The paper confirms that the treatment with a phosphodiesterase type 5 inhibitor (PDE5I), tadalafil, or an alpha-blocker, tamsulosin, in patients with voiding dysfunctions due to benign prostatic hyperplasia (BPH) may improve their erectile function and their urodynamic parameters. The paper also demonstrated that the treatment with both agents is better than the administration of a single agent alone. The use of tadalafil for the treatment of lower urinary tract symptoms (LUTS)/BPH has been recently reviewed by Gacci et al. {1}. Several clinical trials have extensively reported on the efficacy and safety of chronic treatment with PDE5Is – either alone or in combination with conventional therapies – in ameliorating LUTS in men with or without erectile dysfunction (ED) {2}.
