One of these active metabolites, M3G, causes opioid hyperalgesia.40 This rebound effect also increases more the side effects such as nausea and vomiting.40 For this reason, in our study, we preferred to administer the morphine through the subcutaneous route to the children for
pre-emptive analgesia purposes.
Pre-emptive analgesia in patients undergoing appendectomy.
To optimize pain management and outcome, there is a continuous search for new analgesics and alternative routes of delivery.7 Some experimental data have shown that preoperative pain treatment before surgical incision (
pre-emptive analgesia) could prevent spinal cord neuronal hyperexcitability secondary to peripheral stimulation, which is related to hyperalgesia.
Pre-emptive analgesia, which by reducing peripheral and central sensitization minimizes post-injury hypersensitivity (14).
was given as a
pre-emptive analgesia. After preparation, for aseptic surgery they were sedated with Xylazine @ 0.05mg/kg b.
The term "
Pre-emptive analgesia" implies that an analgesic treatment given before nociceptive stimuli reach the central nervous system could prevent or reduce the subsequent pain.4 Anaesthesiologist's main role is to enable the patient to undergo surgical and other painful procedures without pain or anxiety.
This is perhaps due to the fact that
pre-emptive analgesia (preoperatively administered rofecoxib) and multimodal analgesia techniques (postoperative paracetamol and rofecoxib) were used.
However a good
pre-emptive analgesia attenuates neurohumoral stress responses to a great extent.
These findings are consistent with a systematic review of randomized studies on
pre-emptive analgesia, demonstrating that early pre/ intra-operative intervention may not have long lasting analgesic effects into the postoperative period compared with post-incisional administration.17
The purpose of
pre-emptive analgesia is to prevent central sensitization of pain pathways that reduces the amount of analgesic requirements.
INTRODUCTION:
Pre-emptive analgesia, an evolving clinical concept, involves the introduction of an analgesic regimen before the onset of noxious stimuli, with the goal of preventing sensitization of nervous system to subsequent stimuli that could amplify pain.
