Although it was translated into many languages, not many country-specific adaptations exist, and a formal validation procedure of the Russian version has been carried out only recently. The present contribution documents the different steps taken to formally translate and adapt a Russian-specific version of the AUDIT (RUS-AUDIT). Methods: The AUDIT was translated into Russian following an established protocol, revised and adapted to the country context using an expert panel, and field-tested in an iterative approach, in line with WHO rules on instrument translation and adaptation A total of three pilot phases were carried out on 134 patients from primary healthcare (PHC) and 33 patients from specialised alcohol treatment facilities (narcology), guided by a specially established advisory board. It includes the comparison of the summarized responses between the two national groups and the exploration of gender and age influence on the responses. The specific focus is made on differences between responses on different items within national groups. Among other things this will make it possible for us to demonstrate how even rather simple mathematical models of preindustrial political-demographic cycles could help us to account for a paradox that has been encountered recently by political anthropologists. In their Comment in The Lancet Healthy Longevity, Banerjee and colleagues proposed to exclude the... more In their Comment in The Lancet Healthy Longevity, Banerjee and colleagues proposed to exclude the code ‘Old age’ MG2A from the latest version of the International Classification of Diseases, ICD-11, claiming that equating old age to a disease could have the negative consequences of treating calendar age as a disease, raising concerns of ageism. Yet, in fact, the ‘Old age’ code is not a new ICD addition, or one that should raise special concerns of ageism. The synonymic designation of ‘Senility’ or ‘Old age’ was carried over from ICD-10 as it has been a rather technical designation that allowed to establish the cause of death, when it was difficult or impossible to establish other causes. In contrast to the earlier versions, the ICD-11 allows for diverse synonymic interpretations, including those that can be highly useful for a clinician treating older persons, such as ‘Ageing’, ‘Senescence’, ‘Senile state’, ‘Frailty’, and ‘Senile dysfunction’, which refer to a state of health but not the number in the passport. It is becoming increasingly clear that pathological processes of ageing are the major risk factors, and even major underlying causes of mortality and morbidity from non-communicable diseases, and, as we learn from the recent pandemics, also from infectious diseases. To address these ageing-related risk factors, there is a new wave of research and development that seeks to develop new therapies or repurpose older therapies, with the aim to slow and reverse the damage of ageing—ie, preventive, regenerative, and curative medicine. This research and development necessitates appropriate ICD coding. In fact, WHO addressed this issue by including a new extension code XT9T ‘Ageing-related’ in the section aetiology. This code describes ageing-related conditions and can be helpful for research and clinical development. However, there is still a need for coding both the pathological biological ageing process as a whole and its advanced stage as well. The new ICD-11 makes important steps toward that goal, as it provides a double focus for improving the health of older persons. First, by including the old age, senescence, and senile debility in the general symptoms category to target the state of ageing-related ill health, and second by including ‘Ageing-related’ code in the aetiology or causality category to target the pathogenic ageing processes. Thus, far from discriminating against the rights of older persons and fostering neglect for their curative or preventive health care, the ICD-11 codes for old age and ageing-related causality do exactly the opposite: they draw the public and professional attention to the specific health problems of older persons and call to action to improve the prevention and cures specifically for older persons. Thus, these designations are the very opposite of ageism. We advocate for a full therapeutic utilization of the new ICD-11 criteria for ageing-related ill health, by enhancing research and development of corresponding diagnostics and therapeutics to directly address the main risk factors for older persons—the degenerative ageing processes.

In 2009, in response to these challenges, the Russian government formed the Federal Service for Alcohol Market Regulation and published a national strategy concept to reduce alcohol abuse and alcohol-dependence at the population level for the period 2010–2020. Interviews to be conducted with experts to identify problems encountered in the use of existing Russian-language AUDIT versions. A pilot study using a revised translation of the Russian-language AUDIT that incorporates country-specific drinking patterns in the Russian Federation. Results and Conclusions The systematic review identified over 60 different Russian-language AUDIT versions without systematic validation studies. The main difficulties encountered with the use of the AUDIT in the Russian Federation were related to the lack of: A revised version of the Russian-language AUDIT was created based on the pilot studies, and was validated in primary healthcare facilities in all regions in 2019/2020.

The Russian government has since taken measures to reduce availability of non-beverage alcohol. Yet, some types of non-beverage alcohol still remain available for consumers. The aim of this study was to assess the availability and sources of non-beverage alcohol in Udmurtia. Methods A survey of adults on the streets of Izhevsk and its environs was performed on workdays to assess non-beverage drinking patterns in 2018. The questionnaire included questions about socio-demographic status and alcohol use, including non-beverage alcohol consumption and drinking patterns. Results One hundred and sixty-eight people were questioned, of whom, 28% reported consuming non-beverage alcohol. Per capita distilled spirits consumption is found to be the strongest determinant of the adult male mortality rate in Europe as soon as the unrecorded alcohol consumption is taken into account. It turns out to be much stronger than the other tested significant determinants such as per capita health expenditures, smoking prevalence, consumption of hard drugs and per capita consumption of vegetables and fruit. However, the relationship between the two variables is dynamic, and could only be adequately described by nonlinear dynamic models. Hence, we confront a rather paradoxical situation. On the one hand, we observe a millennial trend leading to the growth of both population density and warfare frequency. As a result, from a long-term perspective, we observe a very strong positive correlation between these two variables. But on the other hand, we also observe secular demographic-warfare cycles, which produce negative correlations both for individual cultures and for subsamples of cultures with similar levels of technological and/or political development.
