I think the check and depends functions can be omitted if they don't do anything. See the module_depends and module_check functions of dracut-init.sh.

You should mention how to choose the directories where you give a choice: we prefer to put local sysadmin changes under /etc/; the /usr/ subtree is intended for files supplied by packages and managed by the system's package manage.
