The $100 Twiddler (www.handykey.com), about the size of an electric razor, contains all the essential buttons (Alt, Control, Shift, NumLock and cursor controls) under your thumb and 12 other keys under your other fingers.

I set the numlock key off on reboot and was amazed at how easy it was, and how long I looked for that capability.
