively advantageous and disadvantageous, and (ii) the functional activity of a trait can be assessed according to conflicting norms, given the trait’s place in a hierarchy of functions. Therefore, there may be a dysfunction that can be described in multiple empirically adequate ways. The choices (...) involved in these cases are value-laden. One way in which bioethicists can benefit the medical community is by clarifying the concept of disorder. Since insurance companies refer to the DSM for whether a patient should receive assistance, one must consider the consequences of one’s concept of disorder for who should be provided with care. I offer a refinement of Jerome Wakefield’s hybrid concept of disorder, the harmful dysfunction analysis. I criticize both the factual component and the value component of Wakefield’s account and suggest how they (...) might be improved. I propose that the factual component should be statistical variation analyzed in multilevel and chronological compilations of physiological data. I propose that the value component should prioritize the individual’s authority regarding the experience of suffering from a physiological condition. My account preserves the insight of using a factual and a value component while avoiding the problems that Wakefield’s original account faces. (shrink)

This comparative study explores how Wole Soyinka’s Chronicles from the Land of the Happiest People on Earth (2021) and Chimamanda Ngozi Adichie’s Dream Count (2025) deploy language to portray societal dysfunction in Nigeria. Drawing on theoretical frameworks of Leo Hickey’s pragma-stylistics and Biber and Conrad’s register variation, this paper applies a content analysis method to systematically investigate narrative episodes, dialogue, and descriptive passages, revealing the authors’ stylistic choices. Key findings show how both authors reflect and critique issues of corruption, (...) gender norms, and diaspora challenges through their linguistic choices. This study contributes to sociolinguistic stylistics by offering a comparative model for analysing postcolonial texts, and enriches postcolonial literature studies by highlighting how language is a fundamental tool for articulating and critiquing power structures and social disorder. High blood pressure is an important component of pre-eclampsia. The underlying mechanism of development of hypertension in pre-eclampsia is complicated and still remains obscure. Several theories have been advanced including endothelial dysfunction, uteroplacental insufficiency leading to generalized vasoconstriction, increased cardiac output, and sympathetic hyperactivity. Increased blood flow and pressure are thought to lead to capillary dilatation, which damages end-organ sites, leading to hypertension, proteinuria and edema. Additional theories have been put forward based on epidemiological research, implicating immunological and genetic (...) factors. None of these theories have been substantiated. Based on a review of literature this paper postulates that the initiating event for the development of preeclampsia is intermittent hypoxia associated with irregular breathing during sleep, hypoapnea, apnea,inadequate respiratory excursions during the waking hours and inadequate cardiopulmonary synchronization (abnormal sympatho-vagal balance). Major Depressive Disorder (MDD) is a complex neuropsychiatric condition characterized by widespread alterations in neural circuitry, neurotransmission, and neuroendocrine regulation. This review integrates findings from neuroimaging, molecular, and pharmacological studies to elucidate the neurobiological substrates of MDD, with particular emphasis on fronto-limbic network dysfunction, hypothalamic-pituitary-adrenal (HPA) axis dysregulation, and serotonergic signaling abnormalities. Convergent evidence from resting-state and task-based functional MRI indicates reduced functional connectivity within prefrontal-limbic circuits, including the medial prefrontal cortex, anterior cingulate cortex, amygdala, hippocampus, and hypothalamus, alongside (...) impaired top-down regulation of emotional processing. At the neurochemical level, MDD is associated with serotonergic hypoactivity, altered receptor dynamics (notably 5-HT1A and 5-HT2A), and disrupted monoamine transport, as well as emerging dysregulation in glutamatergic and GABAergic systems that contribute to excitatory-inhibitory imbalance. Additionally, reduced expression of brain-derived neurotrophic factor (BDNF) and impaired synaptic plasticity are consistently observed, linking molecular deficits to circuit-level dysfunction. Hyperactivation of the HPA axis and chronic hypercortisolemia further exacerbate hippocampal and prefrontal abnormalities, reinforcing maladaptive stress responses. Collectively, these findings support a systems-level model in which MDD arises from the interaction of dysfunctional neural networks, impaired neuroplasticity, and neuroendocrine imbalance. Advancing this integrative framework is critical for the development of targeted, mechanism-based therapeutic interventions. (shrink)

Parkinson’s disease is a common movement disorder associated primarily with oxidative stress-mediated degeneration of dopaminergic neurons. Earlier studies showed that Sorghum bicolor-based supplement (SbS) exhibited antioxidant and neuroprotective activities and might likely rescue the death of dopaminergic neurons in Parkinson’s disease. This study examined the effect of SbS on rotenone-induced Parkinsonian-like motor deficits in rats and the involvement of oxidative stress and pro-inflammatory cytokines. Rats were divided into six groups and treated orally with sunflower oil (vehicle-control), rotenone (2.5 mg/kg) alone (...) or in combination with each dose of SbS (50, 100, and 200 mg/kg) and levodopa-carbidopa (10 mg/kg) on an alternate day for 28 days. The changes in motor functions were evaluated on day 28 and the brain concentrations of oxidative stress biomarkers and pro-inflammatory cytokines (tumor necrosis factor-alpha and interleukin-6) were determined. Rotenone caused motor deficits by impaired locomotor activity in the open field test and induced catalepsy in the bar test, which were attenuated by SbS. Rats pretreated with SbS had reduced brain levels of malondialdehyde, nitrite, and pro-inflammatory cytokines compared to rotenone controls. SbS mitigated rotenone-induced depletion of reduced glutathione and antioxidant enzymes in the rat brain. The results suggest that SbS ameliorated rotenone-induced Parkinsonian-like motor dysfunctions by reducing neuronal oxidative stress and pro-inflammatory cytokines in rats. (shrink)

This chapter examines how different kinds of images have been deployed in disparate medical contexts to render long-term fatigue—an elusive and quintessentially intangible pathological physical and mental condition—visually communicable and epistemically explorable in the mid-twentieth and early ­ twenty-first centuries. First, by focusing on two disparate case studies, we intend to perform in-depth analyses of select context-specific uses of images in mediating the production and dissemination of biomedical knowledge of fatigue in different historical contexts. Second, through such an unusual juxtaposition (...) of two mutually independent case studies, each written by a separate author, we want to exemplify the heterogeneity of context-specific functions that diverse images have fulfilled in the medical discourse on fatigue. To achieve this twofold aim, we selected two case studies from different historical periods and medical contexts, characterized by different conceptual framings of fatigue and the use of different types of images, whose separate analyses necessitated employing two different methodological approaches. Abstract This paper explores the metaphor of politics as a game of basketball in which both players and referees repeatedly violate the rules, resulting in a descent into chaos comparable to mixed martial arts (MMA). The analogy serves as a framework to analyze how systemic corruption, weak institutions, and moral decay erode democratic governance. Drawing on real-world examples from multiple political systems, this paper argues that the violation of ethical and institutional rules in politics undermines public trust, polarizes societies, and (...) destabilizes governance. Abstract -/- Cannabis use is associated with anxiety, paranoia, and psychotic symptoms in a subset of individuals, yet the mechanisms responsible for this vulnerability remain incompletely understood. Existing explanations have largely emphasized cannabinoid receptor signaling, dopaminergic modulation, and large-scale changes in neural network dynamics. This paper advances the hypothesis that disruption of olfactory processing may represent an important and previously underrecognized contributor to these neuropsychiatric effects. -/- The central proposal is that impairment of olfactory function reduces the continuity and reliability (...) of environmental sensory information, increasing perceptual uncertainty and promoting greater reliance on internally generated interpretations. In susceptible individuals, this shift may increase the likelihood of anxiety, paranoid ideation, and related cognitive disturbances. The hypothesis generates specific, falsifiable predictions regarding the relationship between objective measures of olfactory performance and the severity of cannabis-associated psychiatric symptoms. -/- To facilitate independent evaluation, the accompanying article compiles and organizes the relevant biomedical literature concerning cannabis, olfactory function, and neuropsychiatric outcomes. By making this body of evidence readily accessible, the present work aims to provide both a clear statement of the hypothesis and a foundation for future experimental testing. Psychiatric conditions are commonly regarded as mental disorders or dysfunctions of the mind. Yet there is a wealth of historical theorizing about the mind that conceives of these conditions as, in some sense, a matter of design rather than dysfunction. This intellectual legacy is the topic of Justin Garson’s penetrating study, Madness: A Philosophical Exploration (2022). In this paper, I interpret Garson’s book as a genealogy (in the Foucauldian sense) of the “anti-tradition” that he labels “madness-as-design”. I argue that (...) viewing the intellectual legacy that Garson analyzes through this genealogical lens has two benefits. First, it encourages us to identify other instances of madness-as-design (or madness-by-design), particularly those with an overtly political dimension, such as psychiatric conditions in a colonial context. Second, it should lead us to question the category of madness itself, which turns out to be radically disjointed, particularly since it cannot be unified under the rubric of disorder or dysfunction. Artefacts do not always do what they are supposed to, due to a variety of reasons, including manufacturing problems, poor maintenance, and normal wear-and-tear. Since software is an artefact, it should be subject to malfunctioning in the same sense in which other artefacts can malfunction. Yet, whether software is on a par with other artefacts when it comes to malfunctioning crucially depends on the abstraction used in the analysis. We distinguish between “negative” and “positive” notions of malfunction. A negative malfunction, (...) or dysfunction, occurs when an artefact token either does not or cannot do what it is supposed to. A positive malfunction, or misfunction, occurs when an artefact token may do what is supposed to but, at least occasionally, it also yields some unintended and undesirable effects. We argue that software, understood as type, may misfunction in some limited sense, but cannot dysfunction. Accordingly, one should distinguish software from other technical artefacts, in view of their design that makes dysfunction impossible for the former, while possible for the latter. (shrink)

In this paper, I look into the debate about the status of addiction as a disease. Although addiction is widely regarded as a disease, several authors have put forward reasons for agnosticism or skepticism about the appropriateness of the disease label. Any attempt to address this issue directly is complicated by its relationship to several other contentious issues, both on the side of theories of addiction and on the side of theories of disease. My primary aim in this paper is (...) to identify the major points of contention. My secondary aim is to offer a limited defense of the disease view. The crux of the debate is whether addiction is aptly pictured as the result of psychological dysfunction. The main hurdle for the psychological dysfunction claim is that we currently lack a relatively unified account of the behavioral trait that most strongly suggests dysfunctional processes: the loss of expectable ability to control behavior. I argue that there is a sense in which the dysfunction claim may be warranted even in the absence of a unified causal account. However, this requires assumptions that are to some extent controversial, most notably, the impaired control view of addiction and the acceptability of dysfunction-talk based on personal-level behavioral traits. How do people understand the concepts of mental health and disorder? The objective of this paper is to examine the impact of several factors on people’s judgments about whether a condition constitutes a mental disorder or a healthy state. Specifically, this study examines the impact of the source of the condition, its outcome, individual valuation (i.e., the value the individual attaches to the condition), and group valuation (i.e., the value the relevant group attaches to the condition). While we find that (...) people’s health and disorder judgments are driven by perceived dysfunction, we also find that health and disorder judgments are impacted differently by these factors. Health judgements are impacted by outcome and individual valuation, and disorder judgments are impacted by condition source. These results suggest that the folk concept of mental health is positive (i.e., mental health is more than the absence of mental disorder) and normativist (i.e., value judgments play a significant role in determining whether a condition counts as healthy), while the concept of mental disorder aligns with a naturalist perspective, at least to the extent that dysfunction plays an important role in categorizing a condition as a disorder. However, our finding that people’s dysfunction judgments are influenced by individual valuation and outcomes poses a strong challenge to naturalist accounts. This paper calls on philosophers of medicine to attend to work on dispositions by metaphysicians. I make that call by drawing on recent work on dispositions to show several things. First, I show that the concept has been undertheorized by a wide range of theories of health and disease. Specifically, I discuss Boorse’s influential biostatistical theory of health, Werkhoven’s recent dispositional theory, and Aas and Wasserman’s neglected revision to Wakefield’s harmful dysfunction analysis. I also show how applying a careful (...) analysis of dispositions clarifies and strengthens each theory. But I show, lastly, that objections to each of these views are more easily grasped by virtue of this analysis. This discussion clarifies and contributes to ongoing debates over how to analyze health and disease. While the ethical significance of caregivers in neurological research has increasingly been recognized, the role of caregivers in brain- computer interface (BCI) research has received relatively less attention. Objectives: This report investigates the extent to which caregivers are mentioned in publications describing implantable BCI (iBCI) research for individuals with motor dysfunction, communication impairment, and blindness. Methods: The scoping review was conducted in June 2024 using the PubMed and Web of Science bibliographic databases. The articles were systematically searched using query (...) terms for caregivers, family members, and guardians, and the results were quantitatively and qualitatively analyzed. Results: Our search yielded 315 unique studies, 78 of which were included in this scoping review. Thirty-four (43.6%) of the 78 articles mentioned the study participant’s caregivers. We sorted these into 5 categories: Twenty-two (64.7%) of the 34 articles thanked caregivers in the acknowledgement section, 6 (17.6%) articles described the caregiver’s role with regard to the consent process, 12 (35.3%) described the caregiver’s role in the technical maintenance and upkeep of the BCI system or in other procedural aspects of the study, 9 (26.5%) discussed how the BCI enhanced participant communication and goal-directed behavior with the help of a caregiver, and 3 (8.8%) articles included general comments that did not fit into the other categories but still related to the importance of caregivers in the lives of the research participants. Discussion: Caregivers were mentioned in less than half of BCI studies in this review. The studies that offered more robust discussions of caregivers provide valuable insight into the integral role that caregivers play in supporting the study participants and the research process. Attention to the role of caregivers in successful BCI research studies can help guide the responsible development of future BCI study protocols. (shrink)

In this paper, we explore whether a living being can have a medical disorder purely in virtue of its current structure, or whether its historical origins are also relevant. We do so by presenting two dialogues based on the Swampman thought experiment. These dialogues bring out two key points. First, although evolutionary history plays a central role in theoretical accounts of disorder, its relevance is less obvious in everyday medical practice. Second, there may be an asymmetry between somatic and mental (...) disorders: somatic disorders are typically identified by biological dysfunction, while many paradigmatic mental disorders appear to involve violations of rationality. Nevertheless, we suggest that dysfunctions—and thereby historical considerations—still play a role in distinguishing normal from pathological irrationality. This dissertation aims to develop a program resource for helping and mental health professionals to foster proactive coping and diminish dysfunctional coping from work stressors. Professionals succumb to chronic stressors and secondary traumatic stress due to their vocation, often disregarding self-care. Should this type of resource be implemented, psychological and social resources would be required. The need for proactive coping is a generally accepted concept, but helping and mental health professionals often lack resources, limiting advocacy and resilience. Self-help resources are (...) frequently perceived as self-indulgence, in addition to the concept that professionals provide care, not the clients receiving care. Moreover, victimization by stigmas and emotional contagion surrounds helping and mental health professions. Addressing these concerns by implementing an accessible and advocate-heavy website may mitigate experienced ramifications by increasing retention, psychological well-being, resilience, and support. In this thesis, I offer a novel, overarching account of the cognitive architecture, evolution, and disorders of the capacity for low mood. First, I offer novel arguments for the propitiousness theory, according to which the proper function of the low mood system (LMS) is to limit resource expenditure in unpropitious circumstances. Following this, I develop an original account of the intentional content of low mood, according to which low mood has the following indicative-imperative content: Good events are, on average, less (...) likely to occur than bad events & Limit [the subject’s] resource expenditure! Next, I argue that the LMS takes input from numerous sensory, interoceptive, and affective systems, but that higher cognition can only effect low mood indirectly—namely, by triggering low-mood-inducing emotions. Finally, I argue that (a) disorders are necessarily constituted by dysfunctional systems (systems that fail to carry out their proper function), (b) most cases of depression (severe, long-lasting low moods) are activations of properly functioning LMSs, and therefore (c) most cases of depression are not in fact disorders. In view of the publication of the DSM-V researchers were asked to discuss the theoretical implications of the definition of mental disorders. The reasons for the use, in the DSM-III, of the term disorder instead of disease are considered. The analysis of these reasons clarifies the distinction between the general definition of disorder and its implicit, technical meaning which arises from concrete use in DSM disorders. The characteristics and limits of this technical meaning are discussed and contrasted to alternative definitions, (...) like Wakefield’s harmful/dysfunction analysis. It is shown that Wakefield’s analysis faces internal theoretical problems in addition to practical limits for its acceptance in the DSM-V. In particular, it is shown that: a) the term dysfunction is not purely factual but intrinsically normative/evaluative; b) it is difficult to clarify what dysfunctions are in the psychiatric context (the dysfunctional mechanism involved being unknown in most cases and the use of evolutionary theory being even more problematic); c) the use of conceptual analysis and commonsense intuition to define dysfunctions leaves unsatisfied empiricists; d) it is unlikely that the authors of the DSM-V will accept Wakefield’s suggestion to revise the diagnostic criteria of any single DSM disorder in accordance with his analysis, because this is an excessively extensive change and also because this would probably reduce DSM reliability. In conclusion, it is pointed up in which sense DSM mental disorders have to be conceived as constructs, and that this undermines the realistic search for a clear-cut demarcation criterion between what is disorder and what is not. Psychiatry classifies depression as a medical disorder graded by severity and treated, in the first instance, by pharmacological correction of a presumed neurochemical fault. This treatise argues that the received framework misidentifies the object it treats. Two theses are defended, and they are of different strengths. The constitutive thesis holds that depression is a philosophical problem necessarily and in every case: the concept is not a natural kind but a value-determination, since to classify a mode of being as disordered is (...) to measure it against a conception of the good and functional life. The etiological thesis holds that for a substantial and systematically under-recognized proportion of presentations, the proximate cause is not neurochemical dysfunction but a disorder of the philosophical self, understood as the developmental faculty of meaning-making, value-determination, and self-constitution. Four modes of philosophical dysregulation are distinguished: the unbuilt, the collapsed, the starved, and the overloaded philosophical self. Because philosophical and psychological dysregulation converge on an almost identical symptom profile, a nosology that admits only the psychological strand must misattribute the former to the latter, and it does. The argument engages the strongest objections, including the charge that reframing distress as philosophical will deter necessary care, and it concedes that a biologically loaded core of melancholic, psychotic, and bipolar presentations demands medical treatment as a matter of urgency. This autobiographical work explores the lived experience of AuDHD, dyslexia, and dyscalculia through a combination of personal narrative, philosophical reflection, and neurodivergent testimony. Moving between childhood, education, relationships, sensory experience, masking, rejection sensitivity, executive dysfunction, and identity formation, the manuscript examines how neurodivergent individuals navigate institutions, social expectations, and self understanding. Rather than presenting a therapeutic success story, the work argues for a re evaluation of difference itself, challenging deficit based models of cognition and proposing authenticity, self knowledge, and (...) acceptance as foundations for human flourishing. Situated at the intersection of disability studies, narrative identity, moral psychology, and autobiographical philosophy, the book offers both personal testimony and a broader reflection on what it means to live outside standard cognitive norms. The goal of this thesis is to propose a naturalistic definition of mental disorders. Naturalistic theories about disease generally attempt to establish what makes a biological phenomenon a disease by relying on the concept of biological function, stating that diseases and biological dysfunctions are equivalent. This implies the need to ground normative judgments on supposedly objective phenomena, which conflicts with descriptive scientific discourse and opens the gate to rebuttals on the supposed objective nature of diseases. However, at least theoretically, the (...) failure of the dysfunction definition of disease does not necessarily imply the failure of the naturalistic programme. The claim that the usual definition of disease as biological dysfunction is not the only possible way of defining disease in a naturalistic fashion will be defended. To back the previous claim, a definition of disease that does not depend on normative nor subjective criteria will be elaborated. It is argued that the 'negative' aspect of diseases should be understood in terms of a decrease in rate of change of the probability of living until a certain moment in time, and that diseases are processes related to negative impact on the viability of a biological organism. As will be shown, this proposal has potential as a solid naturalist alternative, with which disease naturalism can withstand arguments against the supposed subjective nature of dysfunction-based theories by resorting to this new proposal. Following this line of argumentation, empirical evidence about paradigmatic cases of mental disorders, such as schizophrenia, bipolarity and melancholy, can be invoked to argue that mental disorders can satisfy the criteria of being a disease by virtue of generating an increase in the probability of experiencing a negative impact on viability. Hence, this proposal shows promise in explaining the reason why mental disorders should be considered pathological. (shrink)

The post-truth era is as much about defining truth as a concept as it is about identifying which truth is the truth. On one hand, many still subscribe to the idea of a singular, objective reality that can be accessed with the right epistemological tools. On the other hand, a pluralistic approach that embraces equally valid realities, where the positivist lens is just one of many, is becoming an increasingly acceptable explanation for our current social configuration. Truth is not just (...) debatable; it is experiential — personal, even. Reality has reproduced itself into fractured, competing versions depending on the observer, and as a result, discourse has morphed into a shadow of its former constructive self. When faced with this postmodern iteration of meaning, we grasp for explanations as to why others seem so committed to “wrong” realities, and we typically reach for one of three frameworks: pathology, strategy, or machinery. And they’re not enough. Pathology explains epistemic deviance as individual dysfunction, caused by cognitive failures, from bad heuristics to poor education. This locates the problem in the person, bringing forth appropriate interventions like media literacy and inoculation against false beliefs. Strategy blames individual agency, where people rationally engage in disinformation for personal gain. This framing is carried through the proposed fixes, such as combating information interference and manipulation. Machinery sees the problem as a systemic product, where algorithmic influence and institutional authority are understood as creating the necessary conditions for a post-truth reality. This explanation calls for structural and regulatory solutions, including platform regulation and anti-trust action against technological monopolies. What if all three explanations miss something more fundamental? Many draw upon some or all of these frameworks to create comprehensive solutions to the multiple reality “problem,” which itself points to something more foundational that operates underneath these interpretations of the issue. What if people are motivated by something deeper: the existential need to maintain their very being by projecting meaning? And what if the emergence of competing realities isn’t a breakdown of rational discourse or a failure of information systems, but the inevitable result of human beings asserting their right to exist meaningfully in this world? This is the starting point for contestivism: a paradigm that views social reality as perpetual contestation of meaning through and for power, where the stakes are nothing less than being itself. Rather than treating competing worldviews as pathology to cure, strategy to counter, or machinery to reconstruct, contestivism recognizes them as expressions of the fundamental human drive to defend meaning as a condition for existence. The paradigm reveals the structural grammar underlying all meaning contests — including the very debates about truth and reality that define our current moment. It provides analytical tools for understanding how meaning emerges, stabilizes, and transforms without requiring us to adjudicate which meanings are “correct” or assign moral judgment to their proponents. Most importantly, it acknowledges that these contests aren’t peripheral to social life — they are social life. This paper develops contestivism as both a theoretical framework and an analytical lens, demonstrating how it can illuminate phenomena from disinformation to political polarization to social change. By understanding social reality as perpetual contestation rather than fixed truth or infinite relativism, we gain tools for understanding the post-truth era not as a symptom of an underlying problem, but as an inescapable condition of human society. Emotional acceptance is an important practice for recognizing anxiety, anger, appetite, the need for approval, jealousy, loneliness, procrastinatory impulses, and other internal reactions as observable experiences rather than denying or suppressing them. However, even after emotions and desires have been accepted, they may become sources of personality-level self-blame when they are identified with the self itself, as in judgments such as “I am weak,” “I am immature,” “I am ugly,” “I have no willpower,” or “I am lazy.” This paper examines (...) the transition from acceptance to self-identification and then to self-blame as a blind spot of emotional acceptance. The paper does not propose an independently validated psychotherapy. Instead, it presents the Deep Functional-Goal Observation Model as a conceptual framework for understanding and practically interrupting this transition. Within this model, emotions and desires are understood not as expressions of the essence of the self, but as outputs of a living system oriented toward continuation, regulation, protection, recovery, resource acquisition, affiliation maintenance, boundary defense, and relational continuity. This basic directionality is described metaphorically as “survival geometry,” while the local direction of protection, maintenance, avoidance, or acquisition expressed in a specific situation is termed a “deep functional goal.” The central distinction of the model is among the occurrence of an internal reaction, the possible function of that reaction, and the authorization of the behavioral candidate promoted by it. The occurrence of an emotion or desire is not in itself a personal or moral failure. At the same time, the fact that a reaction has an understandable function does not justify the action it encourages. Internal outputs should therefore not become objects of self-condemnation, whereas the behavioral candidates arising from them should be evaluated in relation to values, ethics, health, responsibility, trust, realistic conditions, long-term self-formation, and the dignity of others. Such candidates may then be adopted, limited, delayed, transformed, reduced, or refused. This process is called “system observation” or “system-metacognition.” It involves provisionally examining the conditions and possible functions of a reaction before converting it into a judgment about the self, while also creating a space for judgment between impulse and enactment. Its aim is neither to eliminate emotion nor to accept impulses unconditionally. Rather, it distinguishes causal understanding from normative judgment and seeks to combine a non-punitive attitude toward the self with concrete responsibility for action. The model is not intended to replace cognitive behavioral therapy, acceptance and commitment therapy, mindfulness, metacognitive therapy, compassion-focused therapy, emotion-focused therapy, evolutionary psychology, control theory, or reward research. Many of these approaches already address the functions and adaptive meanings of emotion, self-criticism, metacognitive distance, values-guided action, and emotional transformation. The model’s limited contribution lies not in the functional understanding of living systems itself, but in organizing that understanding into a single observational sequence that separates internal reactions from personality judgments, identifies behavioral candidates, and connects them to decisions about behavioral authorization. The model offers a conceptual framework applicable to everyday anxiety, anger, appetite, approval-seeking, jealousy, loneliness, procrastination, and previously learned protective reactions. However, deep functional goals are hypotheses rather than diagnoses, and observation must remain limited enough not to become rumination. In cases of severe psychological distress, cognitive decline, executive dysfunction, or crisis, internal judgment alone is insufficient, and professional support, environmental adjustment, and interpersonal assistance may be required. The notion of malfunction is critical to biological explanation. It provides a test-bed for the normative character of functional attribution. Theories of biological functioning must permit traits to operate but, at the same time, be judged as malfunctioning (in some naturalized, non-arbitrary sense). Whereas malfunctioning has attracted most attention and discussion in evolutionary etiological approaches, systemic and organizational ones have been less discussed. The most influential of the organizational approaches (by Saborido, Moreno and Mossio) takes a dual-order approach to malfunctions, (...) as a set of functions that fit first-order constitutive norms but fail to obey second-order regulatory ones. We argue that this conception is unnecessarily complicated (malfunctions do not need to arise as a result of two conflicting orders of norms) and too narrow (it excludes canonical cases of malfunctioning). We provide an alternative organizational account grounded on viability theory. The dynamics of the traits that constitute an organism define the normative field of its viability space: sugar must be replaced at certain rate, blood must be pumped at a certain pace, etc. A trait operates normatively when its effects on the viability space correlate positively with the normative field. Three senses of dysfunctionality might be distinguished: subfunctional operations are those that positively correlate with the normative field but quantitatively fail to match the required speed; malfunctional operations are those that do not positively correlate with the normative field; and, nonfunctional traits either don’t operate at all or operate with null effect on the normative field. When you submit something real to a system claiming to seek truth, the system's actual function becomes visible. I submitted a framework to the *Journal of Consciousness Studies* that places consciousness at the center of all empirical knowledge. The framework dissolves the philosophical problems the journal exists to study by showing they stem from one unfalsifiable assumption. The managing editor rejected it for being "too much philosophy" and told me to email celebrity philosophers instead. When pressed, he admitted he never (...) read the materials, citing time pressure for a business-card-sized argument. This reveals several absurdities: a consciousness journal rejecting consciousness-centered epistemology as off-topic, a managing editor handling submissions directly while claiming overwhelming volume, and deflection to unreachable celebrities presented as helpful advice. The system claims to seek paradigm-shifting work but cannot process it when it arrives. My first two submission attempts with this framework both received nonsense replies within days. This is not bad luck. By bringing them reality, I exposed fakery. This paper documents how academic publishing performs inquiry rather than conducting it, visible only when someone possesses a framework that should matter if true. (shrink)

Top-down sabotage isn’t just a theoretical issue, it poses a real danger to how organizations operate, especially when it’s the leaders themselves who instigate it. In Albania, where entrenched hierarchies persist and institutions still grapple with the legacy of transition, this problem becomes even more complex. Here, sabotage from above doesn’t have a single source. Political interference, fragile accountability systems, and a culture shaped by decades of centralized power all contribute. This study examines what truly causes leaders to undermine their (...) own organizations. It uses examples from Albanian universities, government agencies, and private sector companies. What emerges again and again? Leaders prioritizing personal interests, networks based on favoritism and loyalty instead of merit, and a deep-seated reluctance to embrace change. These behaviors don’t just slow progress, they permeate the entire system, making real development nearly impossible. The evidence is clear: top-down sabotage isn’t only about a single problematic leader. It’s built into the organizational structure, embedded in power imbalances and weak institutions. By mapping out these roots, this paper gets to the core of why organizations in Albania face such struggles and offers practical ways to counteract them. More transparency, broader participation in decisions, and stronger oversight of those at the top aren’t just slogans; they are essential first steps to breaking this cycle. (shrink)

The notion of mental disorder plays an important role in criminal law, particularly in relation to competency to stand trial, culpability, and entitlement to special treatment. However, how this concept should be understood in this context is rarely discussed explicitly in recent literature. To address this issue, the present paper explores which concept of mental disorder would be most suitable for criminal law. Using the explicationist methodology, the paper outlines several desiderata for an adequate concept. Among these, it identifies a (...) crucial desideratum labeled the “non-redundancy criterion”. This criterion holds that, to play a non-redundant role in criminal law, the concept of mental disorder must be defined independently of the capacities that underlie criminal responsibility. The paper evaluates several traditional accounts of mental disorder from the philosophy of psychiatry—including objectivist, value-laden, and hybrid views—and assesses which best satisfies this criterion while also fulfilling the broader roles that mental disorders serve in criminal law. The paper concludes that the objectivist understanding of mental disorders provides the most suitable explication for the purposes of criminal law. (shrink)

In his target article and recent interesting book about addiction and the brain, Marc Lewis claims that the prevalent medical view of addiction as a brain disease or a disorder, is mistaken. In this commentary we critically examine his arguments for this claim. We find these arguments to rest on some problematical and largely undefended assumptions about notions of disease, disorder and the demarcation between them and good health. Even if addiction does seem to differ from some typical brain diseases, (...) we believe contrary to Lewis, that there are still good reasons to maintain its classification as a mental or behavioral disorder. Since 2016, there has been an explosion of academic work and journalism that fixes its subject matter using the terms ‘fake news’ and ‘post-truth’. In this paper, I argue that this terminology is not up to scratch, and that academics and journalists ought to completely stop using the terms ‘fake news’ and ‘post-truth’. I set out three arguments for abandonment. First, that ‘fake news’ and ‘post-truth’ do not have stable public meanings, entailing that they are either nonsense, context-sensitive, or contested. (...) Secondly, that these terms are unnecessary, because we already have a rich vocabulary for thinking about epistemic dysfunction. Thirdly, I observe that ‘fake news’ and ‘post-truth’ have propagandistic uses, meaning that using them legitimates anti-democratic propaganda, and runs the risk of smuggling bad ideology into conversations. The philosophy of psychopathology faces a foundational problem: no existing account provides a principled, unified characterisation of what mental disorder is that is neither purely biological, purely normative, nor purely statistical, while remaining applicable across the full range of diagnostic categories and integrating the biological, psychological, and social levels at which mental disorder manifests. Wakefield’s harmful dysfunction analysis correctly identifies dysfunction as central but cannot ground the concept of natural function without circularity. The biopsychosocial model correctly identifies the (...) multi-level character of psychopathology but lacks a unifying structural principle. The DSM and ICD classification systems provide operationally useful symptom clusters but rest on no coherent ontological foundation. -/- This paper proposes that Constraint Theory (CT) — the thesis, established by transcendental argument, that constraint is the ontological primitive constitutive of determinate existence as such — provides that foundation. Mental health, on the CT account, is the condition of appropriate constraint calibration: the maintenance of constraint structures at cognitive, affective, behavioural, and interpersonal levels that are sufficiently determinate to constitute a coherent self and enable goal-directed action, yet sufficiently flexible to respond adaptively to novel constraint environments. Mental disorder is constraint dysregulation: systematic deviation from appropriate constraint calibration in the direction of either excess constraint (rigidity, repetition, foreclosed possibility) or constraint insufficiency (dissolution, disorganisation, loss of coherent selfhood). This account dissolves the demarcation problem by providing a structural criterion for dysfunction that does not depend on contested notions of natural function or statistical norms. It unifies apparently heterogeneous diagnostic categories under a single structural principle. It integrates biological and psychological levels through structural homology rather than reductive identity. It connects naturally to the predictive processing account of psychopathology, interpreting prediction-error dysregulation as a specific form of constraint propagation failure. And it provides a principled foundation for dimensional rather than categorical psychiatric nosology. The paper develops each of these contributions in detail and addresses the principal objections.
