Clinical trials are fundamental to progress in oncology, as they provide the scientific framework for developing, evaluating, and refining cancer prevention, diagnosis, and treatment strategies. Well designed clinical trials are essential to ensure that cancer treatments are evidence-based rather than empirical, improving survival rates and quality of life for patients. Why Only a Few Patient Support Programs Succeed? In India, 50–60% of chronic patients stop therapy within 3 months (World Bank, 2022). That single metric explains why brand loyalty is so fragile. No matter how effective a molecule, doctors alone can’t ensure adherence. Patients need more than just samples and discounts . Some of the needs that may not get articulated are -Regular follow-up reminders -Nutrition + lifestyle guidance -Diagnostic access -Helplines that actually work We all know that whenever brands invest in patient support, adherence rises, outcomes improve, and doctors notice. But here’s the paradox - in a plethora of patient support programs, only a few truly succeed. In my consulting experience, the difference lies in whether programs are built with purpose. When the intent is simply to check a marketing box, programs fail. And when the intent is to truly improve the patient (and caregiver) experience, they succeed. Because the pitfalls are the mirror image of the success factors: Pitfall 1 : Box-ticking intent → Critical Success Factor (CSF) 1: Clear purpose Many PSPs exist to “engage” doctors. The successful ones start with a defined purpose: solve one real barrier in the patient journey — and solve it well. Pitfall 2 Generic activities → CSF 2 Human experience A bland SMS / WhatsApp reminder becomes white noise. A personalized call in the patient’s language — checking on side effects, offering diet tips — makes the patient feel supported. Support that feels human builds adherence. Support that feels generic builds indifference. Pitfall 3: Enrollment numbers → CSF 3: Impact feedback Doctors are often told how many patients were “enrolled.” What they value more is feedback on real impact: -How many stayed adherent for 6 months? -How many avoided hospitalizations? -How many caregivers felt less anxious? Doctors don’t just want numbers. They want to see the difference their patients felt. Pitfall 4: Ignoring caregivers → CSF 4 : Engaging caregivers In chronic illness, the caregiver is often the hidden decision-maker — reminding, motivating, even funding the treatment. Programs that empower caregivers with tools and knowledge are the ones that last. In Indian pharma, patient support isn’t about spend. It’s about investing with purpose — in patients, in caregivers, and in experiences that stick. That’s why only a few programs succeed, and when they do, they become true differentiators. I recently had the opportunity to speak at the 11th edition of PHARMACONNECT’26 in Mumbai. It was a great opportunity to reconnect with friends, colleagues, and business partners, and to exchange views on what is changing across the industry. Several speakers noted that post-pandemic, the focus has clearly shifted. Cost still matters, but availability, compliance, and quality come first. Long-term supplier partnerships and diversified sourcing models like China+1 are now standard practice. Digital transformation came up repeatedly. The move is from reactive firefighting to real-time visibility using AI, advanced planning tools, integrated ERP systems, and control towers. Forecasting is evolving from simple averages to AI-driven models that improve accuracy and agility. Cold chain and export logistics also remain critical. Route validation, temperature monitoring, and tighter control at handover points are essential. The cost of non-compliance is far higher than the cost of doing it right. Bridging clinical to commercial scale is now a mature model, but selecting the right CDMO and ensuring scalable processes is still key to success. Across all discussions, one theme stood out, the technology enables, but people execute. Strong governance, clear risk management, supplier discipline, and continuous upskilling are what will truly build future-ready pharma supply chains. So, the industry is clearly moving from fragmented operations to more integrated, transparent, and patient-focused ecosystems. 𝐁𝐮𝐭, 𝐚𝐫𝐞 𝐰𝐞 𝐫𝐞𝐚𝐥𝐥𝐲 𝐭𝐡𝐞𝐫𝐞 𝐲𝐞𝐭? Many leadership teams are still wrestling with some tough questions. 1.     What about geopolitical and regulatory uncertainty? Do we genuinely have visibility for the next two years? If we do, is this the right time to invest in new assets? If we don’t, how do we protect revenue and manage cash flow in the meantime? 2.     Are we still too dependent on certain geographies for suppliers and raw materials? If the answer is yes, what concrete steps are we taking to rebalance that risk? 3.     How accurate is our demand planning today? Can AI truly improve it? If yes, where do we begin? Who should we trust as a partner? And how much are we willing to invest before we see results? 4.     Compliance costs continue to rise. New guidelines keep coming. Are we just reacting every time something changes, or are we building systems that keep us audit-ready all the time? 5.     The same questions apply to data, warehousing, talent building, and capability ahead of the current requirements. We talk about AI as the solution. But do we truly believe our current data is strong enough to power it? Or are we hesitant because we are unsure of what sits inside our own systems? “Backward integration and having differentiated products was a key strategy, well thought out and being executed as planned, to accelerate our growth in the Finished Dosage segment . With oncology, high-volume, and peptide APIs in the pipeline, API and PFI revenues continue to grow, but finished dosages will always lead the way with a lot of captive consumption involved. We expect to perform much better starting later this year, once ongoing issue is resolved."
