Lilly said the Food and Drug Administration approved Truiicity on September 18 based on results from studies of the medication used alone or in combination with commonly prescribed diabetes medications, including metformin, pioglitazone,
glimepiride and insulin lispro.
Chemically
glimepiride (GLM) is (Figure 1(b)) 3-ethyl-4-methyl-N-[2-[4-[(4-methylcyclohexyl) carbamoylsulfamoyl]phenyl]ethyl]-5-oxo-2H-pyrrole-1-carboxamide.
The objective of this study was to compare safety and efficacy of sitagliptin with
glimepiride in patients with type 2 diabetes mellitus inadequately controlled with metformin monotherapy.
HOUSTON--Glipizide, glyburide, and
glimepiride were independently associated with 59%-68% greater all-cause mortality risks than metformin in a retrospective study of nearly 24,000 type 2 diabetic patients on monotherapy for control of blood sugar.
Researchers evaluated "data from nearly 24,000 patients with type 2 diabetes, average age 62, who were treated with one of three drugs called sulfonylureas (glipizide, glyburide, and
glimepiride) or with another type of drug called metformin."
The drugs, glipizide, glyburide, and
glimepiride, are known as sulfonylureas, which help decrease blood-sugar levels among type 2 diabetes patients by stimulating the pancreas to produce insulin.
The drug is used to control blood sugar and is sold alone or in combination with metformin (see this month's Drug Watch, p5) and
glimepiride. The new cancer warning will appear on the drug's labeling, and the FDA has advised doctors not to use Actos in patients with bladder cancer and to use it with caution in patients who have had bladder cancer.
As with exenatide and the DPP-4 inhibitors, once-daily liraglutide has been studied as either monotherapy or in combination with either metformin, a sulfonylurea (
glimepiride), metformin plus a TZD, or metformin plus a sulfonylurea in various randomized trials.
Treating diabetic patients with pioglitazone (Actos) helps prevent the progression of atherosclerosis, unlike the drug
glimepiride (Amaryl), according to a randomized trial reported in the April 2 issue of the Journal of the American Medical Association.
